Abstract

Purpose

The paper aims to give an introduction into symptom validity assessment (SVA) to non‐experts in the field of neuropsychology.

Design/methodology/approach

The analysis is based on the knowledge of the progress in this field which can be conceived as one of the most prolific developments in forensic neuropsychological assessment.

Findings

By its very nature, clinical neuropsychology is a data‐driven discipline, both on the level of single‐case assessment and in research. In forensic contexts where secondary gain is immanent, uncooperativeness and malingering may threaten the integrity of data, so much so that no valid conclusions may be drawn from the data.

Originality/value

In the last 20 years, clinical and forensic neuropsychologists have been more prominent in the development of methods to detect response distortions and to identify malingering than any other profession. In forensic contexts, neuropsychological evaluations without SVA must be considered incomplete. Clinically and forensically working psychologists should strive to have a thorough knowledge of symptom validity assessment.

Abstract

Purpose

The purpose of this paper is to determine the nature of the academic skills deficits in male offenders and their relation to neurocognitive deficits.

Design/methodology/approach

In total, 72 Finnish male prisoners were tested with regard to reading, spelling, and mathematical abilities.

Findings

Low academic skills, especially reading, were related to poor neurocognitive performance in verbal memory, visual memory, attention, and motor dexterity. The results showed a high number (29-36 percent) of reading and spelling disorders. In all, 15 percent of those with medium to severe problems in academic skills had marked difficulties in mathematics. In total, 88 percent of the participants with at least one problem area in literacy skills had neurocognitive deficits. In the present study, the pervasive neurocognitive deficits, occurring comorbidly with reading and spelling difficulties, seem to refer to a fundamental set of deficits which are only minimally explained by IQ, educational background or training.

Research limitations/implications

Reading and spelling difficulties could be seen as functional illiteracy which, combined with a broad spectrum of neuropsychological function deficits, pose a challenging task for rehabilitation. Only after proper identification of deficits has been achieved is it possible to set goals and select the appropriate means for rehabilitation. One obvious limitation is the moderate number of subjects (n=72).

Practical implications

It may not be enough just to train reading or develop literacy activities among prisoners; focussing intervention on comprehensive neurocognitive deficits is also necessary.

Originality/value

Correlates and comorbidity between academic difficulties and neurocognitive deficits among offenders, especially in arithmetic difficulties, have been less studied.

Abstract

Purpose

This study aims to evaluate the dementia assessment and diagnosis care provided to adults with intellectual disability. The authors selected recommendations from the National Institute for Health and Care Excellence (NICE) standards which could be evidenced in clinical notes and aimed to identify characteristics which may be associated with improved adherence to these recommendations.

Design/methodology/approach

The study population was adults with an intellectual disability who were diagnosed with dementia between January 2019 and December 2022 by a UK-based intellectual disability service. Data to demonstrate adherence to selected recommendations and demographic and clinical characteristics were extracted from electronic patient records.

Findings

The authors identified 41 individuals. A mean of six of the eight recommendations were adhered to. There was low adherence with structural imaging to support dementia subtype diagnosis (9 individuals, 22%). This may be linked with the low percentage of people diagnosed with vascular dementia (1 individual, 2%) despite a national figure of 20%. No demographic or clinical characteristics were associated with level of adherence recorded. The authors found incomplete recording of diagnostic clinical coding in electronic patient records. This may disadvantage this population, as they cannot be readily identified for post diagnostic support or resource allocation.

Originality/value

To the best of the authors’ knowledge, this is the first study to examine adherence to these NICE guidelines in this population.

Requests for tests and measuring instruments for use in class assignments and faculty and student research are both familiar and frustrating to most academic librarians. In typical scenarios, an education student wants to measure aggression in children or a nursing student needs a test for patient mobility. Even the faculty member who may know the name of a scale may not know its author or how to obtain a copy. All are looking for a measure applicable to a specific situation and each has come to the library in hopes of walking away with a copy of the measure that day. Those familiar with measurement literature know that accessing measures can be time consuming, circuitous, and sometimes impossible. The standard test reference books, such as the Mental Measurements Yearbook and Tests in Print (both of which are published by the Buros Institute, University of Nebraska, Lincoln, Nebraska), are of limited use. These books typically do not include actual instruments or noncommercial tests from the journal and report literature. While these standard reference books are essential to a test literature collection, sole use of them would mean bypassing large numbers of instruments developed and published only in articles, reports, papers, and dissertations. Sources are available to locate additional measurements, tests, and instruments, but they are widely dispersed in the print and electronic literature.

Abstract

It was suggested by Fidelman that Kantian ideas, namely, actual Infinity, the cosmos, a reason for the cosmos and consciousness, involve a cognitive conflict. This cognitive conflict is caused by a competition between the left‐ and right‐hemispheric cerebral mechanisms, and it causes us to feel that we encounter a paradox. It is suggested in this study that quantum particles too are Kantian ideas. According to Kant the logic of experience does not necessarily apply to Kantian ideas, which are not part of experience. Thus, Kant explained the paradoxes related to them. It is suggested that the cognitive paradoxes related to quantum particles are also related to a conflict between the hemispheric mechanisms.

Abstract

Finds a statistically significant relation between top‐down concept‐mapping and the right cerebral hemisphere, and between bottom‐up concept‐mapping and the right hemisphere. Correlates scores on concept‐mapping with scores on hemispheric tests, and compares the scores of the subjects on hemispheric tests with the preferable style of concept‐mapping. Concludes that top‐down concept‐mapping, the right hemisphere, and Frege’s logic are mutually related. Similarly, bottom‐up concept‐mapping, the left hemisphere, and Russell’s logic are mutually related.

Abstract

Purpose

A significant number of people held within the secure forensic hospital estate in this country suffer from neuropsychological deficits (e.g. Lowings, 2010; Williams et al., 2010). These deficits, many of them cognitive, have a detrimental impact on patient's abilities to benefit from psycho-educational materials thereby slowing the progress of their rehabilitation. Understanding these difficulties enables interventions to be more effectively targeted and facilitated to the patients’ cognitive strengths. It was against the backdrop of the evidence that a review was commissioned into neuropsychological activity at Rampton Hospital.

Design/methodology/approach

The review of the neuropsychological assessment activity was confined to the male patients residing within Mental Health Services (MHS) and the National High Secure Learning Disability Service (NHSLDS) of the hospital. With regards to the MHS, 129 patient files were examined in order to identify the volume of neuropsychological assessment previously undertaken and the make up of the neuropsychological test batteries administered with the current patient group. With regard to the NHSLDS 48 files were similarly examined. In addition, MHS psychologists were surveyed to assess their knowledge of patients within that service who to their knowledge had previously suffered an acquired brain injury (ABI) or had suffered a significant neurological illness likely to have resulted in cognitive impairment.

Findings

A scoping exercise identified that a significant amount of neuropsychological assessment was being undertaken, albeit in an unstructured way. This led to a number of patients being subjected to a significant delay before the need for neuropsychological testing and rehabilitation possibilities became apparent. There was a huge variance on when a patient was tested ranging from one month to 14 years and the size of the test battery used, with the number of tests conducted per patient ranging between one single test to 16 tests during a patients stay at the hospital.

Practical implications

Opportunities to more appropriately place patients based upon their abilities could be achieved if the hospital were to adopt a policy of conducting neuropsychological assessments, including cognitive functioning, for all patients upon admission to the hospital. A proportion of patients were known to have had a ABI or a significant illness likely to have resulted in cognitive impairment. This was considered to be an underrepresentation and the taking of a thorough history specifically covering ABI and specific illnesses known to have an impact on cognitive ability and behaviour was also recommended.

Originality/value

In light of this review, services are to take a more systematic approach to assessing neuropsychological difficulties. Consideration is being given to neuropsychological screening becoming part of the admission process.

Abstract

Purpose

This paper aims to examine the relationship between neuropsychological functioning as assessed following admission to a high secure psychiatric care (HSPC) hospital and subsequent time spent in long-term segregation (LTS). Evidence from forensic populations suggests neuropsychological functioning difficulties can increase vulnerability to interpersonal violence. However, the impact of this relationship on restrictive interventions used in these settings is poorly understood.

Design/methodology/approach

This study quantitatively examined the neuropsychological profiles of 80 male HSPC patients as assessed during routine admission assessments, comparing data against any subsequent LTS duration during a one-year period, and a non-LTS control (n = 27).

Findings

Analysis found individuals who were willing and able to complete a routine neuropsychological admissions assessment spent significantly less time in LTS than those unable to complete the assessment. Performance within a test of novel problem solving (Key Search Test) was significantly worse in the LTS group than controls. Performance within a visual memory task (Immediate Recall section of the Rey Complex Figure Test) significantly correlated with LTS duration. Additional findings suggest the absence of self-reported planning difficulties as measured by a Dysexecutive Questionnaire (DEX) negatively correlated with LTS duration, while self-reported restlessness was positively correlated with LTS.

Practical implications

This has implications for early assessment of LTS risk and potential use of cognitive interventions to reduce the use of restrictive practices.

Originality/value

The results suggest some aspects of neuropsychological performance as assessed during admission to a HSPC hospital appear to be related to subsequent time spent in LTS.

Abstract

Purpose

The purpose of this paper is to discover if there is a correlation between scores on the Beck’s Depression Inventory (BDI) and the Cognitive and Physical Functioning Questionnaire (CPFQ) scores of 43 patients with major depression.

Design/methodology/approach

In total, 43 adult patients with major depression were evaluated during their regularly scheduled outpatient appointment in a mental health clinic.

Findings

There was an R2 value of 0.6544 between the patients’ scores, a moderate-to-strong correlation which matches other observations that cognitive impairment increases in conjunction with severity of depression. This correlation lends further clinical support to the legitimacy of using the CPFQ as a simpler alternative to traditional neuropsychological testing, with further testing of the correlation between CPFQ and traditional neuropsychological testing results being a worthwhile potential field of study.

Originality/value

Cognitive dysfunction is a frequent comorbidity in patients with depression, but while there is a brief and effective self- assessment for depression, the BDI, in common use, there is no equivalent test for cognitive dysfunction, and physicians are forced to rely on less accessible methods of neuropsychological testing.

Design/methodology/approach

The paper employs a logical analysis approach.

Findings

There are three characteristics of neuropsychological studies: the particularity of the study subjects; the specialty of the study scheme; and insufficient data from traditional statistical methods. Grey system theory is appropriate for analyzing the data collected in neuropsychological studies.

Originality/value

After several years' significant development, grey system theory has been applied in various subjects successfully. However, the application in psychology and medicine is still a rarity.
