Abstract
The Australian National Standards for Mental Health Services (Commonwealth Department of Health and Family Services, 1996) were developed as a plank of the first National Mental Health Plan (Commonwealth Department of Human Services and Health, 1992). Over the two subsequent national five‐year plans, they have become the basis for accreditation surveys for all Australian mental health services, both hospital and community components, whether acute or rehabilitation oriented, throughout the psychiatric career of all mental health service users and their families. The development and implementation of these standards are described. Innovations in this set of standards are detailed, specifying requirements of each phase of care, including access, entry, exit and re‐entry, and the parallel development and training of paid consumer and family carer surveyors. Largely due to the brevity and clarity of these innovations, because of a broad consultation process, and incorporation of interventions and service delivery systems that are both evidence‐based and congenial to service users, they have achieved a wide acceptance among, and championing by, service user and family carer networks. A recent review of the national standards was timely and welcomed, but is still incomplete, contentious in its protracted process, has a lack of consistent consultation and contains diluted and disorganised results. Implementation guides will now be developed to be superimposed on this revision in an attempt to improve and navigate it.

In prior articles in both volume 8 (number 4) and volume 10 (numbers 3/4) of Collection Building, bibliographies of U.S. government publications on AIDS were covered. The first bibliography covered both executive branch and legislative branch materials from 1981 to September 1986. The second bibliography covered only legis‐lative materials from 1986 to 1989. This article complements the second bibliography in its coverage of executive branch materials from 1986 to 1989 and also updates the first work. While 1986 to 1989 is the framework, some items inadvertently omitted from the earlier work are included here.

Abstract
English governance has repeatedly had a tendency to veer between national, regional and local centres of power and influence. This has often led to profound disagreements, sometimes even open conflict. National policy guidance is usually helpful, if developed through consultation, to steer a clear, coherent direction for the system. But a narrow, excessively top‐down, mechanistic target‐driven approach can lead to a prevailing culture of ticking boxes at the expense of real patient priorities. Government ministers and civil servants, however, are often caught in a tension between being too dogmatic, or alternatively too flexible and giving responsibility to local agencies, whereupon people may complain about a ‘postcode lottery’ in services. Balancing perspectives and narratives in a coherent way for policy development and implementation and service improvement is a major challenge of leadership. The creation of the National Institute for Mental Health in England (NIMHE) was designed to bring together the local, regional and the national in a form that would see policy and practice mutually developed and nurtured at all levels of governance.

Abstract
Purpose
Mental health reform needs to engage leaders across jurisdictions and governments/communities. This paper aims to describe the history leading to the establishment of the Australian National Mental Commission (the Commission) in 2012 and to identify opportunities/challenges in cross jurisdictional/sectoral reform of mental health services.
Design/methodology/approach
This case study is based on the observations of the author who has held lead state and national roles prior to leading the Commission. It sets the establishment of the Commission in the broader context of changing community attitudes, strong consumer/carer advocacy, increased financial support for mental health and establishment of state commissions.
Findings
Cross sectoral/jurisdictional reform needs leadership from government heads and central agencies. Risks relate to the need to effectively articulate state and national commissions and challenges accompanying multilateral stakeholders with high and often conflicting expectations, and tensions with other high priority issues that also require alignment between Federal and State leaders and agencies.
Research limitations/implications
There may be potential bias based on the perspective of the narrator, and the benefits of direct experience in multiple jurisdictions.
Practical implications
The paper identifies critical success factors in the establishment/operation of the first national Commission and the establishment of relationships with state commissions, relevant to other cross jurisdictional bodies.
Originality/value
The paper provides an insight into the factors leading to the establishment of the first national Commission and its articulation with state commissions and the early operational challenges facing cross jurisdictional reforms in changing political climates.

Abstract
The European Union‐supported Child and Adolescent Mental Health in an Enlarged Europe (CAMHEE) project aimed to provide an overview of the challenges, current practice and guidelines for developing effective mental health promotion and mental illness prevention policy and practice across Europe. As part of this work, an analysis was undertaken of the situation in England, making use of a bespoke data collection instrument and protocol.Our analysis suggests that there has been significant effort and investment in research, needs assessment, policy, human resource and service developments in CAMHS over the last 20 years, leading to a more detailed understanding and availability of services. Much of the emphasis has been on assessment and management of difficulties, however in recent years attention has begun to focus on mental health promotion. National standards and programmes such as Every Child Matters (Department for Education and Skills, 2004) have acted as catalysts for a number of national initiatives.

A distinction must be drawn between a dismissal on the one hand, and on the other a repudiation of a contract of employment as a result of a breach of a fundamental term of that contract. When such a repudiation has been accepted by the innocent party then a termination of employment takes place. Such termination does not constitute dismissal (see London v. James Laidlaw & Sons Ltd (1974) IRLR 136 and Gannon v. J. C. Firth (1976) IRLR 415 EAT).

Health‐related publications represent United States government publishing at its finest. But for a few major exceptions such as Index Medicus, many of these sources tend to be overlooked when ordering for the nursing reference collection, or lie buried in the Government Publications Depository collection, unknown and unused. Not only is the U.S. government the premier source for health statistics and indexes to the medical periodical literature, its agencies have also produced extensive bibliographies on health‐related topics and comprehensive treatises on disease. While prices of government publications have been on the increase in recent months and will undoubtedly continue to rise, in most cases they still compare favorably with the prices of trade publications.

Since 1948 the Health Service has been a subject of policy on which Labour has consistently rated higher than the Tories in public opinion. But the standard of care within the National Health Service is seen as falling. What difference will the new plan make? There are undoubted good intentions, but also the potential for confusion as commissioning and service delivery are put in the structural mixer once again.
