Abstract
Purpose
Complex organizations increasingly rely on middle managers as strategic linking-pins between the top and bottom levels of the organization. Using social identity theory and commitment theory as the foundation, this study evaluates a management and leadership development program (MLDP) intended to engage middle managers as strategy creators and implementers. We also evaluate the cascading effects of leadership development by assessing changes in subordinates' identification with the leader, and commitment to the work unit and organization.
Design/methodology/approach
Using a sample of 107 manager participants and 913 of their subordinates, this study measures differences in both manager and subordinate identification and commitment prior to and after the completion of a 6 months strategically oriented MLDP.
Findings
Despite the organizations' best intentions, manager identification with and commitment to the organization decreased after completion of the MLDP. Similarly, subordinates identification with the leader and commitment to the organization also decreased at Time 2.
Research limitations/implications
The results paint a complex picture of the nuances of social identification as an outcome of MLDPs, and problematize the notion of cascading effects on subordinates within the organization. Researchers are encouraged to further examine organizational attitudes and perceptions as outcomes of MLDPs.
Practical implications
Suggestions are offered regarding how practitioners can manage strategically oriented MLDPs in order to avoid identity confusion and promote strategic action.
Originality/value
Strategically oriented MLDPs are increasingly popular in organizations. This study is one of the first to evaluate the theoretical mechanisms through which these programs may affect managers and problematize these effects for complex organizations.

Abstract
Purpose
Since the emergence of new public health in the 1970s, health has not merely been considered the absence of disease, but physical, mental and social wellbeing. This article seeks to analyzes the implications of this broad concept of health at an organizational level.
Design/methodology/approach
The paper presents a qualitative case study of boundary drawing in a Danish municipal agency in charge of planning and conducting health promoting and disease preventing activities from 1989 to 2005. The theoretical framework draws on Niklas Luhmann's organization theory.
Findings
Two different organizational answers were found to the challenges inherent in the broad concept of new public health. First, the organization tried to increase its size and incorporate as many aspects of the environment as possible. This expansive strategy jeopardised the identity of the organization. Second, the organization tried to keep clear and tight boundaries and from this position irritate entities in the environment. This limitative strategy made the organization spend relatively more energy on organizing and controlling itself than on public health work.
Practical implications
The case study shows how a broad concept of health makes boundary management topical in organizations dealing with health promotion and disease prevention. Organizations in charge of public health activities need to reflect on how they can create intelligent compensations for the disadvantages involved in an expansive or a limitative strategy.
Originality/value
The broad concept of health inherent in new public health has been widely accepted and yet its challenges to organizational boundary drawing have attracted little attention. This paper provides an analysis of these challenges.

This case can be used in management of international business courses to illustrate the analysis of market attractiveness, the importance of fit between firm capabilities and market requirements, and the effects of multimarket competition. It describes the international expansion challenges facing EAC Nutrition, the infant formula division of a Danish conglomerate, in early 2002. Growth in EAC's core markets of Thailand and Malaysia has stagnated and EAC is contemplating three expansion options: entry into India, geographic expansion within China, and product line expansion in existing markets.

Abstract
Purpose
The purpose of this paper is to introduce the IJPSM special issue on marketization to clarify the conceptual foundations of marketization as a phenomenon within the public sector and gauge current marketization trends based on the special issue’s seven papers.
Design/methodology/approach
Conceptual clarification and cross-cutting review of seven papers analysing marketization in six countries in three policy areas at the level of local government.
Findings
Four ideal-types models are deduced: quasi-markets involving both provider competition and free choice for users; classical contracting out; benchmarking and yardstick competition; and public-private collaboration. Based on the review of the seven papers, it is found that all elements in all marketization models are firmly embedded but also under dynamic change within public service delivery systems. The review also identifies limitations and modifications of the four ideal-type models. A key trend is a move towards public-private collaboration and cross-sectorial and inter-organizational governance arrangements.
Research limitations/implications
Continued research on marketization would benefit from development of more fine-tuned theoretical models which are sensitive to the realm of the dynamics within particular policy and institutional contexts.
Practical implications
Policy-makers should balance normative objectives against the experiences gained at the level of implementation.
Originality/value
The special issue shows that marketization still is a concurrent phenomenon which is driving substantial change in public service delivery systems as well as is under dynamic change itself.

Abstract
Purpose
In organizational health promotion research, health promotion capacity is a central concept that is used to describe the abilities of individuals, organizations, and communities to promote health. The purpose of this paper is to discuss the theoretical underpinnings of the literature on health promotion capacity building and, further, to suggest an alternative theoretical perspective which draws on recent developments in organizational theory.
Design/methodology/approach
The paper begins by a critical discussion of the capacity building literature, which is juxtaposed with the relational perspective of contemporary organizational theory. The theoretical argument is developed in reference to the case of Danish municipal health promotion agencies, drawing on secondary sources as well as ethnographic fieldwork among public health officers.
Findings
The capacity building literature tends to reify the concept of capacity. In contrast, this paper argues that health promotion capacity is constantly defined and redefined through processes of organizing. The case study suggests that, faced with limited resources and limited knowledge, health promotion officials attain a sense of capacity through an ongoing reworking of organizational forms.
Research limitations/implications
Organizational health promotion research should look for the organizational forms that are conducive to health promotion practices under shifting social circumstances.
Originality/value
This paper makes explicit an inherent theoretical tension in the capacity building literature and suggests a novel theoretical framework for understanding organizational capacity.

Purpose
To examine the availability and quality of clinical guidelines on perioperative diabetes care in hospital units before and after a randomised clinical trial (RCT) and international accreditation.
Design/methodology/approach
Interventional “before‐after” study in 51 units (38 surgical and 13 anaesthetic) in nine hospitals participating in a RCT in the greater Copenhagen area; 27 of the units also underwent international accreditation.
Findings
The proportion of units with guidelines increased from 24/51 (47 percent) units before to 38/51 (75 percent) units after the trial. Among the 27 units without guidelines before the trial, significantly more accredited units compared to non‐accredited units had a guideline after the trial (9/10 (90 percent) compared to 5/17 (29 percent). The quality of the systematic development scale and the clinical scales improved significantly after the trial in both accredited units (both p<0.001) and in non‐accredited units (both p<0.02). The improvement of the systematic development scale was significantly higher in accredited than in non‐accredited units (p<0.01).
Originality/value
The combination of conducting both the DIPOM Trial and international accreditation led to a significant improvement of both dissemination and quality of guidelines on perioperative diabetic care.

Abstract
Purpose
The purpose of this paper is to investigate and report the impact of glass frit variation in silver thick film pastes used as surface conductors in low temperature co‐fired ceramics technology (LTCC), especially on the properties such as warpage of LTCC associated with conductors, microstructure of the fired thick films, sheet resistance and adhesion on LTCC.
Design/methodology/approach
Silver thick film paste compositions were formulated by changing the silver glass frit ratio. The compatibility of these formulated paste compositions with LTCC (DP 951AX) substrate were evaluated. The properties such as microstructure developments, the change in sheet resistance, warpage of LTCC substrate with respect to glass frit ratio of the developed silver films on LTCC were evaluated.
Findings
The results reveal that the glass frit percentage used in paste formulation is equally responsible for the disturbance in the properties such as microstructure, warping and electrical properties of the fired thick films on LTCC. It was observed that the paste composition, in particular sample SP10B containing the highest glass frit percentage, is compatible with the LTCC tape under processing conditions. The sheet resistance value in the range of 5 mΩ/□ and the fired films showed very good adhesion (3.95 N), irrespective of the glass frit composition.
Originality/value
The paper provides useful evaluations of properties such as microstructure developments, changes in sheet resistance and warpage of LTCC substrate with respect to glass frit ratio of the developed silver pastes on LTCC.
