Abstract

Purpose

This study aims to facilitate access to vascular disease screening for low-income individuals living in remote and conflict areas based on the results of a pilot trial in Colombia. Also, to increase the amount of diagnosis training of vascular surgery (VS) in civilians.

Design/methodology/approach

The operation method includes five stages: strategy development and adjustment; translation of the strategy into a real-world setting; operation logistics planning; strategy analysis and adoption. The operation plan worked efficiently in this study’s sample. It demonstrated high sensibility, efficiency and safety in a real-world setting.

Findings

The authors developed and implemented a flow model operating plan for screening vascular pathologies in low-income patients pro bono without proper access to vascular health care. A total of 140 patients from rural areas in Colombia were recruited to a controlled screening session where they underwent serial noninvasive ultrasound assessments conducted by health professionals of different training stages in VS.

Research limitations/implications

The plan was designed to be implemented in remote, conflict areas with limited access to VS care. Vascular injuries are critically important and common among civilians and military forces in regions with active armed conflicts. As this strategy can be modified and adapted to different medical specialties and geographic areas, the authors recommend checking the related legislation and legal aspects of the intended areas where we will implement this tool.

Practical implications

Different sub-specialties can implement the described method to be translated into significant areas of medicine, as the authors can adjust the deployment and execution for the assessment in peripheral areas, conflict zones and other public health crises that require a faster response. This is necessary, as the amount of training to which VS trainees are exposed is low. A simulated exercise offers a novel opportunity to enhance their current diagnostic skills using ultrasound in a controlled environment.

Social implications

Evaluating and assessing patients with limited access to vascular medicine and other specialties can decrease the burden of vascular disease and related complications and increase the number of treatments available for remote communities.

Originality/value

It is essential to assess the most significant number of patients and treat them according to their triage designation. This management is similar to assessment in remote areas without access to a proper VS consult. The authors were able to determine, classify and redirect to therapeutic interventions the patients with positive findings in remote areas with a fast deployment methodology in VS.

Plain language summary

Access to health care is limited due to multiple barriers and the assessment and response, especially in peripheral areas that require a highly skilled team of medical professionals and related equipment. The authors tested a novel mobile assessment tool for remote and conflict areas in a rural zone of Colombia.

Abstract

Purpose

To examine how art is shaped by war, outside of the official commemorative projects of the First World War. The purpose of this paper is to examine the experience of a surgeon/artist who knew first-hand the horror of industrial scale of destruction. It speculates on how his medical education and surgical knowledge in the treatment of the casualties informed his art and considers how such scientific discourses may have contributed to a new modernist language.

Design/methodology/approach

The double career of J.W. Power – a surgeon then an artist – provides a case study to probe such questions. The paper speculates about the connections between these different careers, and considers the implications of becoming an artist for someone who had pre-war university-training, medical expertise and experience as a war surgeon. In particular, consideration is given to how surgical knowledge and contemporary medical debates may have informed a group of later paintings.

Findings

A group of J.W. Power’s late paintings stand apart from his other subjects as they suggest states of physical or psychological damage. Indeed by the 1930s shell shock was recognised as a war-related psychological injury. These paintings then may not only be an act of remembrance, but also potentially a reflection on that new discourse.

Research limitations/implications

It remains a compelling idea that by the 1930s Power had found a modern abstract language capable of revisiting the traumatic subject of his hospital sketches. The implications of the war-time surgery on his art was delayed and remains highly ambiguous, however it invites, indeed encourages, such speculation.

Originality/value

The paper is the first to examine the cultural impact of the medical career of the artist J.W. Power. His medical training and experience as a war-time surgeon is shown to have been significant to his later painting, for he knew the regenerative powers of modern surgery, of how such knowledge had the power to repair and to heal.

Purpose

The purpose of this paper is to detect wound infection by electronic nose (Enose) and to improve the performance of Enose.

Design/methodology/approach

Mice are used as experimental subjects. Orthogonal signal correction (OSC) is applied to preprocess the response of Enose. Radical basis function (RBF) network is used for discrimination, and the parameters in RBF are optimized by particle swarm optimization.

Findings

OSC is very suitable for eliminating interference and improving the performance of Enose in wound infection detection.

Research limitations/implications

Further research is required to sample wound infection dataset of human beings and to demonstrate that the Enose with proper algorithms can be used to detect wound infection.

Practical implications

In this paper, Enose is used to detect wound infection, and OSC is used to improve the performance of the Enose. This widens the application area of Enose and OSC.

Originality/value

The innovative concept paves the way for the application of Enose.

Abstract

Purpose

The purpose of the paper is to propose a new optimization algorithm to realize a synchronous optimization of sensor array and classifier, to improve the performance of E-nose in the detection of wound infection. When an electronic nose (E-nose) is used to detect the wound infection, sensor array’s optimization and parameters’ setting of classifier have a strong impact on the classification accuracy.

Design/methodology/approach

An enhanced quantum-behaved particle swarm optimization based on genetic algorithm, genetic quantum-behaved particle swarm optimization (G-QPSO), is proposed to realize a synchronous optimization of sensor array and classifier. The importance-factor (I-F) method is used to weight the sensors of E-nose by its degree of importance in classification. Both radical basis function network and support vector machine are used for classification.

Findings

The classification accuracy of E-nose is the highest when the weighting coefficients of the I-F method and classifier’s parameters are optimized by G-QPSO. All results make it clear that the proposed method is an ideal optimization method of E-nose in the detection of wound infection.

Research limitations/implications

To make the proposed optimization method more effective, the key point of further research is to enhance the classifier of E-nose.

Practical implications

In this paper, E-nose is used to distinguish the class of wound infection; meanwhile, G-QPSO is used to realize a synchronous optimization of sensor array and classifier of E-nose. These are all important for E-nose to realize its clinical application in wound monitoring.

Originality/value

The innovative concept improves the performance of E-nose in wound monitoring and paves the way for the clinical detection of E-nose.

Purpose

Previous research suggests that many active females are not engaging in sports bra use, despite the positive health benefits. The aim of this study was to establish and compare sports bra use, preferences and bra fit issues for exercising females in some of the largest and most diverse global underwear markets (the US, the UK and China).

Design/methodology/approach

A survey covering activity levels, sports bra use and preferences, bra issues and demographics was administered via Qualtrics and completed by 3,147 physically active females (aged ≥ 18 years) from the US (n = 1,060), UK (n = 1,050) and China (n = 1,037).

Findings

In general, participants were 25–29 years, 121 to 140 pounds, 34B bra size and pre-menopausal. “I cannot find the right sports bra” was the most frequent breast barrier to exercise (25.4%). Three-quarters of women wore a sports bra during exercise, with significantly higher use in China (83.9%), compared to the UK (67.2%). A third of all participants reported sports bra shoulder straps “digging into the skin”. Sports bra preferences were: compression sports bras with a racer back, wide straps and thick straps in the US and the UK; thin straps in China and adjustable straps and underband, no wire and maximum breast coverage in the US and the UK, including nipple concealment and with padded/moulded cups.

Originality/value

Information provided on differences in sports bra use, preferences and bra issues across three major global markets could be utilised by brands and manufacturers to optimise bra marketing and fit education initiatives and inform future sports bra design and distribution strategies.

Purpose

Globally, private and public organisations invest ever increasing amounts of money, time and effort to develop leadership capabilities in current and future leaders. Whilst such investment results in benefits for some, the full value of developmental strategies on offer is not always realised. Challenges inhibiting achievement of full value include struggling to identify learning programs that best fit with the organisational structure, culture, mission and vision and difficulties in maximising engagement of personnel at multiple levels of the management structure.

Design/methodology/approach

The purpose of this study is to introduce a pathway for health services to develop and embed simulation-based educational strategies that provide targeted learning for leaders and teams. Aligning this approach to leadership development through presentation of case studies in which the model has been applied illustrates the pathway for application in the health-care sector.

Findings

The findings of the approach to leadership development are presented through the presentation of a case study illustrating application of the ADELIS model to simulation-based learning.

Practical implications

The ADELIS model, outlined in this study, provides a guide for creating customised and flexible learning designs that apply simulation-based learning, enabling organisations to develop and provide leadership training for individuals, units and teams that is appropriately fit for purpose.

Originality/value

The key contribution to health-care leadership development offered in this study is the rationale for using simulation-based learning accompanied by a model and pathway for creating such a pedagogical approach, which embraces the reality of workplace circumstances.

THE beginning of a new volume is necessarily a time for reflection. Our journal is now forty‐four years old and has appeared without intermission, always with the purpose, enunciated by its founder, James Duff Brown—to furnish librarians of all kinds and ages with a thought‐exchange and a medium of expression independent of any other control than the editor's conviction that what was published was sincere in intention and likely to be of use to the profession. This does not mean, as our pages to‐day witness, that matters of controversy or even of severe criticism of those who lead the profession officially are excluded. On the contrary, we believe that the best spur to advance is a critical vigilance. Thus it has occurred occasionally that our writers have been at variance with some current policy of the Library Association, some phases of its examinations or its conference policy. Occasionally, too, there have been criticisms of library authorities which an official journal might hesitate to make because those authorities may be in membership of the Library Association. Such criticism was never more necessary than now. The library movement has to be kept alive under the greatest strain in history; indeed, it should progress.

Purpose

For more than 25 years, the National History Day (NHD) program has a number of distinctive features that make it a uniquely powerful collaborative vehicle for information literacy instruction. By requiring that student participants do in‐depth research using primary source materials, NHD strongly encourages integrated learning of historical content and information‐seeking processes, and thus partnerships between history teachers and librarians. Because few middle and high schools have extensive collections of primary source materials, NHD also promotes partnerships between K‐12 schools and academic libraries, public libraries, and museums in making primary source materials available to students. This case study aims to draw on one academic library's three‐year experience of partnering in NHD events in its community.

Design/methodology/approach

Describes the NHD program, highlighting the commonalities between NHD learning goals; the National Standards for History: Historical Thinking Standards (Grades 5‐12); the American Association of School Libraries' Information Literacy Standards for Student Learning; and the Association of College and Research Libraries' Information Literacy Competency Standards for Higher Education.

Findings

Shows how one academic library was able to successfully implement NHD programming, especially in the area of library instruction, to engage students in their own learning.

Originality/value

NHD participation by higher education librarians, collaborating with their K‐12 counterparts, can be a powerful learning vehicle for elementary and secondary students to learn historical content knowledge, historical thinking skills, and information literacy skills.

Abstract

Purpose

This is the second part of a two-part paper which aims to provide an insight into the ethical and legal issues surrounding certain classes of robot. This part is concerned with law.

Design/methodology/approach

Following an introduction, this paper first describes the European RoboLaw project and then considers legal issues and activities relating to civilian airborne drones, driverless road vehicles and assistive robots. It concludes with a short discussion.

Findings

The legal issues associated with many classes of robot are the topic of much debate, and efforts are underway to create appropriate legislative frameworks. A project is presently seeking to create a framework for the development of a Europe-wide “Robolaw” and in certain cases, laws are already being formulated to accommodate recent robotic developments. These deliberations are rapidly gaining pace and are now also considering future generations of highly autonomous and intelligent robots.

Originality/value

This paper provides an insight into the highly topical and complex issue of robot law.
