Abstract

CSF Buxmont Academy operates eight school/day treatment programs that use restorative practices, which includes a culture in which restorative characterizes staff interaction with students, and staff-to-staff and student-to-student relationships as well. This chapter presents analyses of the outcome experiences from two waves of discharge cohorts: 919 students during school years 1999–2000 and 2000–2001 and 858 during 2001–2002 and 2002–2003. Outcome measures include program completion rates, changes in self-esteem and anti-social attitudes, and the relationship between the length of program participation and post-release recidivism rates after controlling for individual risk factors. Recidivism rates were significantly related to length of program participation.

Traditional and nontraditional interdisciplinary teams bring together varied professionals with diversity in expertise and thought. The purpose of interdisciplinary teams within healthcare settings is to support clients or patients in the achievement of physiological, psychological, and spiritual health. However, in absence of cultural awareness and cultural sensitivity, the physiological, psychological, and spiritual health needs of clients or patients can go unmet. Hence, it is imperative that interdisciplinary teams seek consultation when team members are unfamiliar with the culture of patients or clients to whom the team is assigned to provide care. Increasing cultural awareness and cultural sensitivity must be a collaborative effort between the interdisciplinary team and the client or patient. Nontraditional interdisciplinary teams conceptually examine teams that impact others in settings outside of education and health care.

Abstract

Purpose

Reexamination and reinterpretation of the process of deinstitutionalization of public mental hospital inpatients.

Methodology/approach

A comprehensive review of related research is presented and lessons learned for the sociology of mental health are identified.

Findings

The processes of both institutionalization and deinstitutionalization were motivated by belief in the influence of the social environment on the course of mental illness, but while in the early 19th century the social environment of the mental hospital was seen as therapeutic, later in the 20th century the now primarily custodial social environment of large state mental hospitals was seen as iatrogenic. Nonetheless, research in both periods indicated the benefit of socially supportive environments in the hospital, while research on programs for deinstitutionalized patients and for homeless persons indicated the value of comparable features in community programs.

Research limitations/implications

While the process of deinstitutionalization is largely concluded, research should focus on identifying features of the social environment that can maximize rehabilitation.

Practical implications

The debate over the merits of hospital-based and community-based mental health services is misplaced; policies should instead focus on the alternatives for providing socially supportive environments. Deinstitutionalization in the absence of socially supportive programs has been associated with increased rates of homelessness and incarceration among those most chronically ill.

Originality/value

A comprehensive analysis of deinstitutionalization that highlights flaws in prior sociological perspectives and charts a new direction for scholarship.

Abstract

While social network support has been found to be critical for persons with schizophrenia and other severe mental disorders, these persons are at risk for having smaller, less functional networks than non-clinical populations. Multi-purpose or multiplex network ties are especially beneficial because they are known to be associated with positive outcomes. This study examined which types of factors were related to network multiplexity over a ten-year period in a sample of 234 persons with severe mental illness, three-fourths of whom had schizophrenia-spectrum disorders. We asked whether clinical characteristics, the nature of the sheltered-care social environments in which subjects were living at baseline, and the number of residential care episodes predicted network multiplexity at follow-up. Using three different conceptualizations of network multiplexity as outcomes, we found that, in general, a prior history of long-term institutionalization, social environments rated higher on practical orientation, and fewer residential care episodes over the study period predicted networks deficient in tie multiplexity. Contrary to expectations, psychopathology was related to only one of the multiplexity outcomes.

With the increasing trend toward online curriculum in academe, business school educators are faced with the challenge of discerning the best way to develop global leadership competencies in their students in virtual classrooms. The challenge is difficult enough in face-to-face classroom milieus, let alone in virtual teaching conditions. This chapter is a report of one educator’s experience in applying an experientially rigorous competency development program from a traditional classroom setting to an online course. The chapter explores what constitutes an experientially rigoros learning process and concludes with an elaboration of principles that instructors can apply when deploying such pedagogies in online courses for the purpose of developing global leadership.

Abstract

This chapter summarizes findings and conclusions from recent studies exploring the role of motivation-readiness factors in drug abuse treatment. The research focuses on populations entering drug treatment, particularly therapeutic community programs in community- and prison-based settings. However, findings from studies in other modalities and from samples not entering treatment are also discussed. Issues addressed include (1) the nature of the motivational concept in recovery, (2) motivation as a variable affecting treatment retention and outcomes, (3) motivation in the treatment process, (4) differences in motivation across treatment populations and modalities, (5) client correlates of motivation, and (6) motivational enhancement. Conclusions highlight the critical role of motivation-readiness factors in understanding treatment-seeking, retention, and outcomes. Key implications are discussed for research, theory, treatment practice, and health care policy. These implications underscore issues relating to the interaction of motivation and treatment processes, the interaction of motivation and treatment demands, differences in motivation among special populations, client correlates of motivation, and self-selection and study designs.

Abstract

Purpose

To utilize LGBTQ affirmative theory to inform clinical work which affords queer youth with disabilities agency and authorship in their negotiations of sexuality.

Methodology/approach

The authors use a case study to explore the use of queer affirmative theory and peer consultation to guide and evaluate an ongoing clinical case of a young gay man with cerebral palsy as he negotiates his developing sexuality amid powerful messages from media, pornography, friends, and parental influence.

Findings

This paper finds that a queer affirmative therapy model which explores themes of intersectionality, and utilizes nuanced views of sexual identity, sexual behavior, and gender identity are useful to practitioners to encourage agency and authorship for queer disabled people in their negotiations of ability, sexuality, identity, and behavior.

Originality/value

This paper provides an alternative approach to nurturing queer identity by (1) creating refuge for emerging sexualities; (2) allowing for difficult dialogues where ability, sexuality, and gender can be pragmatically discussed, performed, and negotiated; (3) tolerating the discomfort of these difficult dialogues and pushing through to nurturing the unique queerness that evolves out of these conversations; and finally (4) encouraging transformation of all participants including client and practitioners. The practitioners discuss their own transformation through the co-created dialog with each other and with the client.

Contemporary emphasis on language and communication in mental health research and practice establishes the need for a communication model that addresses the variety of contexts – institutional, social, and cultural – in which attribution of mental disorders, treatment, and recovery occur. A ‘triadic’ approach to communication considers sufferers’: (1) transactions with environmental circumstances; (2) interpretive engagement with symbolic/discursive resources; and (3) relational interactions with others. Foundations for the model derive from pragmatism, systems theory, and theories of dialogue. A brief case study of depressive illness illustrates the value of the model. Implications for the organization of professional expertise in mental health fields are discussed.

‘The children we are dealing with, they are so damaged – their lives are so damaged – they are beyond repair!’ The Clinical Psychologist who said this1 went on to talk about a cluster of children with whom she had worked, children all from the same community, who had Speech & Language Disorder (S&LD), or (and sometimes both) Attention Deficit & Hyperactivity Disorder (S&LD/ADHD).2 What is remarkable about that therapist's statement, made casually, and at the same time a statement by a professional, is the insight expressed in the clause, amplified by the parentheses surrounding it and by and the stress on the word, ‘their lives are so damaged’, that these children's speech and language and related cognitive difficulties are not to be understood psychologically, at the level of the inner life of the individual child, but rather in a broader, holistic sociological context – ‘their lives’ as a whole. In other words that the speech and language disorders of children are to be grasped and understood by use of Mills’ (1959) ‘sociological imagination’ whereby we can come to see and to understand the recursive interrelationship between a person's personal biography, their private troubles of milieu, and the broader historical and public issues of social structure, only by comprehending both together.

Based on ethnographic data and a textual analysis, this chapter highlights the process of “therapization” of Buddhism in Western countries, with a specific emphasis on Tibetan Buddhism in France. Referring to the paradigm of “political economy of health”, as developed in recent medical anthropology, it attempts to explore the relationships between two concepts – economics and health – that had previously been considered separately, in the context of Western Buddhism. Further, this chapter's aim is to expose a potential application of theoretical economic models in an anthropological approach of Buddhist diffusion and appropriation in the West.
