Abstract

Purpose

The prevalence of overweight and obesity in children and adolescents is on the increase in developing countries. Therefore, a cheap, accessible and simple screening tool such as the mid-upper arm circumference (MUAC) is required for the prompt assessment. The purpose of this paper is to determine the usefulness of MUAC in assessing overnutrition in comparison with bioelectrical impedance analysis (BIA).

Design/methodology/approach

Participants included 1,067 children aged 6–18 years recruited from private and public schools in Egor Local Government Area in Benin City, Nigeria. Body fat was estimated by BIA using a Tanita scale, whereas the MUAC was measured with a non-elastic tape. Receiver operating characteristic analysis was used to test the ability of MUAC to determine children and adolescents identified as overweight and obese by BIA.

Findings

The prevalence of overnutrition by MUAC (12.4 percent – overweight 6.0 percent and obesity 6.4 percent) was comparable to that by BIA (12.3 percent – overweight 5.4 percent, obesity 6.9 percent). There was a significant correlation between MUAC and body fat percentage, fat mass, fat mass index and fat-free mass index in both males and females (p=0.000).

Research limitations/implications

This study, in contrast to most other studies on the use of MUAC in the assessment of overnutrition, has the advantage of using BIA cut-offs values against body mass index which does not assess body fat composition. BIA is, however, not the gold standard in the measurement of body fat composition. The optimal MUAC cut-off values of this study may not be representative of the entire country because of its restriction to Benin. Similar studies from different parts of Nigeria will be required to validate this smoothed MUAC percentiles for use in the screening of children and adolescents for overnutrition.

Originality/value

MUAC compares well with BIA in this study and can be a useful, alternative and practical screening tool for assessing obesity in the resource-poor setting.

Abstract

Purpose

This study was aimed at assessing the anthropometry and the nutritional status of a sample of the elderly in Turkey. It was designed as a cross‐sectional study using the mini nutritional assessment (MNA) scores to assess the nutritional status of the elderly living alone, with their family, and in nursing homes.

Design/methodology/approach

The work was planned and carried out so as to assess nutritional status and MNA of 429 elderly people, aged 65 years and over living with their family (n  =  246), in nursing homes (n  =  127) or alone (n  =  56). Body weight, height, mid upper arm and calf circumference of elderly people was measured and they were asked personal questions. The list of questions included if the person pursues an independent life, the number of meals and food groups they consumed, the answers of which were scored and then assessed. With regard to MNA scale, a score of <17 is regarded as an indication of malnutrition, 17–23.5 indicates a sign for malnutrition risk and 24 and higher signifies normal level. The data were analysed with SPSS 10.0 for Windows. Means, standard deviations and cross tabulation tables were used as descriptive statistics. Pearson's correlation coefficients were determined in correlation analysis.

Findings

A statistical analysis of the scores shows that 5.4 per cent of the elderly people living alone, 2.4 per cent of those living in nursing homes and 0.4 per cent living with families could be regarded as malnourished. It was also found that elderly people living in nursing homes have more risk for malnutrition. MNA scores, based on measurements such as body weight, BMI (body mass index) and mid upper arm indicated a significant correlation with the malnutrition.

Practical implications

MNA appears to be a practical and reliable method to assess the nutritional status of elderly people and more importance should be attached to those living alone or in nursing homes to prevent their nutritional problems.

Abstract

Purpose

The purpose of this paper is to develop a replicable model that ensures Household Water Treatment and Safe Storage as well as water treatment facilities at the community level by providing total service coverage at community scale. An intervention was implemented in one of the low-lying areas of Basirhat Municipality (West Bengal, India) that included a number of action programs in order to address household- and community-level water-induced challenges.

Design/methodology/approach

A research study was undertaken to identify the root causes of the problems that are generally spawned from geomorphological, hydro-fluvial, climatic factors and processes and the situation becomes complicated when many other cumulative problem-contexts layovers the existing ones. A number of social and technological innovations were tested in the field and this paper critically examined the intervention processes and outcomes. It was implemented through participatory process by involving related stakeholders working at that scale so that necessary public acceptance is received for scaling up, at least, in the similar physical, social, economic and institutional contexts.

Findings

The problem conceptualization process, spatial assessment for contextualizing the problem, design of interventions for different scales, development of project deployment strategies from field-based learnings contributed in developing a total solution based on fusing of household-level technical solutions, social innovations and actions for community engagements towards sustainability. Mobilized community members in addressing local inundation and waterlogging crisis. Satellite image-based maps shown to make them understand the upper-lower connection of drainage. People also developed their own action plans and engaged themselves in resuscitation of an old canal, removed the garbage that resulted in improved drainage conditions in the area.

Research limitations/implications

Pandemic due to COVID 19 and its related prolonged lock down, West Bengal State Assembly Election, closure of municipal governance system due to the forthcoming municipal election, closure of educational institutions, closure of Anganwadi Centre in the field area were the limitations. Due to the lock down, it was difficult for the team to maintain the time frame as well as the budget. As per the Election’s Code of Conduct gets released no public meeting was allowed without permission, people in the vicinity became suspicious, hence movement of the team members got restricted.

Practical implications

Due to the COVID protocols, the team could not organise mass training programs. It was difficult for the team members to commute in public/private transport, hence filed work got impacted. As the team could not access data from the health department, they developed a strategy of generation data on body mass index, mid-upper arm circumferences and waist-to-hip ratios to understand the status of health and nutrition of the community. It was difficult to access the Public Health Engineering Department’s laboratory situated in the municipality for water sample test. Cost escalated due to extension of the project time.

Social implications

During the second phase (wave) when people lost access to health facilities they requested the team to stop field visit. Women’s empowerment through acquiring knowledge and skill on treatment and safe storage of drinking water at home. Men appreciated and recognized this, which improved the status of women in the society. Children after expressing their willingness to learn the new technology of water purification were given handholding training by their mothers and knowledge transfer has taken place in the next generation. Mobilized community members in addressing local inundation and waterlogging crisis. Originality/value

Household-level solutions include supply of low cost, easy operable, sustainable water purifiers, community-level solution focused on securing water-related challenges at social/public gathering places and wider catchment area level solutions include the engagement of local communities to drain out stagnant waters by clearing drains, creating/digging small canals through collective actions. Geo-spatial techniques (topographical mapping, spatial survey, water quality tests) along with social methods such as participatory appraisals for gathering information on human health, public awareness campaigns and partnership development with local government agencies were the major activities performed as part of the implementation of interventions. It is imperative to mention that water-related challenges in the low-lying settlement areas of Basirhat Municipality have effectively been addressed by relying on necessary theoretical underpinnings (Disaster risk reduction/humanitarian principles) transmitted through application of scientific techniques and mediated through local people and their agencies.

Abstract

Purpose

This paper aims to evaluate the impact of garden cress supplemented biscuits on the nutritional status of malnourished children.

Design/methodology/approach

For the present study, 60 underweight and anemic seven-nine-year-old school children were selected according to the World Health Organization (WHO) classification and divided into two groups, i.e. experimental (30) and control (30). Biscuits (60 g) developed using roasted garden cress seeds were supplemented to the experimental group, while biscuits without garden cress seeds were provided to control group for a period of three months and impact was observed in terms of improvement in nutritional status of subjects before and after the supplementation.

Findings

The food and nutrient intake increased with increased percent nutrient adequacy and sharp increase (p ≤ 0.05) was noticed in cereal, fat and sugar after supplementation. Average height, weight, body mass index and mid-upper arm circumference increased, with 3.56 and 0.87 per cent gain in weight and height (p ≤ 0.05) respectively. Hemoglobin levels increased from 10.6 to 11g/dl with little improvement (p ≤ 0.05) in proteins, albumin and other indices of blood profile and nine subjects fell in non-anemic category.

Research limitations/implications

The diets of both groups were not controlled, which might have varied the results.

Practical implications

Garden cress-seed-enriched biscuits were able to have a positive impact on the nutritional profile of malnourished and anemic school children.

Social/implications

The duration of supplementation was short, which may have affected these results. Therefore, researchers are encouraged to perform long-duration supplementation study for more accurate results.

Originality/value

This paper identifies the need for promoting garden cress seeds in supplementary foods to reduce malnutrition.

Abstract

Purpose

The purpose of this paper is to compare the nutritional status and certain anthropometric characteristics in school-age children with attention deficit hyperactivity disorder (ADHD) and healthy controls.

Design/methodology/approach

A total of 200 children (ages eight to 11), half with ADHD and the other half healthy, participated in the study. A three-day food consumption record was taken and evaluated based on energy and macro-micro nutrient requirements. Weight, height, waist, hip, mid-upper arm circumferences and hand grip strength were measured.

Findings

Energy, protein, fat, saturated fatty acids, monounsaturated fatty acids, carbohydrate, niacin and zinc intakes were found significantly lower than healthy controls for female participants and all nutrient intakes were found significantly lower than healthy controls for male participants (p<0.05). Children with ADHD had lower anthropometric measures (p>0.05).

Research limitations/implications

The study sample size was too small to make results general and subtypes of ADHD and medication types were disregarded during the data collection process which limited the interpretation of the study results.

Originality/value

There are only a few researches about nutritional status of children with ADHD in Turkey. In this study, the authors found that children with ADHD have inadequate and unbalanced diet. Because of this result it is important to evaluate and monitor the nutritional status and habits of children diagnosed with ADHD for a healthy growth and development.

Abstract

Purpose

This study aims to determine the impact of nutrition education intervention on nutritional status of undernourished children aged 6-24 months in the East Mamprusi district of Ghana.

Design/methodology/approach

This was a pre-post interventional study. In all, 153 undernourished children of age 6-24 months and their mothers were recruited from five communities. Underweight, stunting, wasting, mid upper arm circumference (MUAC) and haemoglobin (Hb) levels were determined at baseline and after intervention. Nutrition education was given to the mothers for three months based on the nutrition problems identified at baseline assessment.

Findings

There were improvements in underweight, wasting, MUAC and Hb levels among the children after the nutrition education intervention. The level of improvement at post-intervention differed significantly for underweight (−2.4 ± 0.4, −2.2 ± 0.4; p = 0. 001), wasting (−2.2 ± 0.6, −1.9 ± 0.6; p = 0. 001), MUAC (12.3 ± 0.5, 12.9 ± 0.6; p = 0. 001) and Hb level (10.0 ± 1.4, 10.5 ± 1.4; p = 0. 001) of the children compared to pre-intervention (−1.4 ± 1.8, −1.3 ± 1.7; p = 0.314).

Practical implications

There was significant improvement in the nutritional status of the undernourished children after the nutrition education intervention. Community nutritionists and stakeholders involved in nutrition and health issues concerning infants and young children should consider nutrition education, as it could be an inexpensive and effective strategy in combating childhood undernutrition in resource-poor settings.

Originality/value

Findings of this study provide evidence, suggesting nutrition education as a potential strategy to curb the high rates of child undernutrition in resource-poor communities in Ghana. This study provides insight for larger studies on the effectiveness of nutrition education in curbing child malnutrition in deprived communities in Ghana.

Purpose

In Indonesia, 29.3% of Indonesian women were obese. Dehydrated women have 1.6 times higher risk of becoming obese than normal ones. This study was aimed to further explore the association between gestational weight gain (GWG) and maternal hydration status.

Design/methodology/approach

This longitudinal study involved 50 pregnant women aged 18–35 years. Subject identity, age, parity and household expenses were collected using questionnaire. Urine specific gravity (USG) was used as an indicator for hydration status. Venous blood was collected to measured haemoglobin (Hb) by anemometer method. Anthropometric data such as pre-pregnancy weight, maternal body weight, GWG, body mass index (BMI) and mid-upper arm circumference (MUAC) was collected using standardized tools. Dietary and total water intake (TWI) were measured through three consecutive days of a 24-h food recall. Pearson’s correlation, independent t-test and analysis of covariance were used to appraise the hypothesis.

Findings

The subject’s mean age was 29.3 ± 5.6 years; BMI was 23.5 ± 4.1 kg/m2. MUAC (28.2 ± 4.0 cm) and Hb (11.5 ± 1.0 g/dl) were still in the normal range. Most subjects were well hydrated, with an average USG of 1.016 g/mL. GWG and TWI are significantly correlated (p < 0.01). Based on the hydration status, after corrected with TWI, GWG was not significantly different.

Originality/value

It calls to mind that nutrition and water intake, alongside weight gain monitoring during pregnancy, are part of an effort to prevent pregnancy risks.

Abstract

Purpose

This study aims to assess the nutritional status of children with sickle cell diseases using anthropometric measurements, biochemical markers and dietary intakes.

Design/methodology/approach

The study was conducted in 100 children of 3-12 years of age with sickle cell diseases (SCDs) at the Komfo Anokye Teaching Hospital in the Kumasi Metropolis of Ghana. Weight, height and age of participants were used to calculate body mass index-for-age. The mid-upper-arm-circumference-for-age, weight-for-age (percentiles) and height-for-age (percentiles) were compared with standards growth charts for children. Biochemical measures such as serum albumin and ferritin, as well as full blood count, were assessed. Dietary intake was assessed using 24-h dietary recall and food frequency questionnaire.

Findings

From the study, 73 and 37 per cent of the children with SCD recorded inadequate intake of iron and vitamin E, respectively, when compared to the recommended daily allowance. Out of the 100 participants, 37 per cent were underweight and 22 per cent were stunted. There was significant difference (p < 0.05) in underweight (weight-for-age) prevalence by gender. A multiple variate regression showed a significant association between zinc (r2 = 0.763, p < 0.05) and haemoglobin levels.

Originality/value

The evidence in this paper is relevant for treatment, health education and nutritional counselling of parents with children who have SCD.

Abstract

Purpose

Improving the health of children under the age of five is one of the main objectives of primary healthcare services in most developing countries. The purpose of this paper is to identify the nutritional status of children under the age of five and its impact on cognitive function.

Design/methodology/approach

The cross-sectional study was carried out in a semi-urban community in south-western Nigeria. The sample consisted of 220 school children under the age of five. Anthropometric measurements including weight, height and mid arm circumference were carried out to determine the nutritional status of the children. Cognitive function of the children was assessed using a developmental checklist developed by the American Academy of Pediatrics and their performance was graded as good, fair and poor.

Findings

The overall prevalence of malnutrition in this study was low with stunting, underweight and wasting recording 8.2, 6.8 and 1.8 percent, respectively thereby making stunting the most common malnutrition indicator in this study. A large proportion of the children (88.6 percent) had good cognitive performance while 11.4 and 5.5 percent had fair and poor cognitive performance, respectively. Among the various malnutrition indicators measured in this study, stunting was found to have a statistically significant association with cognitive development (p=0.005).Stunted children have multiple functional disadvantages that persist throughout childhood and poor nutrition almost certainly plays a role. This paper calls for a need for the Nigerian Government to intervene by ensuring that policies are implemented that will ensure that health and nutritional needs of preschool children are guaranteed right from conception.

Originality/value

In view of the importance of nutrition for cognition in children, this research has shown that the cognitive function of preschool children can be affected by stunting and it has contributed to global data on nutrition and cognition particularly in South-Western Nigeria.

Abstract

Purpose

The aim of the study is to assess the dietary diversity score (DDS) and nutritional status of women in the University of Agriculture extension village in rural areas of Ogun State, Nigeria.

Design/methodology/approach

A representative sample of 204 women was surveyed from the rural villages in two seasons using simple random technique. A pretested interview guide was used to collect information on personal data, and socio demographic characteristics of the subjects. Information on dietary diversity was obtained using Food and Nutrition Technical Assistance Project questionnaire (FANTA). Body mass index (BMI), waist‐hip ratio (WHR) and skin fold thickness were used in classifying obesity. Information on food intake was obtained from 24‐hour dietary recall techniques. For each season, a DDS was computed and the nutritional status was assessed. Data were analysed using descriptive statistics; SPSS software package version 16.0.

Findings

The socio economic characteristic revealed that more than half (53 percent) had no formal education, 59 percent were small scale farmers. The DDS increased from 3.55±7.6 to 3.93±4.3 between the two seasons. DDS within the various food groups showed that fruit group increased from 0.48±0.20 to 0.52.±0.10, vegetable group 0.60±0.30 to 0.66±0.30, and meat group 0.48±0.20 to 0.58±0.30 while cereals/grain groups and oil/dairies group decreased from 0.42±0.30 to 0.37, 0.11±0.30 to 0.09, respectively, between the two seasons. The nutrient content of the subjects was calculated from nutrient contents of foods obtained from food composition tables and compared to dietary reference intake (DRI). The results showed that the protein, energy and fat intakes of the subjects increased significantly during the rainy season (p<0.05). Two percent of the subjects gained weight, 4 percent had weight reduction from overweight and obese category. The result of the waist‐hip ratio showed that 78 percent had normal waist‐hip ratio, 14 percent had overweight, and 7 percent were obese. The dietary diversity score of the subjects was low. There is need for nutritional education in the villages.

Originality/value

This may be the first research work to assess the dietary diversity score of rural women in Nigeria.
