Abstract

Purpose

Metabolic syndrome contains metabolic disorders that have association with other chronic diseases. Melatonin is a bioactive compound which is found in plants and also produced in the body. The purpose of this paper is to assess the effect of melatonin supplement on metabolic syndrome components, also leptin and adiponectin blood concentrations in patients with metabolic syndrome.

Design/methodology/approach

A double blind, placebo-controlled, randomized clinical trial was conducted on 70 subjects with metabolic syndrome. Participants received 6 mg/day melatonin or placebo before bedtime for 12 weeks. At the beginning and end of treatment period, blood samples were collected and biochemical parameters were measured. In addition, blood pressure and anthropometric indices were examined before and after the supplementation. Independent sample t-test was used to compare changes in metabolic syndrome components between the two study groups.

Findings

Results showed a significant reduction in waist circumference (−1.54 vs −0.04 cm; p = 0.036), systolic blood pressure (−3.52 vs 0.79 mmHg; p = 0.020), diastolic blood pressure (−1.50 vs 1.73 mmHg; p = 0.014), serum leptin concentration (−2.54 vs 0.27ng/ml; p = 0.041) and an elevation in high-density lipoprotein cholesterol (2.19 vs −0.79 mg/dl; p = 0.038) in the melatonin group compared to the placebo.

Research limitations/implications

If insulin concentration had been measured, it might have revealed better interpretation of melatonin effect on fasting blood glucose.

Originality/value

This study showed that melatonin as a nutritional supplement improved most metabolic syndrome components and concentration of leptin in the melatonin group compared to the placebo.

Abstract

Sleep disorders are extremely common in children and young people, and even more so in those with developmental disabilities. This client subgroup may prove refractory to standard behavioural and other psychosocial interventions that usually help and it is frequent for clinicians to resort to medication approaches given the high levels of personal and family distress that result. There is a need not only for further well structured research in this field but also for efforts to tackle the frequent relative neglect by clinicians of more researched and considered approaches. This review examines these issues selectively, focusing on important clinical issues and predicaments, and attempts a start at an evidence‐based background for practical and rational prescribing in this area of common childhood psychopathology.

Abstract

Purpose

The purpose of this paper is to explore the prevalence and patterns of substance use among Canadian social workers. With legalisation of can professional regulatory bodies are pressed to consider implications of substance use for their members.

Design/methodology/approach

An online survey collected data about demographics and substance use prevalence and patterns. Statistical analysis involved pairwise comparisons, binary logistic regression models and logistic regression models to explore correlations between substance use and demographic and work-related variables.

Findings

Among the respondents (n=489), findings indicate that past-year use of cannabis (24.1 per cent), cocaine (4.5 per cent), ecstasy (1.4 per cent), amphetamines (4.3 per cent), hallucinogens (2.4 per cent), opioid pain relievers (21.0 per cent) and alcohol (83.1 per cent) are higher than the general Canadian population. Years of work experience and working night shift were significant predictors of total number of substances used in the past year. Use of a substance by a person when they were a student was highly correlated with use when they were a professional.

Research limitations/implications

Prevalence of substance use among social workers was found to be higher than the Canadian population; potential due to the anonymous nature of data collection.

Originality/value

This study has implications for social conceptualisations of professionalism and for decisions regarding professional regulation. Previous literature about substance use by professionals has focussed predominantly on implications for increased surveillance, monitoring, and disciplinary action. We contend that since substance use among professionals tends to be concealed, there may be exacerbated social misconceptions about degree of risk and when it is appropriate to intervene.

Abstract

Purpose

Policing requires atypical work hours. The present study examined associations between shiftwork and pregnancy loss among female police officers.

Design/methodology/approach

Participants were 91 female officers with a prior history of at least one pregnancy. Shiftwork information was assessed using daily electronic payroll work records. Any prior pregnancy loss (due to miscarriage) was self-reported. Logistic regression estimated odds ratios (OR) and 95% confidence intervals (CI) for main associations.

Findings

On average, the officers were 42 years old, had 14 years of service, and 56% reported a prior pregnancy loss. Officers who worked dominantly on the afternoon or night shift during their career had 96% greater odds of pregnancy loss compared to those on day shift (OR = 1.96, 95% CI:0.71–5.42), but the result was not statistically significant. A 25% increase in percent of hours worked on night shift was associated with 87% increased odds of pregnancy loss (OR = 1.87, 95% CI:1.01–3.47). Associations were adjusted for demographic and lifestyle factors. Objective assessment of shiftwork via electronic records strengthened the study. Limitations include small sample size, cross-sectional design and lack of details on pregnancy loss or the timing of pregnancy loss with regard to shiftwork.

Research limitations/implications

The present study is preliminary and cross-sectional.

Practical implications

With considerable further inquiry and findings into this topic, results may have an impact on police policy affecting shift work and pregnant police officers.

Social implications

Implication on the health and welfare of police officers.

Originality/value

To our knowledge, there are no empirical studies which associate shiftwork and pregnancy loss among police officers. This preliminary study suggested an association between shiftwork and increased odds of pregnancy loss and points out the need for further study.

Abstract

Hormonal contraceptives are complicated reproductive technologies – both biologically and socially. Deeply embedded in global political-economic agendas and historically underpinned by eugenic movements, hormonal contraceptives have a social life often beyond their intended or imagined uses. Because so much of the discussion around contraceptives focuses on their complex history and volatile present, there has been minimal space to talk about the future of hormonal contraceptives. In this chapter I show that while the past and present are complex, the future is even more so!

As the threat of climate change becomes more palpable, two key anxieties (re)surface. First, a fear around growing populations in the Global South (while in reality Total Fertility Rate (TFRs) are in decline) and second, that of a hormonal body out of sync in the face of environmental changes. Similar anxieties have historically mobilised draconian ‘family planning’ measures in countries (like India) in the first instance. And in the second instance, hormonal manipulations to find ‘balance’ in the body, as opposed to balancing (or coming to a reckoning with) contemporary environments with/in which the body exists.

This chapter is an attempt to bring to the fore the importance of studying hormonal contraceptives in environmentally unstable times. To imagine a space beyond coercion or ‘choice’ as variously imagined, when it comes to reproductive justice vis-à-vis hormonal contraception. I suggest that, just as contraceptives have allowed us access to conversations about both women's autonomy and reproductive control, they now allow us to unpack the limits and potentials of hormonal management via the hormonal contraceptive pill.

Purpose

The management of individuals with autism spectrum disorders (ASDs) requires a multimodal approach of behavioural, educational and pharmacological treatments. At present, there are no available drugs to treat the core symptoms of ASDs and therefore a wide range of psychotropic medications are used in the management of problems behaviours, co-occurring psychiatric disorders and other associated features. The purpose of this paper is to map the literature on pharmacological treatment in persons with ASD in order to identify those most commonly used, choice criteria, and safety.

Design/methodology/approach

A systematic mapping of the recent literature was undertaken on the basis of the following questions: What are the most frequently used psychoactive compounds in ASD? What are the criteria guiding the choice of a specific compound? How effective and safe is every psychoactive drug used in ASD? The literature search was conducted through search engines available on Medline, Medmatrix, NHS Evidence, Web of Science and the Cochrane Library.

Findings

Many psychotropic medications have been studied in ASDs, but few have strong evidence to support their use. Most commonly prescribed medications, in order of frequency, are antipsychotics, antidepressants, anticonvulsants and stimulants, many of them without definitive studies guiding their usage. Recent animal studies can be useful models for understanding the common pathogenic pathways leading to ASDs, and have the potential to offer new biologically focused treatment options.

Originality/value

This is a practice review paper applying recent evidence from the literature.

Abstract

Purpose

This review provides an overview of recent advances in electrochemical sensors for analyte detection in saliva, highlighting their potential applications in diagnostics and health care. The purpose of this paper is to summarize the current state of the field, identify challenges and limitations and discuss future prospects for the development of saliva-based electrochemical sensors.

Design/methodology/approach

The paper reviews relevant literature and research articles to examine the latest developments in electrochemical sensing technologies for saliva analysis. It explores the use of various electrode materials, including carbon nanomaterial, metal nanoparticles and conducting polymers, as well as the integration of microfluidics, lab-on-a-chip (LOC) devices and wearable/implantable technologies. The design and fabrication methodologies used in these sensors are discussed, along with sample preparation techniques and biorecognition elements for enhancing sensor performance.

Findings

Electrochemical sensors for salivary analyte detection have demonstrated excellent potential for noninvasive, rapid and cost-effective diagnostics. Recent advancements have resulted in improved sensor selectivity, stability, sensitivity and compatibility with complex saliva samples. Integration with microfluidics and LOC technologies has shown promise in enhancing sensor efficiency and accuracy. In addition, wearable and implantable sensors enable continuous, real-time monitoring of salivary analytes, opening new avenues for personalized health care and disease management.

Originality/value

This review presents an up-to-date overview of electrochemical sensors for analyte detection in saliva, offering insights into their design, fabrication and performance. It highlights the originality and value of integrating electrochemical sensing with microfluidics, wearable/implantable technologies and point-of-care testing platforms. The review also identifies challenges and limitations, such as interference from other saliva components and the need for improved stability and reproducibility. Future prospects include the development of novel microfluidic devices, advanced materials and user-friendly diagnostic devices to unlock the full potential of saliva-based electrochemical sensing in clinical practice.

Abstract

Purpose

The purpose of this paper is to discuss the challenges of assessment and management of psychotic symptoms in a background of intellectual disability (ID) and treatment-resistant epilepsy caused by a genetic syndrome.

Design/methodology/approach

Ring chromosome 20 [r(20)] syndrome is characterised by the triad of severe refractory epilepsy, mild to severe ID and behavioural problems. This paper describes the presentation of r(20) syndrome in a young woman with moderate ID and treatment-resistant epilepsy, who experiences psychotic symptoms at times of improved seizure control.

Findings

There are several diagnostic possibilities for such a presentation, including psychotic symptoms due to adverse effects of anti-epileptic medications and forced normalisation (alternating psychosis).

Originality/value

This paper advocates judicious use of antipsychotic medication to manage psychotic symptoms, as well as involvement of both patient and close family members throughout all stages of care. It is essential to strike a balance between control of epileptic seizures and psychiatric symptoms, providing an optimal benefit to the patients’ quality of life by meeting their complex needs through a multidisciplinary and multi-agency team input.
