Abstract

Purpose

The purpose of this paper is to present a social network approach for identification of micro-organizational re-design interventions to make more efficient and fluid the knowledge flow in a rehabilitation multidisciplinary team. The structural information of different kinds of knowledge networks within a team is augmented with additional analyses aimed at collecting information about the ways through which participants use knowledge, the motivation behind knowledge exchange, and the non-human knowledge sources used by subjects to perform their work. This paperwork was supported by CNCSIS – UEFISCDI, project number PNII – IDEI 810/2008.

Design/methodology/approach

The authors propose a definition of knowledge network including human and non-human knowledge source (documents and knowledge repositories) as it is more adequate for the analysis of knowledge flows in multidisciplary medical teams. The mapping and analysis of the network are carried out through: elicitation of knowledge flows between people within and outside the team through a structured questionnaire; mapping of the knowledge flows toward non-human knowledge sources; and identification of critical aspects and proposal of re-engineering interventions to make knowledge flow more efficient and effective.

Findings

The analysis of the critical aspects emerged in the field study identifies a number of opportunities to improve the efficiency and effectiveness of knowledge sharing through the re-design of the team network. The re-design interventions concern three main features of knowledge network: “knowledge centralization,” “Over-reliance on External experts,” “Unshared knowledge tools and sources.”

Originality/value

The originality of the work resides in applying social network analysis (SNA) for healthcare management settings, proving evidence and guidelines to show how healthcare organizations can benefit from the adoption of SNA-based approaches.

Abstract

Purpose

The purpose of this paper is to investigate the elements that health care personnel in Thailand believe are necessary for successful adoption of electronic medical record (EMR) systems.

Design/methodology/approach

Initial qualitative in-depth interviews with physicians to adapt key elements from the literature to the Thai context. The 12 elements identified included things related to managing the implementation and to IT expertise. The nationwide survey was supported by the Ministry of Public Health and returned 1,069 usable questionnaires (response rate 42 percent) from a range of medical personnel.

Findings

The key elements clearly separated into a managerial dimension and an IT dimension. All were considered fairly important, but managerial expertise was more critical. In particular, there should be clear EMR project goals and scope, adequate budget allocation, clinical staff must be involved in implementation, and the IT should facilitate good electronic communication.

Research limitations/implications

Thailand is representative of middle-income developing countries, but there is no guarantee findings can be generalized. National policies differ, as do economic structures of health care industries. The focus is on management at the organizational level, but future research must also examine macro-level issues, as well as gain more depth into thinking of individual health care personnel.

Practical implications

Technical issues of EMR implementation are certainly important. However, it is clear actual adoption and use of the system also depends very heavily on managerial issues.

Originality/value

Most research on EMR implementation has been in developed countries, and has often focussed more on technical issues rather than examining managerial issues closely. Health IT is also critical in developing economies, and management of health IT implementation must be well understood.

Purpose

Online medical repositories provide a platform for users to share information and dynamically access abundant electronic health data. It is important to determine whether case report information can assist the general public in appropriately managing their diseases. Therefore, this paper aims to introduce a novel deep learning-based method that allows non-professionals to make inquiries using ordinary vocabulary, retrieving the most relevant case reports for accurate and effective health information.

Design/methodology/approach

The dataset of case reports was collected from both the patient-generated research network and the digital medical journal repository. To enhance the accuracy of obtaining relevant case reports, the authors propose a retrieval approach that combines BERT and BiLSTM methods. The authors identified representative health-related case reports and analyzed the retrieval performance, as well as user judgments.

Findings

This study aims to provide the necessary functionalities to deliver relevant health case reports based on input from ordinary terms. The proposed framework includes features for health management, user feedback acquisition and ranking by weights to obtain the most pertinent case reports.

Originality/value

This study contributes to health information systems by analyzing patients' experiences and treatments with the case report retrieval model. The results of this study can provide immense benefit to the general public who intend to find treatment decisions and experiences from relevant case reports.

Purpose

Waiting time, as an important predictor of queue abandonment and patient satisfaction, is important for resource utilization and patient experience management. Medical institutions have given top priority to reforming the appointment system for many years; however, whether the increased information transparency brought about by the appointment scheduling mechanism could improve patient waiting time is not well understood. In this study, the authors examine the effects of information transparency in reducing patient waiting time from an uncertainty perspective.

Design/methodology/approach

Leveraging a quasi-natural experiment in a tertiary academic hospital, the authors analyze over one million observational patient visit records and design the propensity score matching plus the difference in difference (PSM-DID) model and hierarchical linear modeling (HLM) to address this issue.

Findings

The authors confirm that, on average, improved information transparency significantly reduces the waiting time for patients by approximately 6.43 min, a 4.90% reduction. The authors identify three types of uncertainties (resource, process and outcome uncertainty) in the patient visit process that affect patients' waiting time. Moreover, information transparency moderates the relationship between three sources of uncertainties and waiting time.

Originality/value

The authors’ work not only provides important theoretical explanations for the patient-level factors of in-clinic waiting time and the reasons for information technology (IT)-enabled appointment scheduling by time slot (ITASS) to shorten patient waiting time and improve patient experience but also provides potential solutions for further exploration of measures to reduce patient waiting time.

Abstract

Purpose

Cybersecurity in healthcare has become an urgent matter in recent years due to various malicious attacks on hospitals and other parts of the healthcare infrastructure. The purpose of this paper is to provide an outline of how core values of the health systems, such as the principles of biomedical ethics, are in a supportive or conflicting relation to cybersecurity.

Design/methodology/approach

This paper claims that it is possible to map the desiderata relevant to cybersecurity onto the four principles of medical ethics, i.e. beneficence, non-maleficence, autonomy and justice, and explore value conflicts in that way.

Findings

With respect to the question of how these principles should be balanced, there are reasons to think that the priority of autonomy relative to beneficence and non-maleficence in contemporary medical ethics could be extended to value conflicts in health-related cybersecurity.

Research limitations/implications

However, the tension between autonomy and justice, which relates to the desideratum of usability of information and communication technology systems, cannot be ignored even if one assumes that respect for autonomy should take priority over other moral concerns.

Originality/value

In terms of value conflicts, most discussions in healthcare deal with the conflict of balancing efficiency and privacy given the sensible nature of health information. In this paper, the authors provide a broader and more detailed outline.

Abstract

Purpose

This paper tries to examine attitude toward knowledge sharing (KS) for improving the innovation in medical services. So, this paper aims to analyze the influence of information technology (IT), staff, hospital cultures and patients and their families on the attitude toward KS and also examine the influence of attitude toward KS on innovation in medical services.

Design/methodology/approach

This paper presented a project plan that contains five research hypotheses, with attitude toward KS as a mediating construct. In this paper, Smart PLS 3.0 is implemented to study the calculation. The hypotheses are tested on data collected using structural equation modeling. Data was collected using questionnaires from a hospital in Iran. The statistical population of this research is unlimited. Through using the sample measurement method, the sample size for structural equations has come to be 384.

Findings

The results showed that all theories of the project are proved. Also, they indicated the importance of IT as an important factor in attitude toward KS and the latter's influence on the innovation in medical services. Furthermore, the attitude toward KS influences positively on innovation in medical services. As a general result, the managers of health organizations should empower employees to participate in decision-making and encourage them to resolve problems together and share medical information to promote innovation in medical services.

Research limitations/implications

This research must be carefully defined and accepted because the study information had special restrictions. At first, a prejudice was created, because the sample was self-selected. Second, the participants were living in Iran. There were different cultures and lifestyles among countries. Third, this research sample consisted only of hospital staff and managers. Moreover, little empirical research has been conducted on this topic in Iran. Iran’s culture is considerably different from Western culture, and it is unknown whether the same questionnaire that is used in the Iran context can be used in the Western context. In further studies, this research could be extended to contain other psychosocial elements that permit KS in institutions and increase innovation.

Practical implications

The medical industry is knowledge-intensive, remains a competitive advantage and accumulates knowledge, and the hospitals are the main subject. The role of this research according to providing a functional-theoretical plan enables authorities and practitioners to access thoughts about efficient stimulation attitude toward KS on innovation in medical services in the health sector. Also, the findings of this study at the same time are implemented to study relations between knowledge-sharing elements such as enablers, processes and firm innovation capability.

Originality/value

Modern study design enables the relations between IT and other influential elements on attitude toward KS and innovation in medical services to be analyzed. This design determines the coexistence of the participation and contest relations among participants, which is not studied in past researches. From a management point of view, this research determined various elements essential to a successful attitude toward KS and investigated the usage of these elements for improving innovation in medical services.

Abstract

Purpose

The purpose of this paper is to raise issues about the design of personal health record systems (PHRs) and self-monitoring technology supporting self-care practices of an increasing number of individuals dealing with the management of a chronic disease in everyday life. It discusses the results of an ethnographic study exposing to analysis the intricacies and practicalities of managing diabetes “in the wild”. It then describe and discuss the patient-centric design of a diabetes journaling platform that followed the analysis.

Design/methodology/approach

The study includes ethnometodological investigation based on in depth interviews, observations in a support group for adults with type 1 diabetes, home visits, shadowing sessions and semi-structured interviews with a series of medical experts (endocrinologists, general practitioners and diabetes nurses). Findings informed the design of a proof-of-concept PHR called Tag-it-Yourself (TiY): a mobile journaling platform that enables the personalization of self-monitoring practices. The platform is thoroughly described along with an evaluation of its use with real users.

Findings

The investigation sheds light on a series of general characters of everyday chronic self-care practices, and how they ask to re-think some of the assumptions and connotations of the current medical model and the traditional sick role of the patient – often unreflectively assumed also in the design of personal technologies (e.g. PHR) to be used by patients in clinically un-controlled settings. In particular, the analysis discusses: the ubiquitous nature of diabetes that is better seen as a lifestyle, the key role of lay expertises and different forms of knowledge developed by the patient in dealing with a disease on a daily basis, and the need of more symmetrical interactions and collaborations with the medical experts.

Research limitations/implications

Reported discussions suggest the need of a more holistic view of self-management of chronic disease in everyday life with more attention being paid on the perspective of the affected individuals. Findings have potential implications on the way PHR and systems to support self-management of chronic disease in everyday life are conceived and designed.

Practical implications

The paper suggests designers and policy makers to look at chronic disease not as a medical condition to be disciplined by a clinical perspective but rather as a complex life-style where the medical cannot be separated by other aspects of everyday life. Such shift in the perspective might suggest new forms of collaborations, new ways of creative evidence and new form of knowledge creation and validation in chronic self-care.

Social implications

The paper suggests re-thinking the role of the patient in chronic-disease self-management. In particular, it suggests giving more room to the patient voice and concerns and suggest how these can enrich rather than complicate the generation of knowledge about self-care practices, at least in type 1 diabetes.

Originality/value

The paper sheds light on everyday intricacies and practicalities of dealing with a chronic disease. Studies of self-care practices that shed light on the patient perspectives are sporadic and often assume a clinical perspective, its assumptions (e.g. biomedical knowledge is the only one available to improve health outcome, doctors know best) and implications (e.g. compliance, asymmetry between the specialist and the patient).

Abstract

Purpose

The purpose of this paper is to study the managing of emergencies pertinent to public health which is critical to the well‐being of a society; as such, the management mechanisms employed should be of great interest and significance for research.

Design/methodology/approach

This paper first relies on extensive literature to describe the mechanism used in the USA from three aspects – organizational structure, management system, and logistics network. For the purpose of comparison, the Chinese version of the mechanism is presented from the same three aspects. The two management systems are then compared both qualitatively and quantitatively.

Findings

Deficient areas in Chinese public‐health management mechanism as well as challenging issues associated with supply chain design and coordination for emergency supplies in the context of large‐scale public health emergencies with low frequency but catastrophic impacts are found. Specifically, the following three important research problems are revealed from the comparative study: how to establish an efficient organizational structure that incorporates all the relevant entities in public‐health emergency management? How to establish an information system for emergency management that integrates disease surveillance, control, and prevention? How to design an efficient and cost‐effective logistics network to ensure prompt and sufficient delivery of emergency supplies?

Originality/value

To date, this research has been the first of its kind that compares two countries' emergency management systems in the context of public health management.

Purpose

The purpose of the study is to comprehend medical service quality, information quality and system quality toward actual use of Tele-Health in rural India. The study further validates the impact of the actual use of Tele-Health on sustainable development, thus providing implications to improve upon the Tele-Health penetration in India.

Design/methodology/approach

Data was collected from 326 healthcare practitioners practicing Tele-Health in North Indian states and Structural Equation Modeling was applied to validate the conceptual framework.

Findings

The results indicated that medical service quality, information quality and system quality influence Tele-Health behavioral intentions which in turn impact actual use and sustainable development. This research draws upon a conceptual framework to deepen our understanding of Tele-Health by providing an all-inclusive overview.

Originality/value

The massive topography of India with a prime rural populace instills the need for timely healthcare facilities. Tele-Health is a solution to all these problems but is at a nascent stage. Therefore, there is a vital need to study the factors which improve the penetration of Tele-Health in the Indian context. The model that emerged from the study may be validated by other Indian sub-continental countries so that Tele-Health may be implemented hassle-free.
