Abstract

Purpose

The purpose of this paper is to investigate the way technology affects the provision of prehospital emergency care, upgrading the quality of services offered and significantly reducing the risk of premature termination of the patients.

Design/methodology/approach

The paper presents the development of the eEKAB, a pilot emergency medical information system that simulates the main services offered by the Greek National Instant Aid Centre (EKAB). The eEKAB was developed on an agile system methodology. From a technical perspective, the features and the technology were mainly chosen to provide reliable and user-friendly interfaces that will attract many users. eEKAB is based on three important pillars for offering health care to the patients: the “On-time Incident Reporting”, the “On-time Arrival at the Incident” and “Transfer to the Health Center”. According to the literature review, the emergency medical services (EMS) systems that combine all the features are very few.

Findings

It reduces the total time of the EMS procedures and it allows for an easier management of EMS, by providing a better allocation of human resources and a better geographical distribution of ambulances. The evaluation displayed that it is a very helpful application for the ambulance drivers as it reduces the ambulance response time to arrive in the patient's location and contributes significantly to the general performance of the prehospital medical care system. Also, the survey verified the importance of implementing eEKAB on a larger scale beyond the pilot usage. It is worth mentioning that the younger ambulance drivers had a more positive view for the purpose of the application.

Research limitations/implications

The paper clearly identifies implications for further research. Regarding interoperability, the mobile app cooperates with the Operational Center of EKAB, while further collaboration could be achieved with other operational ambulance handling center, mainly, of the private sector. The system can evolve to include better communications among the EKAB departments. Particularly, the ambulance crew as well as the doctors should be informed with more incident features such as the emergency signal so that they know whether to open the siren, the patient's name, etc. The authors are currently working on implementing some features to provide effective medical health services to the patient in the ambulance.

Practical implications

eEKAB will have very significant implications in case of its enforcement, such as the reduction of the total time of EMS procedures with a corresponding reduction of the operating costs of an accident management system and an ambulance fleet handling system while in parallel informing in time the doctors/clinics. It will provide better distribution of ambulances as well as of total human resources. It will greatly assist ambulance drivers, while reducing ambulance response time to reach the patient's location. In other words, the authors will have a better performance of the whole prehospital care system.

Social implications

Providing emergency care before the hospital is of great importance for upgrading the quality of health services provided at the accident site, thus significantly reducing the risk of premature death of patients. This in itself has a significant social implication.

Originality/value

The paper demonstrates a solid understanding in the field of the EMS systems and the corresponding medical services offered. It proposes the development of an effective, feasible and innovative EMS information system that will improve the existing emergency health care system in Greece (EKAB). An in depth literature review and presentation of the adopted new technologies and the respective architecture take place. An evaluation and statistical validation were conducted for proving the high applicability of eEKAB in case of real-life running.

Abstract

Purpose

Qualitative methods are not suitable to process high volumes of policy texts for exploring policy evolution. Therefore, it is hard to use qualitative methods to systematically analyze the characteristics of complex policy networks. So the authors propose a bibliometric research study for exploring policy evolution from time–agency–theme perspectives to excavate the rules and existing problems of China's medical informatization policy and to provide suggestions for formulating and improving the future medical informatization policies.

Design/methodology/approach

Initially, 615 valid samples are obtained by retrieving related China's medical informatization policy documents, and the joint policy-making agency network and the co-occurrence network models of medical informatization policies are defined, and then the authors research China's medical informatization policies from single-dimension and multi-dimension view.

Findings

The analysis results reveal that China's medical informatization policy process can be divided into four stages; the policy-making agencies are divided into four subgroups by community detection analysis according to the fast unfolding algorithm; the core policy theme keywords are identified based on the eigenvector centrality of the nodes in those networks; the focuses of theme terms are varied in different stages and the correlations between agencies and themes are gradually decentralized.

Practical implications

These findings provide experience and evidence on leveraging informatics in the medical and healthcare field of China. Also, they can help scholars and practitioners better understand the current status and future directions of medical and healthcare informatics development in China and provide a reference to formulate and improve China's future medical informatization policies.

Originality/value

This study proposes a quantitative bibliometric-based research framework to describe transitions and trends of China's medical informatization policy.

Abstract

Purpose

This paper seeks to analyze attempts, in the USA and globally, to create new and improved methods used to manage patient health information: electronic medical records (EHRs). This new system of records management is to be examined to determine the possible benefits for patients, providers, insurers, employers, and others, as well as barriers to the use of EHRs, particularly in those embedded in US law.

Design/methodology/approach

In the USA, and in various other jurisdictions, new laws have been enacted to incentivize the use of EHRs, and the paper examines the regulations and provisions incentivizing the adoption of this type of integrated system for lifelong tracking of health‐related information. There are issues to be dealt with, such as cost, privacy, and legality, but each of these can and must be overcome to effectively manage and communicate health care information

Findings

The use of EHRs is increasing in the USA and globally, both in acute care hospitals and in primary care medical practices, largely accomplished through positive incentives, and penalties for non‐compliance. There are also various countries well on the way to wide‐spread use of electronic health records management.

Social implications

The use of EHRs creates a societal benefit, initially one on an individual level, but one which over time could rise to a level positively affecting health care on a national, even global level.

Originality/value

The paper is based on an analysis of relevant laws/regulations, best practices, and anecdotal/observational evidence. No empirical evidence survey or study was conducted, primarily because the process of implementation of EHRs is too new. It is of value to practitioners, policy makers, and interested public parties.

Abstract

This paper presents a framework which combines data and text retrieval techniques to exercise and evaluate ontology based query expansions. We prepare by using linguistic techniques to identify query and document concepts, locating them in a ontologically defined semantic space. Expansions originate from the identified query concepts, with success determined by matching in the relevant document set. We identify three orthogonal dimensions that can affect query expansion success; relationship source, success measure technique, and query expansion technique. Expansion technique is further divided into six different categories: simple pruning, complex probability, voting, directional, semantic propagation, and multiple source concept. We describe each technique and show examples where they would be useful. The system architecture used facilitates plugging in of various expansion and evaluation routines, and flowing results from one method to the next. The system is useful for microanalysis of query expansion, discovering which components of ontological derived knowledge most influence query expansion success. In this work, we apply our framework to the medical domain.

Abstract

The Fred Meyer Charitable Trust, Division of Library and Information Resources for the Northwest, has funded five research projects that will demonstrate the potential of various techniques and new technologies to facilitate communications and resource sharing in the Northwest. The experience and information derived from these projects will be of value to all libraries and information centers, not just those conducting the research. The techniques and technologies being evaluated include: simultaneous remote searching, which uses inexpensive terminals and modems; a mini‐computer‐based union list and resource sharing network (INFONET); networks using facsimile machines; networks that transmit documents that have been optically scanned into bit‐map image files; and use of optical character recognition equipment to capture ASCII machine‐readable information that can be broadcast by television stations to user‐sites. Abstract

Isfahan University of Medical Sciences, as one of the major medical universities in Iran, is an academic organization which is also responsible for all health‐related services in Isfahan province. Abstract

Purpose

In recent years, many development projects of the medical systems encounter difficulties and eventually fail. Failure is often due to very complicated and changeable medical procedures and the inconsistent understanding between system stakeholders, especially the healthcare providers, and information technology staff. Many research results also indicate that poor communication easily results in negative consequences during the implementation of the medical information system. To effectively overcome this obstacle, the purpose of this paper is to propose an enhanced Delphi method to assist in reaching consensus during the software development with some additional steps.

Design/methodology/approach

As an alternative to the traditional way to elicit pertinent feedback from respondents, the enhanced Delphi method stresses the systematic, flexible, and cyclic stages to construct a questionnaire with viewpoints from different types of panelists and a self-assessment procedure as a validating step to measure the improvements in the system implementation.

Findings

The better communication between the members of project team does increase the comprehensive assessment of a project.

Originality/value

Based on a practical case, the enhanced Delphi method really demonstrates good performance and effectiveness.

Abstract

Purpose

The purpose of this paper is to provide practitioners and researchers a more condensed and structured perspective on the adoption of information management systems by the health-care industry, given the spread and the increased amount of research concerning the topic.

Design/methodology/approach

This paper is a literature review. A Technology (What?) – Context and before adoption Analysis (When and Why?) – Implementation (How?) – Outcomes (What for?) framework is used to present the trends concerning information technology adoption in an accessible manner.

Findings

The main finding is that small or large health-care organizations should no longer focus on information systems’ adoption but should adopt a digital transformation paradigm. By considering this paradigm, management practices related to information technologies’ adoption projects should be complemented by practices related to the continuous organizational changes and readaptation of the organizational strategy, to benefit the advantages information systems can offer.

Practical implications

The main recommendation for health-care industry managers is to adopt specific practices to manage the digital transformation process of their organizations, as they should understand that it is no longer about adopting information technologies, but about managing the associated organizational change.

Originality/value

Instead of focusing on specific information systems’ adoption as other papers do, this paper provides a holistic understanding of the information technologies and management practices which are used in the field.

Abstract

Purpose

The purpose of this paper is to evaluate the contribution of medical information systems (IS) to efficient use of information when diagnosing chest pain complaints with suspected acute myocardial infarction (AMI) as regards ordering of tests and accuracy of diagnosis.

Design/methodology/approach

In total, 102 physicians were asked to diagnose three cases of chest pain in patients consulting an emergency department (ED) in a simulation study. Half of the participants had access to a medical IS with complete patient information and the other half of the physicians did not.

Findings

It was found that participants who viewed the medical IS ordered fewer clinical examinations. Participants who viewed the medical histories made a more accurate main differential diagnosis (DD) of AMI. Physicians with access to the medical history reported significantly higher levels of confidence in their decisions, regardless of seniority.

Originality/value

The findings suggest that IS leads to better utilization of medical services, greater efficiency and lower costs and thus has implications for other healthcare sectors.
