Abstract

Purpose

India has over half a million diabetics, with many others at risk. The Indian Diabetes Risk Score (IDRS) is a simple and validated tool used for mass screening of diabetes mellitus type 2 at the community level. This study assessed the vulnerability of developing diabetes in adults of a rural community of West Bengal using the IDRS and finds out the relationship of the risk of developing diabetes with socioclinical variables.

Design/methodology/approach

Multi-stage sampling was employed to select one eligible nondiabetic adult from selected families residing in the rural field practice area of a medical college in West Bengal. They were interviewed with a predesigned and pretested data collection schedule and examined.

Findings

Among 197 participants, 83.8% were female, 51.8% were illiterate and 57.4% came from Class IV of Prasad's socioeconomic scale. Of participants, 22.8% had existing known morbidities, and 23.9% had some form of substance addiction. In total, 46.8% of the participants on whom the IDRS could be applied (n = 175) were at high risk of developing diabetes (Score = 60). Gender and existing comorbidities significantly predicted a high risk of diabetes.

Originality/value

A large proportion of the Indian population yet to be diagnosed with diabetes are at a high risk of having the disease. Early detection of the disease can help curtail its complications and reduce its clinical, social and economic burden. Mass screening tools like the IDRS thus become a very important tool in India's attempts to fight diabetes.

Abstract

Purpose

The aim of this study was to compare the defined indicators of tuberculosis (TB) control program in the intervention and control prisons, after implementation of the national clinical protocol for TB and HIV management in Iranian prisons, suggesting active health service provision in all steps of service provision.

Design/methodology/approach

This was quasi-experimental study conducted among inmates of two prisons in Iran. Great Tehran prison was purposively chosen as the intervention prison and Karaj prison was purposively chosen as control prison as well. Intervention and control prisons were compared in terms of the TB indicators within three periods (before intervention, during implementation and follow-up period) from October 2013 to June 2014.

Findings

Number of inmates with TB symptoms who underwent TB workup was four times more in intervention prison compared to control prison (9.3 vs 2.5 cases out of 1,000 inmates per month in the case prison compared to the control prison). Such difference was also significant in the intervention prison, comparing before and during the intervention period. The patient finding in case prison increased significantly after the intervention (223.6 vs 81.8 cases out of 100,000 inmates per year). The number of TB cases who received HIV testing increased from 50 to 100%.

Originality/value

Active health service provision has significantly improved indicators in the intervention prison. The authors recommend implementation of this guideline in all prisons of Iran. Integration of other diseases with high burden among prisoners is also recommended in the active health services provision.

Abstract

Purpose

This paper aims to identify the interaction of different intervention strategies implemented in Malaysia towards flattening the curve of COVID-19 cases. Since the outbreak of COVID-19, many approaches were adopted and implemented by the Malaysian government. Some strategies gained quick wins but with negative unintended consequences after execution, whereas other strategies were slow to take effect. Learning from the previous strategies is pivotal to avoid repeating mistakes.

Design/methodology/approach

This paper presents the cause, effect of and connection among the implemented COVID-19 intervention strategies using systems thinking through the development of a causal loop diagram. It enables the visualisation of how each implemented strategy interacted with each other and collectively decreased or increased the spread of COVID-19.

Findings

The results of this study suggested that it is not only essential to control the spread of COVID-19, but also to prevent the transmission of the virus. The Malaysian experience has demonstrated that both control and preventive strategies need to be in a state of equilibrium. Focusing only on one spectrum will throw off the balance, leaving COVID-19 infection to escalate rapidly.

Originality/value

The developed feedback loops provided policy makers with the understanding of the merits, pitfalls and dynamics of prior implemented intervention strategies before devising other effective intervention strategies to defuse the spread of COVID-19 and prepare the nation for recovery.

Abstract

Purpose

The latest novel coronavirus disease 2019 (COVID-19) pandemic continues to have a significant social and financial impact globally. It is very essential to study, categorize and systematize published research on mitigation strategies adopted during previous pandemic scenario that could provide an insight into improving the current crisis. The goal of this paper is to systematize and identify gaps in previous research and suggest potential recommendations as a conceptual framework from a strategic point of view.

Design/methodology/approach

A systematic review of Scopus and Web of Science (WoS) core collection databases was performed based on strict keyword search selections followed by a bibliometric meta-analysis of the final dataset.

Findings

This study indicated that the traditional mitigation techniques adopted during past pandemics are in place but are not capable of managing the transmission capability and virulence of COVID-19. There is a greater need for rethinking and re-engineering short and long-term approaches to prevent, control and contain the current pandemic situation.

Practical implications

Integrating various mitigation approaches shall assist in flattening the pandemic curve and help in the long run.

Originality/value

Articles, conference proceedings, books, book chapters and other references from two extensive databases (Scopus and WoS) were purposively considered for this study. The search was confined to the selected keywords outlined in the methodology section of this paper.

Abstract

Purpose

The prevalence of overweight and obesity in children and adolescents is on the increase in developing countries. Therefore, a cheap, accessible and simple screening tool such as the mid-upper arm circumference (MUAC) is required for the prompt assessment. The purpose of this paper is to determine the usefulness of MUAC in assessing overnutrition in comparison with bioelectrical impedance analysis (BIA).

Design/methodology/approach

Participants included 1,067 children aged 6–18 years recruited from private and public schools in Egor Local Government Area in Benin City, Nigeria. Body fat was estimated by BIA using a Tanita scale, whereas the MUAC was measured with a non-elastic tape. Receiver operating characteristic analysis was used to test the ability of MUAC to determine children and adolescents identified as overweight and obese by BIA.

Findings

The prevalence of overnutrition by MUAC (12.4 percent – overweight 6.0 percent and obesity 6.4 percent) was comparable to that by BIA (12.3 percent – overweight 5.4 percent, obesity 6.9 percent). There was a significant correlation between MUAC and body fat percentage, fat mass, fat mass index and fat-free mass index in both males and females (p=0.000).

Research limitations/implications

This study, in contrast to most other studies on the use of MUAC in the assessment of overnutrition, has the advantage of using BIA cut-offs values against body mass index which does not assess body fat composition. BIA is, however, not the gold standard in the measurement of body fat composition. The optimal MUAC cut-off values of this study may not be representative of the entire country because of its restriction to Benin. Similar studies from different parts of Nigeria will be required to validate this smoothed MUAC percentiles for use in the screening of children and adolescents for overnutrition.

Originality/value

MUAC compares well with BIA in this study and can be a useful, alternative and practical screening tool for assessing obesity in the resource-poor setting.

Abstract

Purpose

The purpose of this paper is to develop a brief screening instrument to identify risk factors of factory workers experiencing mass fainting illness (MFI) due to work-environmental determinants.

Design/methodology/approach

A factory-based cross-sectional study was conducted among 740 workers in October 2017 and was completed with face-to-face interviews. Data analyses included univariate logistic regression, backward stepwise linear regression and multiple logistic regression. Sum scores on significant items and receiver operator characteristic curves were used to compute potential cut-off points and the sensitivity and specificity rates.

Findings

Significant work-environmental factors were identified as working at very high speeds, having less influence on the choice of working partners, perceived high temperature at work, having less opportunity to do their best at work, and concern about losing a job in the next six months. In developing a screening instrument, a 6.5 cut-off point that corresponded to 99.6 percent sensitivity and 92.2 percent specificity was identified.

Originality/value

The study concludes that this MFI-instrument could potentially be used to prevent MFI. By understanding the policy implications, the government body, employers, workers, development partners and stakeholders should work toward preventing MFI. Implementing a preventive measure is therefore warranted due to the health education impact.

Abstract

Purpose

The paper highlights the process-handling during the Enhanced Movement Control Order (EMCO) in combating pandemic COVID-19 in Malaysia.

Design/methodology/approach

Malaysia first issued an EMCO following a cluster that involved a religious gathering. The EMCO was issued to lockdown the area, undertake screening, treat positive cases and quarantine their close contacts. Active case detection and mass sampling were the main activities involving the population in both zones.

Findings

One hundred ninety-three confirmed COVID-19 cases were identified from the total population of 2,599. Of these cases, 99.5% were Malaysians, 31.7% were aged >60 years and all four deaths (Case Fatality Rate, 2.1%) were elderly people with comorbidities. One hundred and one cases (52.3%) were asymptomatic, of which 77 (77%) were detected during mass sampling. The risk factors contributing to the outbreak were contacts that had attended the religious gathering, regular mosque congregants, wedding ceremony attendees and close household contacts. Malaysia implemented an effective measure in the form of the EMCO to contain the COVID-19 outbreak, where the last cases were reported 16 days before the EMCO was lifted.

Originality/value

The residents’ compliance and inter-agency cooperation were essential elements to the success of the EMCO. A targeted approach using an EMCO should be implemented in a future pandemic.

Abstract

Purpose

Breast cancer is one of the most prevalent malignancies worldwide. This study aimed to compare the level of knowledge and attitude of females regarding Breast cancer and to determine the role of knowledge, attitude and barriers in performing regular self-examination.

Design/methodology/approach

Non-physician females aged 18 years old or above were enrolled in this cross-sectional study in two groups of usual clients and healthcare staff from January 2018 to January 2019 from a healthcare center in Sari, a major city in the northern district of Iran. A questionnaire was used to score the participants’ knowledge and attitude levels using questions about the participants' knowledge and attitude towards Breast cancer along with their status on Breast self-examination and barriers. Mean scores were used for statistical analysis using SPSS V25. p < 0.05 was considered significant.

Findings

A final dataset of 279 females were collected. A significant difference in the knowledge and attitude regarding breast cancer was found between the two study groups (p < 0.001). The practice of BSE was significantly lower in the usual clients compared to non-physician healthcare staff (p < 0.001). The most frequent barriers for not performing a regular BSE were fear of finding a mass in usual clients (17.8%) and lack of confidence in healthcare staff (3.8%).

Research limitations/implications

Limitations include single-centered sample selection.

Practical implications

Given the importance of early detection in breast cancer prevention and the general taboo regarding breast cancer screening methods in certain parts of the world, leading to poor results in early detection and prevention, the authors believe that it is of superior importance to address and promote positive attitudes in general population towards breast self-examination.

Abstract

Purpose – This paper aims to detect or identify the presence of hydrocarbon infiltration on sampling point in the Rambe River area according to the obtained VOCs and the adsorbed SVOCs.

Design/Methodology/Approach – The Gore-sorber method has been used to capture volatile organic compounds (VOCs) and semi-volatile organic compounds (SVOCs) as indicators of subsurface hydrocarbon generation and entrapment. This method is usually used in environmental surveys for the oil investigations in certain areas for surface survey screening, designed to collect a broad range of VOCs and (SVOCs) at lower concentrations, quickly and inexpensively. The results also indicated a general correlation between the GORE-SORBER and reference method data. The research was conducted in Rambe River Village, Tebing Tinggi sub-district of Tanjung Jabung Barat district, Jambi Province Indonesia. The collection of the Gore-Sorber modules were analyzed using a gas chromatography-mass spectrometer thermal desorption (GC/MS).

Findings – The results showed that from all sampling points in Tebing Tinggi areas, the dominant components detected are carbonyl sulphide, dimethyl sulfide, ethane, propane, butane, 2-methyl butane, pentane, and carbon sulfide with carbon chain in the range C2-C5. These hydrocarbon gases (C1-C4) which may be from thermogenic or microbial processes. The highest concentrations of carbonyl sulfide were 392.67 ng and dimethyl disulfide 261.90 ng.

Abstract

Purpose

To construct a risk score using both clinical and intra-oral variables and to determine a risk score to screen individuals according to their risk of hyperglycemia.

Design/methodology/approach

A cross-sectional study was carried out among 690 Thai dental patients who visited the Special Clinic, Faculty of Dentistry, Mahidol University and a mobile dental unit of His Majesty the King of Thailandss Dental Service Unit. Participants aged ≥25 years without a previous history of type 2 diabetes mellitus were included in the study. Participants diagnosed with severe anemia and polycythemia were excluded. Questionnaires were used to collect demographic data. Point-of-care HbA1c, body mass index (BMI), blood pressure and periodontal status were analyzed.

Findings

A total of 690 participants were included in the study. A risk scoring system including five variables was developed. It exhibited fair discrimination (area under the curve = 0.72, 95%CI 0.68–0.71). The risk score value of 9 was used as the cut-off point for increased risk of abnormal HbA1c. Subjects that had a total risk score of 9 or more had a high probability of having abnormal HbA1c and were identified for referral to physicians for further investigation and diagnosis.

Originality/value

A risk score to predict hyperglycemia using a dental parameter was developed for convenient evaluation in dental clinics.
