Abstract

Purpose

The purpose of this paper is to identify barriers to seeking help among abused Iranian women.

Design/methodology/approach

This qualitative research was carried out using content analysis approach. The participants were 24 married women who were selected from health care centers, recreational centers, and two universities in Tehran, Iran. Semi-structured interviews were used to collect the data.

Findings

During data analysis, four themes emerged that explained barriers to seeking help in the participants: “fear of negative consequences of help-seeking,” “lack of resources,” “beliefs and attitudes,” and “indirect experiences of not being supported.”

Originality/value

This study has practice and policy implications for promoting help-seeking among abused women. Based on the results, to promote help-seeking, intervention plans must focus on empowering women to improve their education and employment, eliminating attitudes and beliefs hindering help-seeking, and fortifying formal and informal support systems for abused women.

Abstract

Purpose

Insufficient physical activity leads to an increase in chronic diseases. Skills-based health education methods are supposed to be more successful than traditional methods to promote healthy behaviors. Skills-based health education is an approach to create healthy lifestyles and skills using participatory methods. The purpose of this paper is to examine the effects of skill-based education method on physical activities of female adolescents.

Design/methodology/approach

This was an experimental study on 80 female public high school students aged 12-14 years old in Tehran. The participants were randomly divided into two groups: experimental group of 40 girls who were educated using “Skills-based education method,” and control group of 40 girls who were educated by lecture as a routine educational method for health promotion. Data were collected using a valid and reliable questionnaire to assess physical activity-related knowledge, attitudes and behavior. Data were gathered three times; before, immediately after (Phase 1) and two months after educational intervention (Phase 2). Data analysis was performed using SPSS 20.

Findings

Intra-group comparison demonstrated that both educational methods can increase knowledge, attitude and behavior of the students after intervention comparing to before intervention (RMANOVA; p<0.05). However, the comparison between groups demonstrated a significant higher increase of knowledge (t-test; p<0.01 and p<0.0; phase 1 and 2, respectively), and behavior (t-test; p<0.01 and p=0.002; phase 1 and 2, respectively) but not attitude (p=0.083 and p=0.125, phase 1 and 2, respectively) among the students participated in the experimental “skilled based health education” group rather than the control “lecture” group.

Originality/value

Skill-based health education comparing to routine lecture-based education creates deep and long lasting knowledge and behavior and is highly recommended for promoting physical activity and other health-related knowledge and among adolescents.

Abstract

Purpose

Improving hospital service access and mothers' care are said to be the best approaches for decreasing maternal mortality. This study aims to evaluate prenatal care (PNC) and suggest ways to improve hospital and health center maternity services.

Design/methodology/approach

This was a cross‐sectional descriptive study to evaluate prenatal care services in three domains: service structure; care process; and outcomes. Using non‐randomized quota sampling, 600 PNC clients were assessed in 12 pre‐natal clinics. Two checklists were used to assess facilities and care process and a questionnaire to assess client satisfaction. Validity and reliability were assessed and confirmed.

Findings

Six hundred subjects, averaging 29.3 (±9.4) weeks pregnancy, were included. Assessing different care processes demonstrated the following compliance to standards: counseling process 55.5 per cent (±21.2); history‐taking 48.71 per cent (±11.4); vital signs assessment 53.4 per cent (±10.6); general examination 30.2 per cent (±13.3); obstetrical examination 91.8 per cent (±27); blood tests 93.8 per cent (±21.9); urinary tests 86.9 per cent (±26.3); clients' education about peri‐natal‐risk 39.7 per cent (±27.1). Results showed that 0.54 per cent (±13.1) of clients were satisfied with care processes.

Research limitations/implications

Clients and providers were informed about the study's evaluation processes, which may have affected the results.

Practical implications

Study method and results can be used to improve PNC services.

Originality/value

This is the first time a study has focused on Iranian PNC structures, processes and outcomes.

Abstract

Purpose

Quality improvement of reproductive health care has been announced as one of five global strategies to accelerate progress toward reproductive health goals. The World Health Organization emphasises the evaluation of structure, procedure and outcome of health services to improve quality of care. This study aims to assess the quality of provided care in labour and delivery units in two selected Kordestan Medical Science University hospitals.

Design/methodology/approach

A descriptive study methodology was utilised to assess the quality of care provided to 96 women with normal pregnancies. Two checklists were used to observe procedures of care and structure together with a questionnaire utilised to assess satisfaction ratings of patients. Data were analysed by SPSS 11.5.

Findings

Midwifery care was provided in different stages of labour, with the following mean percentages of compatibility with desirable situation: first stage of labour (71.4 per cent), second stage of labour (63.03 per cent), third stage of labour (80.63 per cent) and first 2 hours after labour (70.50 per cent). The lowest scores were related to the domains of “emotional support”, “hand wash” and “assessment of vital signs”.

Originality/value

The paper develops instructions for care provision or promotion of partograph use for continuous monitoring and evaluation of quality of care by managers. Increasing midwifery personnel and providing facilities for accompanied people to improve quality of emotional care can lead to quality improvement, and finally the women's health and satisfaction.

Abstract

Purpose

Quality of family planning services is now considered as a global concern and importance. The purpose of the present study is to assess quality of family planning services in the urban health centers of Shahid Beheshti Medical Science University.

Design/methodology/approach

This was a descriptive study to assess structure, process and outcome of the family planning care services. A total of 15 health centers of Shahid Beheshti Medical Science University were randomized and the quality of provided care by 65 family planning care providers of these health centers were observed and assessed using checklists. A total of 75 family planning clients were interviewed to assess their knowledge about their selected family planning method all of whom completed the related forms of satisfaction with the services. The tools for data collection were two observational checklists, and two questionnaires with subsequent data analyzed using SPSS 11.5. In total 75 clients with an average age 24 (7±4) (mean±SD) participated in the study. The provision of family planning services by 65 providers was assessed by observation at three intervals. Of the providers, 60 (92.3 per cent) were midwives.

Findings

Mean satisfaction score of clients was 83.3±9.05 percent meaning clients were highly satisfied with the services. Mean knowledge score of clients about their family planning method of use was 59.1±18 percent, which shows their moderate knowledge about their method of use.

Originality/value

A more advanced tool to assess quality of care of family planning services with more focus on special care for the contraceptives was developed. Quality of care in family planning services of Shahid Beheshti Medical Science University health centers showed adequate facilities and equipment and to have trained personnel however their client's knowledge about the selected method was moderate. Therefore, interventional programs are needed to improve counseling and the educational process of clients, which should be considered in future research and planning of the programs.

Abstract

Purpose

Using a health belief model (HBM), this study aims to assess the knowledge, attitudes and practices of Iranian midwives in relation to HIV/AIDS protection behavior and to determine the needs of interventional programs for promotion of the behavior among midwives of maternity care units.

Design/methodology/approach

This was a cross‐sectional study in five selected hospitals in Isfahan. All 58 midwifery personnel of maternity wards of these hospitals participated in the study. Tools for data collection were a checklist to assess midwives' practice and a questionnaire to assess knowledge, attitude, and the HBM of midwives about HIV/AIDS‐protection methods.

Findings

A total of 58 midwifery personnel with average working experience of 10.92±7.98 years were assessed in the study and with a high knowledge, positive attitude and moderate practice about HIV/AIDS protection methods. The midwives perceived two main barriers, which impacted on their self‐efficacy and their protection behavior. These barriers were the emergency conditions of the work and the low availability of protective equipment.

Originality/value

HIV/AIDS protection behavior and HBM of midwives can be promoted by overcoming management barriers such as inadequate midwifery personnel in emergency conditions and insufficient protective equipment. The behavior also needs to be promoted by educational interventions which focus on improving midwives' perceived risk of HIV/AIDS infection.
