Abstract

Purpose

Academic healthcare systems face great challenges in coordinating services across a continuum of care that spans hospital, community providers, home and chronic care facilities. The Johns Hopkins Community Health Partnership (J-CHiP) was created to improve coordination of acute, sub-acute and ambulatory care for patients, and improve the health of high-risk patients in surrounding neighborhoods. The paper aims to discuss this issue.

Design/methodology/approach

J-CHiP targeted adults admitted to the Johns Hopkins Hospital and Johns Hopkins Bayview Medical Center, patients discharged to participating skilled nursing facilities (SNFs), and high-risk Medicare and Medicaid patients receiving primary care in eight nearby outpatient sites. The primary drivers of the program were redesigned acute care delivery, seamless transitions of care and deployment of community care teams.

Findings

Acute care interventions included risk screening, multidisciplinary care planning, pharmacist-driven medication management, patient/family education, communication with next provider and care coordination protocols for common conditions. Transition interventions included post-discharge health plans, hand-offs and follow-up with primary care providers, Transition Guides, a patient access line and collaboration with SNFs. Community interventions involved forming multidisciplinary care coordination teams, integrated behavioral care and new partnerships with community-based organizations.

Originality/value

This paper offers a detailed description of the design and implementation of a complex program to improve care coordination for high-risk patients in an urban setting. The case studies feature findings from each intervention that promoted patient engagement, strengthened collaboration with community-based organizations and improved coordination of care.

Abstract

Drawing on accounts from 22 lesbian couples with children conceived using donor insemination, this chapter explores how the respondents’ selection of parent terms, such as “momma” and “mommy,” influences day-to-day negotiation of parenthood. Term selection was affected by personal meanings respondents associated with terms as well as how they anticipated terms would be publicly received. Couples utilized personalized meanings associated with terms, such as terms used by families of origin or reflected in a parent’s cultural background, to help non-biological mothers feel comfortable and secure in their parenting identities. Some families also avoided terms that non-biological mothers associated too strongly with biological motherhood and felt uncomfortable using for themselves. Families also considered whether parent terms, and subsequently their relationships to their children, would be recognizable to strangers or cause undue scrutiny to their family. However, not all of the families selected terms that were easily decipherable by strangers and had to negotiate moments in which the personal meanings and public legibility of terms came into conflict. Overall, these accounts illustrate the importance of parent terms for lesbian-parent families, and other nontraditional families, as a family practice negotiating both deeply personal meanings surrounding parent–child relationships and how these terms, and the families, are normatively recognizable in public spaces.

Abstract

Today, the telecommunication issue in Europe is possibly the most important influence in the growth and establishment of the on‐line information market in that region. Europe is behind the USA in developing cheap, publicly available switched dial‐up networks on demand such as Tymnet and Telenet in America. The situation is complicated by the different regulations in each country and by the monopoly status afforded to the PTTs, the European postal, telegraph and telephone agencies. In Europe, it is illegal for a third party to access a network leased to any two parties. Thus, a circuit leased to Tymshare for communicating with its computers in Europe or the United States cannot be sold by Tymshare to any other party, although such ‘illegal’ access has been tolerated by the PTTs for some time. A number of PTTs, however, are now ‘legalizing’ the traffic by controlling third party access to Tymshare's nodes in Europe. As usual, the consumer has to pick up the tab for the added circuits, equipment, billing and overhead required for such control. It might be that, as the European countries develop their own networks, they will be motivated to encourage traffic by reducing charges and simplifying access. In the meantime On‐Line Review will monitor the scene which should really be the concern of the telecommunications experts, leaving our readers to get on with the business of accessing information efficiently and without restrictions.
