Abstract

Purpose

Following the development of a service that consisted of a “single point of contact” to coordinate end-of-life care (EoLC), including EoLC facilitators and an urgent response team, we aimed to explore whether the provision of coordinated EoLC would support patients being cared or dying in their preferred place and avoid unwanted hospital admissions.

Design/methodology/approach

Using a realist evaluation approach, the authors examined “what worked for whom, how, in what circumstances and why”. Multiple data were collected, including activity/performance indicators, observations of management meetings, documents, satisfaction survey and 30 interviews with service providers and users.

Findings

Advance care planning (ACP) increased through the first three years of the service (from 45% to 83%) and on average 74% of patients achieved preferred place of death. More than 70% of patients avoided an emergency or unplanned hospital admission in their last month of life. The mechanisms and context identified as driving forces of the service included: 7/7 single point of contact; coordinating services across providers; recruiting and developing the workforce; understanding and clarifying new roles; and managing expectations.

Research limitations/implications

This was a service evaluation and the outcomes are related to the specific context and mechanisms. However, findings can be transferable to similar settings.

Practical implications

“Single point of contact” services that offer coordinated EoLC can contribute in supporting people to be cared and die in their preferred place.

Originality/value

This paper provides an evaluation of a novel approach to EoLC and creates a set of hypotheses that could be further tested in similar services in the future.

Abstract

THE Reference Department of Paisley Central Library today occupies the room which was the original Public Library built in 1870 and opened to the public in April 1871. Since that date two extensions to the building have taken place. The first, in 1882, provided a separate room for both Reference and Lending libraries; the second, opened in 1938, provided a new Children's Department. Together with the original cost of the building, these extensions were entirely financed by Sir Peter Coats, James Coats of Auchendrane and Daniel Coats respectively. The people of Paisley indeed owe much to this one family, whose generosity was great. They not only provided the capital required but continued to donate many useful and often extremely valuable works of reference over the many years that followed. In 1975 Paisley Library was incorporated in the new Renfrew District library service.

Abstract

Purpose

This paper aims to report on a qualitative study that explores senior citizen consumers ' empowerment through social engagement in third places and their subsequent loyalty to third places.

Design/methodology/approach

Data were collected via a qualitative research design from four seniors ' clubs in Australia using focus groups (12), and participant observation. The data were analyzed using QSR NVivo software using an interpretive approach.

Findings

Ten themes based on 34 codes were identified in the study, which were grouped to develop a conceptual model of the antecedents and consequences of seniors ' empowerment in third places for further testing. The themes under consumer empowerment were important in assisting to understand the significance of place and social engagement within the place to empower seniors with a positive self-image, better access to information and exercise of choices for smarter purchase decisions. The themes under social capital contributed to a better understanding of the ramification of social capital to marketing knowledge. This was particularly so for marketing constructs embedded in community or aggregate level issues such as value co-creation, relationship marketing, customer involvement and related outcomes. Finally the study reports three types of loyalty: cognitive, ultimate and communal loyalty, that underpinned the behavioral, attitudinal and cognitive dimension of loyalty. The findings suggested that consequent to seniors ' social capital and feeling of empowerment in third places they display loyalty to the third place.

Research limitations/implications

The present study has three theoretical implications; first, it extends knowledge into the notion of third place which underlies the broader domain of servicescape. It also extends understanding of the significance of third places in practicing consumer centric marketing through consumer empowerment. The study also contributes to understanding how third places enhance seniors ' social capital through social engagement.

Practical implications

The managerial implications suggested by the findings provide a number of aspects that managers may consider in relation to service places in three key broad categories of customer-firm interest: improve customer patronage through community engagement, improve local business practices via customer-owner friendship, and redesigning spatial settings to deliver meaningful customer experiences.

Originality/value

This paper uses the concepts of social engagement within customer community in third places for the development of social capital and empowerment. It provides a customer centric focus to servicescape and incorporates recent works on third places, empowerment, social capital and loyalty.

Abstract

Purpose

For leadership and management of Western health systems, good quality relationships are a fundamental cornerstone of organising health and social care (H&SC) delivery, delivering benefits across organisations and communities. The purpose of this paper is to explore the extant management, H&SC literature, grounded in older people care, reveal behaviours, processes and practices that if readily identified across a context will support healthy relationships across the “whole system” of stakeholders.

Design/methodology/approach

An academic/practitioner group designed and guided a scoping literature review of the H&SC and broader management literature to identify and extract important behaviours, processes and practices underlying the support of high-quality relationships. A search strategy was agreed and key health and management databases were interrogated and 51 papers selected for inclusion. Working with the practitioners, the selected papers were coded and then organised into emergent themes.

Findings

The paper outlines the relational behaviours, processes and practice elements that should be present within an older peoples care community, to support a healthy relational environment. These elements are presented under the five emergent literature themes of integrity, compassion, respect, fairness and trust. These five topics are examined in detail. A way forward for building statements using the review material, that may be applied to reveal relational patterns within older people care, is also explored and outlined.

Research limitations/implications

All literature reviews are subject to practical decisions around time, budget, scope and depth restraints. Therefore potentially relevant papers may have been missed in the review process. The scoping review process adapted here does not seek to make any major considerations with regards to the weighting of evidence behind the primary research.

Originality/value

This paper contributes to a growing need for designers of health systems to more fully understand, measure and draw on the value of relationships to help bridge the gap between diminishing resources and the expanding demand on H&SC services.

Abstract

Purpose

This paper aims to investigate the effect of mining supervisor behaviour, safety motivation and perceived job insecurity on Ghanaian underground miner’s safety citizenship behaviour.

Design/methodology/approach

The authors proposed a conceptual framework that tested supervisor behaviour as an independent variable, safety motivation as a mediator variable, perceived job insecurity as a moderator variable and safety citizenship behaviour as a dependent variable. The authors tested the hypothesized relationships using 351 valid responses collected through a structured questionnaire using hierarchical regression analysis.

Findings

Results revealed that both components of supervisor behaviour significantly influenced safety motivation and safety citizenship behaviour. Furthermore, safety motivation could mediate the relationships between both components of supervisor behaviour and safety citizenship behaviour. Also, perceived job insecurity failed to moderate the relationship between safety motivation and safety citizenship behaviour.

Originality/value

This current study is vital for managerial practices. The complex conceptual framework also contributes to offering different ways of understanding how supervisors’ behaviours can catalyze improvement or worsen safety outcomes.

Abstract

The completeness of a vegetarian diet is often questioned. Nevertheless, the literature indicates that a well balanced vegetarian diet is often healthier since the prevalence of prosperity diseases is significantly lower in vegetarians compared to omnivores. A total of 24 vegetarian students were examined regarding food intake, several critical blood parameters, physical performance and anthropometrical measures. The vegetarian population was compared with 24 omnivorous students for the same parameters. A lower energy intake for the vegetarians compared to the non‐vegetarians was found. The distribution of the three macronutrients differed significantly between the two populations. Vitamin intake was comparable and within the recommended daily intake. A similar profile was found for the mineral intake. The blood profile showed significantly lower vitamin B12 values for the vegetarians compared to the non‐vegetarians but values were within the recommended limits. Physical performance and anthropometric were completely comparable between the two populations.
