Abstract

Purpose

Surgical robot systems have been used in single-port laparoscopy (SPL) surgery to improve patient outcomes. This study aims to develop a vision robot system for SPL surgery to effectively improve the visualization of surgical robot systems for relatively complex surgical procedures.

Design/methodology/approach

In this paper, a new master-slave magnetic anchoring vision robotic system for SPL surgery was proposed. A lighting distribution analysis for the imaging unit of the vision robot was carried out to guarantee illumination uniformity in the workspace during SPL surgery. Moreover, cleaning force for the lens of the camera was measured to assess safety for an abdominal wall, and performance assessment of the system was performed.

Findings

Extensive experimental results for illumination, control, cleaning force and functionality test have indicated that the proposed system has an excellent performance in providing the visual feedback.

Originality/value

The main contribution of this paper lies in the development of a magnetic anchoring vision robot system that successfully improves the ability of cleaning the lens and avoiding the blind area in a field of view.

Abstract

Purpose

Previous studies indicate adherence to pre-operative antibiotic prophylaxis guidelines has been inadequate. The purpose of this paper is to determine adherence rates to current perioperative antibiotic prophylaxis guidelines in gynecologic surgery at a tertiary care, academic institution. As a secondary outcome, improving guidelines after physician re-education were analyzed.

Design/methodology/approach

A retrospective chart review (2,463 patients) was completed. The authors determined if patients received perioperative antibiotic prophylaxis in accordance with current guidelines from the America College of Obstetricians and Gynecologists. Data were obtained before and after physician tutorials. Quality control was implemented by making guideline failures transparent. Statistical analysis used Fisher’s exact and agreement tests.

Findings

In total, 23 percent of patients received antibiotics not indicated across all procedures. This decreased to 9 percent after physician re-education and outcome transparency (p<0.0001). Laparoscopy was the procedure with the lowest guideline compliance prior to education. The compliance improved from 52 to 92 percent (p<0.0001) after re-education.

Practical implications

Gynecologic surgeons overuse antibiotics for surgical prophylaxis. Physician re-education and transparency were shown to enhance compliance.

Originality/value

Educational tutorials are an effective strategy for encouraging physicians to improve outcomes, which, in turn, allows the healthcare system a non-punitive way to monitor quality and mitigate cost.

Abstract

Purpose

Previous research indicates that vitamin D and omega-3 co-supplementation may benefit overall health, but current evidence regarding its effects on lipid profile remains unclear. The present systematic review and meta-analysis aimed to examine the effects of vitamin D and omega-3 co-supplementation on lipid profile (total cholesterol [TC], low-density lipoprotein [LDL], triglyceride [TG] and high-density lipoprotein [HDL]) in adults.

Design/methodology/approach

In this systematic review and meta-analysis, relevant studies were obtained by searching the PubMed, Scopus and Web of Science databases (from inception to January 2022). Weighted mean differences and 95% confidence intervals were estimated via a random-effects model. Heterogeneity, sensitivity analysis and publication bias were reported using standard methods.

Findings

Pooled analysis of six randomized controlled trials (RCTs) revealed that vitamin D and omega-3 co-supplementation yielded significant reductions in TG (p = 0.631). A pooled analysis of five trials indicated a significant association between omega-3 and vitamin D treatment and reductions in TC (p = 0.001) and LDL (p = 0.001). Although, pooled analyses of omega-3 and vitamin D did not significantly affect HDL.

Originality/value

The findings suggest that vitamin D and omega-3 co-supplementation lowers TG, TC and LDL in adults. Future, large-scale, RCTs on various populations are needed to elucidate further beneficial effects of vitamin D and omega-3 co-supplementation on lipid profile and establish guidelines for clinical practice.

Abstract

Purpose

This study aims, a systematic review and meta-analysis, to evaluate the effects of folic acid (folate) on patients with depression.

Design/methodology/approach

Related articles were found by searching PubMed, SCOPUS, Web of science and Cochrane’s Library, from inception to January 2022. Weighted mean differences (WMD) were pooled using a random-effects model. Heterogeneity, sensitivity analysis and publication bias were reported using standard methods.

Findings

Pooled analysis of six randomized controlled trials revealed that folic acid supplementation decreased the depression score in the Beck Depression Inventory (WMD: −3.9; 95% CI: −5.3 to −2.4, p < 0.001) compared with control group, without heterogeneity (I2 = 0.0%, p = 1.000). It also lowered the depression score in the Hamilton (HAM) Depression Inventory (WMD: −3.5 mg/dL; 95% CI: −4.6 to −2.4, p < 0.001) compared with control group, with moderate heterogeneity (I2 = 71.8%, p = 0.007). Moreover, subgroup analysis showed that the folic acid supplementation reduced HAM in all subgroups. Meta-regression analysis demonstrated that there is no evidence of a significant linear relationship between dose and duration of folic acid supplementation and changes in HAM. Also, based on the non-linear dose response, no evidence of a relationship between dose and duration of folic acid supplementation and changes in HAM was found.

Originality/value

Folic acid supplementation could possibly have an effect on lowering depression in patients. However, the clinical trials thus far are insufficient for clinical guidelines and practice.

Abstract

Purpose

The purpose of this paper is to develop the case for studying non-interaction in networks, particularly instances of intentional avoidance of interaction.

Design/methodology/approach

The paper is based on the analysis of instances of interaction avoidance across four case studies in medical technology development, food product development, food distribution network change, and regional innovation in construction.

Findings

Some answers are provided to the questions of why and how actors may seek to avoid interaction. Five modes of interaction avoidance are identified and outlined. Within these modes, interaction avoidance took place in order to protect knowledge, enforce progress, economise in business networks, avoid wasting resources, and maintain opportunities respectively. This list is not seen to be exhaustive of the theme, and further studies are encouraged.

Originality/value

Few inter-organisational network studies have dealt explicitly with interaction avoidance or non-interaction.

Abstract

Purpose

The aim of this audit was to determine if patients with a diagnosis of a suspected ectopic pregnancy had been managed in accordance with the evidence‐based guidelines policy of the obstetrics and gynaecology department at City Hospital, Birmingham. In particular, the authors wished to review the surgical management of tubal ectopic pregnancy.

Design/methodology/approach

The authors retrospectively analysed 50 cases of tubal ectopic pregnancy managed over 15 months between October 2001 and December 2002.

Findings

A total of 26 patients (52 per cent) were managed successfully by the laparoscopic approach with no major intraoperative or postoperative complications; 24 patients (48 per cent) had a laparotomy. Salpingectomy was the preferred procedure performed either laparoscopically or by traditional open surgery. A total of 30 patients (60 per cent) had their surgery where the registrar at various grades of training was the main surgeon. The estimated blood loss, the need for blood transfusion and the length of hospital stay in the laparoscopy group were significantly less than those in the laparotomy group.

Originality/value

This audit demonstrates that, in the hands of trained personnel, laparoscopic management of tubal ectopic pregnancy is more beneficial with maximum safety and efficacy.

Abstract

Purpose

The purpose of this paper is to study the performance of the approximated model of biological tissue for development of complex 3 D models. The comparison of results from the complex model of liver tissue and results from the approximated model is provided to validate the proposed approximation method.

Design/methodology/approach

The proposed model of hepatic tissue (respecting its heterogeneous character up to the microstructure of hepatic lobules) is used for analysis of current field distribution within this tissue. Initially, the complex model of tissue structure (respecting the heterogenous structure) is presented, considering its complicated structure. Consequently, the procedure for the approximation of a complex model is being described. The main motivation is the need for simple, fast and accurate simulation model, which can be consequently used within more complex modeling of human organs for investigation of negative impacts of electrosurgical equipment on heterogenic tissue structure. For these purposes, the complex and approximated model are mutually compared and evaluated.

Findings

The obtained results are exploitable for the analysis of the probability of injury formation in sensitive tissue structures, and the approximated model shall serve for optimization of complex and time-consuming analyses.

Research limitations/implications

Research limitations include development of precise and fast electro-magnetic simulation model of biological tissue.

Practical implications

Practical implications is focused on the optimization processes of the electro-surgical procedures.

Originality/value

The originality of the paper concerns the approximation method of organic tissue modeling.

Abstract

Purpose

This paper aims to illustrate the growing importance of surgical robots by providing a short historical background and details of a selection of today’s products and their applications.

Design/methodology/approach

Following a short introduction, this first provides an historical perspective. It then discusses systems used in minimally invasive surgery (MIS). This is followed by examples of products aimed at other surgical applications and, finally, brief concluding comments are drawn.

Findings

The concept of robotic surgery arose in the 1960s and trials commenced in the 1980s. The business is now in a highly dynamic phase with numerous recent product launches and a growing number late-stage developments. MIS systems are attracting much interest but robotic technology is also being applied to many additional surgical procedures.

Originality/value

This provides an insight into the emergence and commercialisation of robotic surgical systems.
