After briefly describing self‐managed integrated community teams, the authors explore potential and actual methods of evaluating their structures, processes and outcomes. Primary health care staff in three comparable sites were studied using non‐participant observation, interviews, focus groups and questionnaires. After describing the fieldwork, the authors examine integrated team structures, which are characterised by a large number of barriers that integrated teams face. Processes surrounding different working practices are explored next. Ways of unifying health care professional practice in integrated teams are suggested using evidence from both the literature and fieldwork. Outcomes that emerged after one year of the new teams’ lives are discussed in detail. The difficulty in establishing acceptable outcomes, especially the validity and reliability of outcome measures, is considered. Throughout, the positive and negative aspects of integrated teams emerging from the fieldwork are compared and contrasted with issues in the literature. Finally, recommendations are made to help strengthen integrated teams in the UK.

Diseases due to nutritional deficiencies might well be considered something from the poverty and grime of Victorian times and unknown to the people of this affluent society. It may come as a shock to many people, therefore, to learn that the rising incidence of rickets among the young in some of our big cities is causing grave concern; that iron deficiency anaemia, not altogether uncommon in women and in the undernourished but rarely of any great severity, is being found in a much more severe form in a great many West Indian infants, the hæmoglobin frequently not amounting to 50%; and that among the many skin lesions of coloured children there is at least the suggestion of riboflavin and perhaps other vitamin deficiencies. All this despite the blessings of the welfare state and a half‐century of local authority personal health services. It casts no reflection on these services, however; their work has resulted in vastly improved child health in this country, which speaks for itself.

There are four main narcotics operations in the Caribbean: drug production; the consumption and abuse of drugs; drug trafficking; and money laundering. These have numerous effects, including arms trafficking, corruption, crime, and an adverse impact on tourism, an economic mainstay of several Caribbean nations. These operations and effects have implications for several aspects of political, socio‐economic, and security conduct in the region, including the area of governance. This article examines some of the connections involving arms trafficking, corruption and governance.

The purpose of this paper is to analyze the reception and impact of Jürgen Habermas’s global academic best seller in the USA between 1974 and 2018. It specifically addresses the consequences of the long delay in the publication of the English translation of Habermas’s 1962 public sphere concept until 1989 in the context of Habermas’s paradigm shift from the Kantian ideal of a participatory democracy to a systems-theoretical interpretation of deliberative democracy, which informs Between Facts and Norms (1992/1996).

This paper can be classified as a “conceptual paper” that draws on empirical research, namely, Adrian Rauchfleisch’s (2017) bibliometric co-citation analysis of two decades of public sphere research, which features a multi-dimensional scaling of these research communities based on the distance matrix of the co-citation network.

As the 22,000 scholarly citations for structural transformation as of April 2018 already indicate, this paper confirms in detail that Habermas’s original public sphere concept attracts significantly more academic interest on an interdisciplinary basis than Between Facts and Norms, which no longer pursues a critical theory of contemporary democracy. Instead, this shift toward a uniquely sophisticated theory construction in the realm of normativity produces a work in Rechtstheorie (Thomas McCarthy) that is by definition removed from political practice. The paper demonstrates that only the criteria developed in structural transformation can be applied to the analysis of constitutional crises in the USA.

The Government's proposals for the establishment of a Greater London Council were contained in the White Paper on London Government, published last November. In the main, the Government support the recommendations of the Royal Commission, although many of these were controversial and aroused much opposition, particularly from the London County Council itself. The present two‐tier form of local government is to be replaced by a Greater London Borough as the primary unit responsible for its own area and an elected Greater London Council with larger and broader functions for a very much larger area. This will consist of the present area of the London County Council, Middlesex and parts of Hertfordshire, Essex, Kent and Surrey, coinciding with the approximate area of the metropolitan police district.
