Abstract

This bibliography is offered as a practical guide to published papers, conference proceedings papers and theses/dissertations on the finite element (FE) and boundary element (BE) applications in different fields of biomechanics between 1976 and 1991. The aim of this paper is to help the users of FE and BE techniques to get better value from a large collection of papers on the subjects. Categories in biomechanics included in this survey are: orthopaedic mechanics, dental mechanics, cardiovascular mechanics, soft tissue mechanics, biological flow, impact injury, and other fields of applications. More than 900 references are listed.

Abstract

Purpose

The purposes of this paper are to investigate the biotribological behaviour of Vitamin E-blended highly cross-linked ultra-high molecular weight polyethylene (HXL-UHMWPE) under multi-directional motion by using a CUMT II artificial joint hip simulator and compare it with HXL-UHMWPE and conventional UHMWPE.

Design/methodology/approach

The biotribological behaviour of conventional, highly cross-linked and Vitamin E-blended highly cross-linked UHMWPE acetabular cups counterfaced with CoCrMo alloy femoral head under multi-directional motion were investigated by using CUMT-II artificial hip joint simulator for one-million walking cycles. The test environment was at 36.5 ± 0.5°C and 25 per cent bovine serum was used as lubricant. A Paul cycle load with a peak of 784 N was applied; the motion and loading were synchronized at 1 Hz.

Findings

The wear resistance of Vitamin E-blended highly cross-linked UHMWPE was significantly higher than that of highly cross-linked and conventional UHMWPE. The wear marks observed from the worn surface of UHMWPE were multi-directional, with no dominant wear direction. Only abrasion occurred on the surface of Vitamin E-blended highly cross-linked UHMWPE, while yielding and accumulated plastic flow processes occurred on the surface of conventional UHMWPE and flaking-like facture and abrasion occurred on the surface of highly cross-linked UHMWPE.

Originality/value

Besides the prevention of oxidative degradation, blending with Vitamin E can also reduce the incidence of fatigue crack occurred in the surface layer of HXL-UHMWPE samples. Therefore, the wear resistance of HXL-UHMWPE under multi-directional motion can be further enhanced by blending with Vitamin E.

Abstract

Purpose

This study aims to systematically investigate the tribological behaviors of metal and polyethylene using lubricants composed of four synovial fluid (SF) components. In addition, the changes in protein conformation during wear were analyzed to establish the correlation between protein conformation and tribological properties.

Design/methodology/approach

A pin-on-disk tester with multidirectional sliding motion was used for tribological properties observation between metal and polyethylene pairs. Simulated SFs with four main constituents were used as the testing lubricants. Differential scanning calorimetry and Raman were used to characterize the changes in protein conformation during wear.

Findings

The coupling of lipids and hyaluronic acid further suppressed protein denaturation. The protein structures of the adsorption film and the ensnared protein chains in the friction zone were maintained to a certain extent, thus improving the friction and wear of polyethylene.

Originality/value

These findings established the correlation between protein conformation and friction and wear, promoting the understanding of the lubrication mechanism of artificial joints.

Abstract

Purpose

In order to prevent adverse events during the discharge process, coordinating appropriate community resources, medication reconciliation, and patient education needs to be implemented before the patient leaves the hospital. This coordination requires communication and effective teamwork amongst staff members. In order to address these concerns, the purpose of this paper is to incorporate the TeamSTEPPS principles to develop a discharge plan that would best meet the needs of the patients as they return to the community.

Design/methodology/approach

Through a gap analysis, barriers to discharge were identified from the following disciplines: nursing, social work, physical and occupational therapy, psychology, and rehabilitation physician. To improve communication, weekly meetings and twice-weekly huddles were implemented so that concerns regarding discharge obstacles could be identified and resolved. Visibility of discharge dates were improved by use of graduation certificates in patient rooms and green ribbons on patient wheelchairs.

Findings

After implementation of this discharge intervention, length of stay was reduced providing cost savings to the hospital, patient satisfaction on HCAHP surveys improved and demonstrated patient satisfaction with the discharge process, and readmission rates improved.

Originality/value

This study demonstrated that effective teamwork and communication can improve patient safety and satisfaction during the discharge period.

Abstract

Purpose

The use of the balanced scorecard has been subject to increasing scrutiny and criticism in academic literature. The purpose of this paper is to explore the limitations of, and implications for, the Performance Assessment Framework (PAF) as a balanced scorecard approach in the NHS. Although Kaplan and Norton suggested that the balanced scorecard can be adapted for strategic performance management purposes in the public sector, this study aims to argue that such claims fail to give sufficient weight to the political context in which a public sector organization operates.

Design/methodology/approach

Semi‐structured interviews were employed to investigate the perceptions about the PAF of local managers and whether and how they incorporated central government's performance targets into their local operations within two health authorities. Furthermore, in order to examine these two health authorities' performance measurement practices, documents relating to their internal performance reports and local delivery plans were analysed.

Findings

Empirical findings drawn from local health authorities indicate that the use of the PAF was primarily for legitimacy seeking purposes rather than for rational performance improvement. For central government, the PAF was used to make the performance of the NHS visible to the public so that the public would receive the signal that central government has attempted to deliver government mandates. For local health authority managers, in order to seek legitimacy from central government, imposed performance indicators were incorporated into their local performance measurement practice. However, the use of the PAF was symbolic and ceremonial and had little impact on improving performance valued by local managers in NHS.

Originality/value

This study agrees with institutional theorists' argument that the use of performance measurement systems should take into account politics and power faced by an organization. In the NHS, performance measurement might be used by local NHS organizations primarily as a ceremonial means of demonstrating their symbolic commitment for legitimacy seeking purposes.

Abstract

Purpose

In the high-altitude, high-voltage electromagnetic interference operation environment, due to the parameters perturbation for robot control model caused by uncertainties and disturbances, and with the poor effective of the conventional proportional–integral–derivative (PID) control to parameters perturbation system, the mathematical model of power cable live operation robot joint PID closed-loop control system is established.

Design/methodology/approach

The corresponding joint motion robust PID control method is also proposed based on Kharitonov theory, the system robust stability conditions including the sufficient and necessary conditions are deduced and obtained and the solving process of robust PID control parameters stability region is provided.

Findings

Finally, the simulation research on robot joint motion PID control system is also launched in MATLAB environment based on Kharitonov theory. The results show that the conventional PID control obtains better control effect only to nominal model but is ineffective to parameter perturbation system, while robust PID obtains sound control effect to parameter perturbation system. Compared with H8 robust PID, the Kharitonov robust PID has better control effect which meet the system design requirements of joint motor quickly response, high tracking accuracy and sound stability. Finally, the validity and engineering practicability are verified by 220-kV living replacing damper operation experiment.

Originality/value

This paper has described the development of a damper replacement power cable live maintenance robot experimental prototype, which greatly improves operation efficiency and deals with the safety problem of operation in a high-voltage environment. A general manipulator motion control model of the power cable robot is established; the Kharitonov theory-based parameter perturbation robust motion control method of damper replacement robot is also obtained. Through the simulation comparison, it is verified that the Kharitonov control has more superiority for dealing with the parameter perturbation systems under the premise of ensuring the stability motion. The field experiment has further confirmed the engineering practicability.

Abstract

Purpose

The purpose of this paper is to review the causes of long waiting time in Hong Kong public hospitals and to suggest solutions in the service, organisational, systems, financial and policy perspectives.

Design/methodology/approach

The paper is a review of waiting time of public hospital services. Total joint replacement, which is one of the elective surgeries in public hospitals, is presented as a case study.

Findings

The average waiting time of semi-urgent and non-urgent patients in the accident and emergency departments of public hospitals is two hours, and that of specialist outpatient (SOP) clinics is from 1 to 144 weeks. For total joint replacement, it is from 36 to 110 months. Measures like Government subsidisation programme for the replacement surgery and employing adequate physiotherapists, Chinese medicine practitioners, clinical psychologists and nurses to reduce the waiting time are suggested. Issues concerning the healthcare system of Hong Kong, such as structural reform, service delivery model, primary care, quality and process management, and policy reviews, are also discussed.

Originality/value

The ‬over-reliance of public services has resulted in long waiting time in public hospitals in Hong Kong, particularly in the emergency services and SOP clinics. However, the consequences of long waiting period for surgical operations, though much less discussed by the media and public, can be potentially detrimental to the patients and families, and may result in more burdens to the already stretched public hospitals‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬‬.

The decision to offer joint replacement surgery should be viewed as entering into a long‐term relationship rather than a brief encounter. The rise of consumerism allied to the expansion of general practitioner fund‐holding and implementation of the ‘Patients' Charter’ has resulted in increased pressures on outpatient clinics, and the interface between primary and secondary care is central to the development of the NHS. The NHS reforms have produced guidelines on waiting times for outpatient attendances and operations, with the emphasis on a consultant‐based service. There are, however, no such guidelines for postoperative follow‐up.
