Abstract

Young children who are referred to mental health agencies because of oppositional defiant disorder (ODD) and conduct problems (CP) frequently have comorbid diagnoses or symptoms such as attention deficit disorder (ADD) with or without hyperactivity (ADHD), language/learning and developmental, or autism spectrum disorders. Research has shown that the Incredible Years Child Dinosaur programme offered to children with comorbid issues is successful at reducing behaviour problems and increasing social and emotional competence. This article examines ways in which this small group therapy programme is tailored to address the individual goals of each child so that the intervention is developmentally and therapeutically appropriate. It discusses group composition, as well as the importance of specific content and teaching methods for children with ADHD, academic and language delays and mild autism.

Abstract

Families referred to child welfare for maltreatment and neglect are frequently mandated to attend parenting programmes. Evidence‐based parenting programmes (EBPs) are under‐utilised or not delivered with fidelity for this population. The Incredible Years (IY) parenting programme is an EPB that has been proven to reduce harsh parenting, increase positive discipline and nurturing parenting, reduce conduct problems and improve children's social competence. There is also promising preliminary evidence that IY is an effective intervention for families involved in child welfare and for foster parents. This article describes how the updated IY parenting basic programme is delivered with fidelity to this population.

Abstract

This article reports on three pragmatic randomised controlled trials (RCTs) to help children at risk of conduct disorder (CD): two involved the Incredible Years (IY) BASIC parent programme and the other concerned the IY teacher programme. All three interventions took place in regular service settings in North‐ and Mid‐Wales. In all three studies, staff from the provider agency delivered the programme and participated in RCT evaluations in which participants were randomly allocated to intervention or waitinglist control conditions. After a brief introduction to research into the prevention or treatment of CD, and the issues to be considered by services in selecting and delivering interventions, this article summarises the structure, content and evidence base of the IY programmes. The three Welsh studies are briefly described before exploring what factors contributed to service participation in the trials and the benefits and lessons learned in undertaking them.

Abstract

The evidence‐based Incredible Years Teacher Classroom Management (TCM) Programme was developed to meet demands from teachers for strategies to manage disruptive behaviours in the classroom (Webster‐Stratton, 1999). This article describes the programme and reports on its first use in the UK. In the first study 23 teachers attended the five‐day classroom management programme, 20 completed the final satisfaction questionnaire and 21 participated in a semi‐structured follow‐up interview. Teachers who implemented the training in their classrooms reported satisfaction with the programme and believed that the strategies taught were effective and improved pupils' conduct. In the second study, blind observation of teacher classroom behaviour was undertaken in 21 classes: 10 teachers had received the TCM training and 11 had not. Teachers who received TCM training gave clearer instructions to children and allowed more time for compliance before repeating instructions. The children in their classes were more compliant than children in the classes of untrained teachers. The implications of these findings are discussed.

Abstract

The aim of this article is to summarise the available evidence from systematic reviews about the effectiveness of interventions to prevent or treat child physical abuse and neglect. A computerised search was undertaken of major electronic databases up to December 2005 using key search terms. Only systematic reviews were included in which the primary studies evaluated the effectiveness of targeted or indicated interventions for child physical abuse or neglect. A total of 31 systematic reviews were identified and 15 met all the inclusion criteria. They covered a range of interventions/services, including home visiting, parenting programmes, multi‐component interventions, intensive family preservation services, family‐focused casework and multi‐systemic family therapy. There was limited evidence of the effectiveness of services in improving objective measures of abuse and neglect, due in part to methodological issues involved in their measurement, but good evidence of modest benefits in improving a range of outcomes that are associated with physical abuse and neglect, including parental and family functioning and child development. The results also showed some interventions (eg. media‐based and perinatal coaching) to be ineffective with high‐risk families. The evidence provided by these reviews has clear implications for children's services in the UK and other western developed countries.

Abstract

Purpose

Adolescent problem behaviours (substance use, delinquency, school dropout, pregnancy, and violence) are costly not only for individuals, but for entire communities. Policy makers and practitioners that are interested in preventing these problem behaviours are faced with many programming options. The purpose of this review is to discuss two criteria for selecting relevant parenting programmes, and provide five examples of such programmes.

Design/methodology/approach

The first criterion for programme selection is theory based. Well-supported theories, such as the social development model, have laid out key family-based risk and protective factors for problem behaviour. Programmes that target these risk and protective factors are more likely to be effective. Second, programmes should have demonstrated efficacy; these interventions have been called “evidence-based programmes” (EBP). This review highlights the importance of evidence from rigorous research designs, such as randomised clinical trials, in order to establish programme efficacy.

Findings

Nurse-Family Partnership, The Incredible Years, the Positive Parenting Program (Triple P), Strengthening Families 10-14, and Staying Connected with Your Teen are examined. The unique features of each programme are briefly presented. Evidence showing impact on family risk and protective factors, as well as long-term problem behaviours, is reviewed. Finally, a measure of cost effectiveness of each programme is provided.

Originality/value

The paper proposes that not all programmes are of equal value, and suggests two simple criteria for selecting a parenting programme with a high likelihood for positive outcomes. Furthermore, although this review is not exhaustive, the five examples of EBPs offer a good start for policy makers and practitioners seeking to implement effective programmes in their communities. Thus, this paper offers practical suggestions for those grappling with investments in child and adolescent programmes on the ground.

Abstract

Purpose

Evidence‐based practices (EBPs) are increasingly being implemented in real‐world settings. While intervention effectiveness is dependent on fidelity, interventions are often adapted to service settings according to the needs of stakeholders at multiple levels. This study aims to examine the naturalistic implementation of The Incredible Years (IY) parenting programme in a residential substance abuse treatment programme for pregnant and parenting women.

Design/methodology/approach

The study took place in a residential substance abuse treatment programme serving pregnant and parenting women and their children. Participants included 120 female clients. The primary IY facilitator was a master's level counselling psychologist. In person observations of IY sessions were completed by a trained bachelor's level anthropologist. Ethnographic field notes were collected and then coded in keeping with a priori themes and to identify emergent themes. The Parent Group Leader Checklist was used to evaluate quality and integrity of the IY basic parent programme.

Findings

Quantitative analyses indicate that fidelity varied by type of checklist activity. Specifically, adherence to the IY programme was highest in beginning topic activities, setup, and home activity review, and lowest in role play, vignettes, and wrap‐up activities. Qualitative analyses revealed a number of adaptations in implementation of IY. Adaptations fit into two broad categories: modification of programme delivery and modification of programme content. Within each of these categories modifications included organisation‐driven adaptations, provider‐driven adaptations, and consumer‐driven adaptations.

Practical implications

Changes to evidence‐based practice generally take two forms – adaptations consistent with model intent and theoretical approach and changes that represent drift from core elements of the EBP. The challenge for implementation science is to develop frameworks in which models can be adapted enough to make them viable for the service context (or the service context adapted to fit the model), yet avoid drift and maintain fidelity. Attending to the complexities of adaptation prior to and during implementation in a planned way is likely to help organisations better utilise EBPs to meet their unique needs while maintaining fidelity.

Originality/value

The paper shows that identification of types of intervention adaptations and drift allows for consideration of systematic approaches, frameworks, and processes to increase adherence during EBP implementation in community mental health and substance abuse treatment settings.

Purpose

Bereaved parents often face the complex situation of managing their own grief while parenting bereaved children who are at increased risk of social, emotional and behavioural difficulties. The current evaluation was a pilot study aimed at determining the feasibility of the Incredible Years (IY) BASIC parenting programme as an intervention for bereaved families.

Design/methodology/approach

A total of seven parents of children (aged four to 12) participated in a 12‐week IY BASIC parenting programme. Participants completed a range of pre‐intervention, post‐intervention, six month follow‐up measures and semi‐structured interviews.

Findings

The results showed statistically significant reductions in parental stress, parental wellbeing, child behaviour problems, and grief.

Originality/value

There is strong evidence for the effectiveness of IY with diverse parenting populations but this is the first known study of the programme with bereaved families. The findings support its use with such families where child behaviour is a concern.

Abstract

Purpose

The purpose of this paper is to explore whether the changes in externalising behaviour for young aggressive children differ between two evidence-based parent training (PT) programmes after treatment. The treatment formats between these programmes differ, and the authors were particularly interested in whether this influenced the results for participants with co-occurring problems (child variables such as heightened levels of attention and internalising problems, and parental variables such as marital status and education) and the consequent additional risk of poorer treatment outcomes.

Design/methodology/approach

A comparison of the individual treatment programme “Parent Management Training – Oregon model” (PMTO) and the group intervention programme “The Incredible Years” (IY) basic training sessions. Outcomes were explored in matched samples from two earlier Norwegian replication studies. The participants were matched on pre-treatment characteristics using a quasi-experimental mis-matching procedure.

Findings

There were no significant differences between the two interventions in parent ratings of externalising behaviours and the lack of differing effects between the two treatments remained when the co-occurring risk factors were introduced into the analyses.

Research limitations/implications

The participants were matched on pre-treatment characteristics using a quasi-experimental mis-matching procedure.

Practical implications

A possible implication of these findings is that parents should be allowed to choose the treatment format of their preference. Further, individual PT may be more appropriate in rural settings with difficulties in forming group interventions.

Social implications

Treatment effects did not differ between these two evidence-based interventions.

Originality/value

To the best of the knowledge independent comparisons of two evidence-based PT interventions are not previously conducted.

Abstract

Both the government and local service providers in the UK are becoming increasingly aware of the possibility of improving child outcomes through the delivery of parenting programmes. Government initiatives, such as Sure Start, Pathfinders and Flying Start are a positive step forward, yet programmes sometimes fail to work in service settings. This article describes the components necessary to deliver effective interventions, exploring the need to choose an evidence‐based parent programme, implement it with fidelity and evaluate the outcome. It describes the steps taken in North and Mid Wales to do this and reports briefly on the successful outcomes achieved by delivering the Incredible Years Basic Parenting Programme to the parents of high‐risk children in Sure Start areas.
