Abstract

Purpose

The purpose of this paper is to reveal the problems associated with unsatisfactory delivery of services in the current healthcare system of China as experienced by patients of diverse social‐economic backgrounds, including service quality, accessibility and affordability.

Design/methodology/approach

Summarizing the developmental problems of China's contemporary healthcare system, this article is principally a literature review to investigate interdisciplinary issues, including the role of social medical insurance agency, continuous quality improvement and performance measurement system.

Findings

The problems associated with unsatisfactory results of the output from the current healthcare system include the level of quality of services, accessibility and affordability, experienced by the patients of various social economic backgrounds. Developing an integrated system to provide adequate performance measurement complementary with the existing compliance emphasis is suggested, in order to uphold a continuous improvement initiative.

Research limitations/implications

This article is based on interdisciplinary literature review, examining current problems in China's healthcare system and qualitative analysis of the phenomenon. No data are collected to examine the problems identified. In‐depth, ethnographic studies would enable further understanding about the hurdles in the development of a quality healthcare system.

Practical implications

Provides a practical framework for stakeholders to develop an integrated performance measurement system to rationalize resource allocation process that enhances continuous quality improvement.

Originality/value

This paper suggests the adoption of an approach of management science in dealing with China's healthcare system problems and that some fundamental issues found to be critical in developed countries' experience, when striving for performance improvement are not attained under China's current healthcare system. Explores the fundamental issues pertinent to China's current healthcare system and the possible use of performance measurement system for dealing with existing deficiencies.

Abstract

Purpose

The purpose of this paper is to contribute to the extant literature regarding the exploitation of digital technologies by illustrating how this type of IT can influence business process improvements in the healthcare industry.

Design/methodology/approach

The paper reports an illustrative case study for MSD Italy, the Italian subsidiary of the USA-based company Merck & Co., Inc. The group sells drugs for human use in Italy but is also active in the veterinary (MSD Animal Health) industry, with Vree Health, and in solutions and software-based services for the healthcare industry.

Findings

The results show that the adoption of digital technologies could improve the performance of main healthcare business processes, particularly those processes that can be simplified with the adoption of information technology. More specifically, digital technologies could increase efficiency and, at the same time, allow for the delivery of better quality and reduced response times, with many benefits for several stakeholders, such as national health systems, clinicians and patients.

Originality/value

Although some studies report the need for effective business processes for sustainable healthcare systems, there is a lack of literature regarding the specific implications of the adoption of such digital technologies for the business process management of healthcare firms. This paper attempts to fill in this gap.

Abstract

Purpose

Expert knowledge is an important organisational resource, and organisations need to retain the knowledge learned by experience, which can be shared as part of inter-professional learning. In a healthcare context, radio-frequency identification (RFID) and ZigBee technologies can be used together, to provide real-time information for decision support and to create a secure and reliable smart hospital management information system (SHMIS) that allows the dynamic control of objects and transforms operational processes, while minimising any potential risks to patients and staff. Currently, the RFID technology in Saudi Arabia is being solely used for the monitoring of newborn infants, and some difficulties have been encountered during the different stages of tagging and monitoring. The current system in Medina Maternity and Children’s Hospital (MMCH) uses battery-powered active RFID tags, which are expensive and require routine maintenance. This study aims to discuss the way in which the MMCH in Medina, Saudi Arabia, could be transformed into an SHMIS.

Design/methodology/approach

The extraordinary growth of RFID and ZigBee technologies has made it possible to identify, locate and track objects in various environments in real time. The RFID technology is a non-contact identification technology that is cheap and reliable but has limited range in the case of passive tags. ZigBee has greater range and lower power consumption, giving more precise location of the object’s movements (0.6 m). Passive RFID when combined with ZigBee technology can be used to improve services provided by healthcare organisations through continuous data collection and supporting real-time decision-making, by applying expert knowledge of domain experts to data produced by communication from electronic and sensor technologies.

Findings

A prototype object-tracking system using RFID and ZigBee was developed to support the knowledge transformation for knowledge reasoning for decision support (KRDS), and the outcome of this research was validated with domain experts in hospitals in Saudi Arabia. Two feasibility case studies were conducted at MMCH in Saudi Arabia, to evaluate the proposed system. A survey was also conducted to address the requirements at MMCH, and the researcher adopted a range of strategy techniques, including interviews and meetings with staff, and the setting up of communities of practise (CoPs) at the target hospital.

Research limitations/implications

This paper has investigated the transformation process of an automatic healthcare tracking and monitoring systems for the purpose of developing a smart system in Saudi hospitals. For the scope of the project, the prototype implementation was restricted to a laboratory environment, to demonstrate the proposed proof of concept. The next phase will include conducting a scale up of the system, with implementation and testing done in a real hospital environment.

Originality/value

This paper proposes a prototype application of an (SHMIS that allows the dynamic control of objects and transforms operational processes, while minimising any potential risks to patients and staff.

Abstract

Purpose

Service‐oriented architecture is becoming increasingly important for healthcare delivery as it assures seamless integration internally between various teams and departments, and externally between healthcare organizations and their partners. In order to make healthcare more efficient and effective, we need to understand and evaluate its processes, and one way of achieving that is through process modeling. Modeling healthcare processes within a service‐oriented environment opens up new perspectives and raises challenging questions. The purpose of this paper is to investigate one of these questions, namely the suitability of web service orchestration and choreography, two closely related but fundamentally different methodologies for modeling web service‐based healthcare processes.

Design/methodology/approach

The authors use a case‐based approach that first developed a set of 12 features for modeling healthcare processes and then used the features to compare orchestration and choreography for modeling part of the scheduled workflow.

Findings

The findings show that neither methodology can, by itself, meet all healthcare modeling requirements in the context of the case study. The appropriate methodology must be selected after consideration of the specific modeling needs. The authors identified usability, capabilities, and evolution as three key considerations to assist with selection of a methodology for healthcare process modeling. Further, sometimes one method will not meet all modeling needs and hence the authors recommend combining the two methodologies in order to harness the benefits of modeling healthcare processes in a service‐oriented environment.

Originality/value

Although literature exists on process modeling of web services for healthcare, there are no criteria describing necessary features for micro‐level modeling, nor is there a comparison of the two leading service composition methodologies within the healthcare context. This paper provides some necessary formalization for process modeling in healthcare.

Abstract

Purpose

The purpose of this paper is to determine water availability, sanitation and hygiene (WSH) services, and healthcare waste management in Jordan healthcare facilities.

Design/methodology/approach

In total, 19 hospitals (15 public and four private) were selected. The WSH services were assessed in hospitals using the WSH in health facilities assessment tool developed for this purpose.

Findings

All hospitals (100 percent) had a safe water source and most (84.2 percent) had functional water sources to provide enough water for users’ needs. All hospitals had appropriate and sufficient gender separated toilets in the wards and 84.2 percent had the same in outpatient settings. Overall, 84.2 percent had sufficient and functioning handwashing basins with soap and water, and 79.0 percent had sufficient showers. Healthcare waste management was appropriately practiced in all hospitals.

Practical implications

Jordan hospital managers achieved major achievements providing access to drinking water and improved sanitation. However, there are still areas that need improvements, such as providing toilets for patients with special needs, establishing handwashing basins with water and soap near toilets, toilet maintenance and providing sufficient trolleys for collecting hazardous waste. Efforts are needed to integrate WSH service policies with existing national policies on environmental health in health facilities, establish national standards and targets for the various healthcare facilities to increase access and improve services.

Originality/value

There are limited WSH data on healthcare facilities and targets for basic coverage in healthcare facilities are also lacking. A new assessment tool was developed to generate core WSH indicators and to assess WSH services in Jordan’s healthcare facilities. This tool can be used by a non-WSH specialist to quickly assess healthcare facility-related WSH services and sanitary hazards in other countries. This tool identified some areas that need improvements.

Abstract

Purpose

COVID-19 was indeed a global epidemic that revolutionized the way of life, especially health-care services. The way health care will be delivered will undergo a dramatic change in the future. The aim is to analyse the increasing usage of health care systems along with digital technology and IoT especially during pandemic.

Design Methodology Approach

This research paper deals with users’ perception and their recommendation status of IoT-based smart health-care monitoring devices based on their perception, experience and level of importance to enhance the quality of life. An effective artificial neural networking (ANN)-based predictive model is designed to classify the user’s perception of usage of IoT-based smart health-care monitoring wearables based on their experience and knowledge.

Findings

The model developed has 96.7% accuracy. Among the various predictors chosen as inputs for the model, the findings indicate that self-comfort and trusted data from the device are of high priority. The present study focused only on some common factors derived from previous studies.

Research Limitations Implications

Although the performance of the proposed system was noticed to be good, the size of the sample is also limited to a few responses. Implications for future research and practices are discussed.

Originality Value

This is a novel study that aims to develop an ANN model on analyzing the user’s perception of IoT-based smart health-care wearables with the effect of COVID-19 pandemic. This paper elaborates on the ongoing efforts to restart the health-care services for survivability in the new normal situations.

Abstract

Purpose

The purpose of this paper is to predict Twitter satisfaction by healthcare professionals through integrating constructs of Csikszentmihalyi’s flow theory, quality dimensions and usefulness.

Design/methodology/approach

Survey responses of 108 physicians from a variety of specialisations in the United Arab Emirates have been validated and analysed by means of partial least squares-based structural equation modelling method using smartPLS software.

Findings

Service quality has emerged as the most influential quality dimension that positively impact flow state and perceived usefulness of Twitter, while information quality, surprisingly, does not show any effect. The findings also indicate that flow state plays a significant role in shaping physicians’ satisfaction with Twitter. The study also enhances our understanding concerning the effects of perceived usefulness on flow state and satisfaction.

Research limitations/implications

Understanding factors that influence Twitter satisfaction can help healthcare managers construct appropriate intervention strategies for maximising professional benefits of social media and minimising user resistance. This is important because top managers usually ratify traditional practices that are only of limited effect. Also, the findings help vendors to accentuate user’s concerns in addition to system functionalities in social media applications.

Originality/value

The paper is an early attempt to propose a model for social media success in a professional context in general and healthcare in particular. It also one of first studies that examine social media satisfaction through integrating contemporary information system success and acceptance models with flow theory.

Purpose

Obstetric adverse outcomes (AOs) are an important topic and the use of composite measures may favor the understanding of their impact on patient safety. The aim of the present study was to estimate AO frequency and obstetric care quality in low and high-risk maternity hospitals.

Design/methodology/approach

A one-year longitudinal follow-up study in two public Brazilian maternity hospitals. The frequency of AOs was measured in 2,880 randomly selected subjects, 1,440 in each institution, consisting of women and their newborn babies. The frequency of 14 AOs was estimated every two weeks for one year, as well as three obstetric care quality indices based on their frequency and severity as follows: the Adverse Outcome Index (AOI), the Weighted Adverse Outcome Score and the Severity Index.

Findings

A significant number of mothers and newborns exhibited AOs. The most prevalent maternal AOs were admission to the ICU and postpartum hysterectomy. Regarding newborns, hospitalization for > seven days and neonatal infection were the most common complications. Adverse outcomes were more frequent at the high-risk maternity, however, they were more severe at the low-risk facility. The AOI was stable at the high-risk center but declined after interventions during the follow-up year.

Originality/value

High AO frequency was identified in both mothers and newborns. The results demonstrate the need for public patient safety policies for low-risk maternity hospitals, where AOs were less frequent but more severe.

Abstract

Purpose

The purpose of this paper is to explore the readiness of contracted and non-contracted first-level healthcare facilities in Pakistan to deliver quality maternal and neonatal health (MNH) care. A balanced scorecard (BSC) was used as the assessment framework.

Design/methodology/approach

Using a cross-sectional study design, two rural health centers (RHCs) contracted out to Aga Khan Health Service, Pakistan were compared with four government managed RHCs. A BSC was designed to assess RHC readiness to deliver good quality MNH care. In total 20 indicators were developed, representing five BSC domains: health facility functionality, service provision, staff capacity, staff and patient satisfaction. Validated data collection tools were used to collect information. Pearson χ2, Fisher’s Exact and the Mann-Whitney tests were applied as appropriate to detect significant service quality differences among the two facilities.

Findings

Contracted facilities were generally found to be better than non-contracted facilities in all five BSC domains. Patients’ inclination for facility-based delivery at contracted facilities was, however, significantly higher than non-contracted facilities (80 percent contracted vs 43 percent non-contracted, p=0.006).

Practical implications

The study shows that contracting out initiatives have the potential to improve MNH care.

Originality/value

This is the first study to compare MNH service delivery quality across contracted and non-contracted facilities using BSC as the assessment framework.
