Abstract

Purpose

Although resilience is heavily studied in both the healthcare and organizational change literatures, it has received less attention in healthcare information technology (HIT) implementation research. Healthcare organizations are consistently in the process of implementing and updating several complex technologies. Implementations and updates are challenged because healthcare workers often struggle to perceive the benefits of HITs and experience deficiencies in system design, yet bear the brunt of the blame for implementation failures. This combination implores healthcare workers to exercise HIT resilience; however, how they talk about this construct has been left unexplored. Subsequently, this study explores healthcare workers' communicative constitution of HIT resilience.

Design/methodology/approach

Twenty-three physicians (N = 23), specializing in oncology, pediatrics or anesthesiology, were recruited from one healthcare organization to participate in comprehensive interviews during and after the implementation of an updated HIT system DIPS.

Findings

Thematic analysis findings reveal physicians communicatively constituted HIT resilience as their (1) convictions in the continued, positive developments of newer HIT iterations, which marked their current adaptive HIT behaviors as temporary, and (2) contributions to inter-organizational HIT brainstorming projects in which HIT designers, IT staff and clinicians jointly problem-solved current HIT inadequacies and created new HIT features.

Originality/value

Offering both practical for healthcare leaders and managers and theoretical implications for HIT and resilience scholars, this study's results suggest that (1) healthcare leaders must work diligently to create a culture of collaborative HIT design in their organization to help facilitate the success of new HIT use, and (2) information technology scholars reevaluate the theoretical meaningfulness a technology's spirit and reconsider the causal nature of a technology's embedded structures.

Abstract

Purpose

This paper reports on an action research study based optimization project related to healthcare IT implemented on the Faroe Islands. The aims were to study what constitutes value in the public healthcare setting by applying and activating existing resources in the organization, hence answering the overall research question: How can a resource-based view (RBV) improve benefits management (BM) practices?

Design/methodology/approach

By applying a RBV to findings from an action research study of an optimization project of an integrated health information system (HIS), a framework of capabilities needed in a public HIS setting to create value was developed.

Findings

The theoretical contribution is a framework explaining how BM practices and, hence, value can be interrelated in a public healthcare IT system.

Research limitations/implications

The study shows the need for academic IT professionals to structure and facilitate value generation, especially in the form of creating an innovative and learning environment in the form of an action research based project.

Practical implications

This study suggests which actors should be motivated and developed in order to ensure value in healthcare IT projects. Having value creation in mind, the model could have potentially broad applicability in a variety of healthcare IT settings.

Social implications

The findings leads to better usage of public healthcare resources.

Originality/value

The present research studies real problems in a real setting, thus providing distinct ideas on how to improve public value creation by direct engagement of researchers.

Abstract

Purpose

This paper aims to assess the business value realised from radio frequency identification (RFID)‐enabled healthcare transformation projects as compared with other industries. The paper starts with a review of RFID technology in the healthcare industry and further extends to an in‐depth analysis of mini‐case studies collected from RFID Journal, a leading professional journal dedicated to RFID technology, in order to identify the major benefits of the implementation of RFID systems as well as its business value achieved. In addition, there is further analysis that is being carried out on other industries to have an overview of the benefits of RFID implementation.

Design/methodology/approach

Key findings on RFID systems are being obtained from a thorough review of case studies collected from the RFID Journal's database – which for this purpose, provided 20 cases from the healthcare industry and 49 cases from other industries – so as to assess the business value of RFID‐enabled healthcare transformation projects.

Findings

Implementation of RFID systems in healthcare resulted in enhanced automational, informational and transformational effects that helped to eliminate paper‐based processes, manual processes and low visibility of patients, staff, equipments and data, etc. Such a transformation definitely gave rise to high financial performances, patient satisfaction and better decision quality on their treatments, which in turn provided a high control, co‐ordination and planning of the healthcare organisation. Further, the results showcase the business value of RFID technology and the benefits gained within the healthcare sector, in comparison with other industries. Overall, this case analysis has indicated that implementation of RFID clearly produced evident effects at the process level of an organisation, thus leading to substantial gains at the organisational level.

Originality/value

This paper delivers a review of case studies on RFID‐enabled transformation projects and process innovations, mainly in the healthcare industry, albeit it extends to other industries. For this reason, a review on RFID technology is being discussed while 69 cases are being scrutinised. The results from this paper therefore provide significant evidence of RFID‐enabled healthcare and organisational business value.

Abstract

Purpose

The purpose of this article is to present a conceptual model that posits the strategic relationships between information technology, clinic operations and physicians and the subsequent outcomes to patients, physicians and clinics which can lead to competitive advantages in the healthcare environment.

Design/methodology/approach

This paper is based on a review of the literature and proposes a conceptual model of the strategic relationships essential for success. The scope of the paper is based on the legal, economic and political triggers impacting the strategic relationship between electronic medical records, clinic interoperability and physicians as owners/users.

Findings

The paper presents the formation of a conceptual model which identifies the strategic alignment between clinics, physicians and information technology, more specifically, electronic medical records.

Research limitations/implications

This paper is limited in that it is not an empirical investigation but a conceptual model of future research endeavours. Future research endeavours should seek empirical findings related to the relationships proposed in the model.

Practical implications

Physicians, clinics and patients should be aware of the impact electronic medical records have on the health environment as well as the potential competitiveness due to health consumerism enabled by electronic medical records.

Social implications

Electronic medical records, personal health records and electronic health records are infiltrating society; subsequently health consumers should determine how this technology may impact their healthcare.

Originality/value

The value of this paper is to provide a conceptual model as a basis for future empirical research and awareness of changes in the competitiveness of the healthcare environment.

Abstract

Purpose

Healthcare organisations are often described as less innovative than other organisations, since organisational culture works against innovations. In this paper, the authors ask whether it has to be that way or whether is possible to nurture an innovative culture in a healthcare organisation. The aim of this paper is to describe and analyse nurturing an innovative culture within a healthcare organisation and how culture can support innovations in such a healthcare organisation.

Design/methodology/approach

Based on a qualitative case study of a healthcare unit that changed, within a few years, from having no innovations to repeatedly generating innovations, the authors describe important aspects of how innovative culture can be nurtured in healthcare. Data were analysed using inductive and deductive analysis steps.

Findings

The study shows that it is possible to nurture an innovative culture in a healthcare organisation. Relationships and competences beyond healthcare, empowering structures and signalling the importance of innovation work with resources all proved to be important. All are aspects that a manager can influence. In this case, the manager's role in nurturing innovative culture was very important.

Practical implications

This study highlights that an innovative culture can be nurtured in healthcare organisations and that managers can play a key role in such a process.

Originality/value

The paper describes and analyses an innovative culture in a healthcare unit and identifies important conditions and strategies for nurturing innovative culture in healthcare organisations.

Abstract

Purpose

Health Insurance Portability and Accountability Act implementation in the USA caused waves in the medical world about documentation storage, flow and access. Protecting patients from information falling into the wrong hands is admirable, but the Act has influenced more than just documentation; it has slowed the research process and complicated basic US medical care. This article aims to discuss Health Insurance Portability and Accountability Act's effects on documentation and patient care and future US healthcare options.

Design/methodology/approach

A chronological approach is used to lay out the Act's effects. Using process flow maps, the pre‐ and post‐Act environment is analyzed to discover differences in the two processes. Then a critique of the new environment leads to future movement recommendations by the US government and the healthcare industry.

Findings

True to the US government's track record, by the time the Act was passed, it was already outdated in terms of IT management capabilities. In addition to trying to comply with these outdated practices, the Act's wording is so vague that hospital staff are not sure with what they are even complying. The Act could be improved with some simple changes to wording and updating.

Research limitations/implications

This article attempts to take a massive problem with far reaching implications, drill down to the key issues and make managerial recommendations based on findings. This provides a more detailed problem view that can only be understood at a high level owing to its complexity. Importantly, the key issues developed in the article support US government reform for legislation, which is not an easy task. There were studies available on the Act's cost to patients, hospitals, clinics and general costs in the USA. However, all the research was site specific and easily contradicted by other sources. Additionally, source reliability was questionable at best, as publications came from specific hospitals and clinics.

Practical implications

Throughout the study two themes were clear – the Act's outdated nature and vague wording. The more research that was done, the more confusing the information began to get, it seems even experts have a hard time understating and complying with the Act. One thing is clear. The Act is confusing and outdated. Because the problem is so large and fragmented, people are not sure where to start fixing the predicament. Arming US hospitals, clinics and doctors with basic knowledge can give them a common springboard to start changing the current environment.

Originality/value

It is clear that the problem is large and confusing. Consolidating research results seems a valuable tool to help understand what is wrong with US healthcare. This article makes a case that updating and improving the directive's ambiguous nature helps create a less frustrating US healthcare system.

Abstract

Purpose

In the healthcare management domain, there is a lack of knowledge concerning the role of resilience practices in improving patient safety. The purpose of this paper is to understand the capabilities that enable healthcare resilience and how digital technologies can support these capabilities.

Design/methodology/approach

Within- and cross-case research methodology was used to study resilience mechanisms and capabilities in healthcare and to understand how digital health technologies impact healthcare resilience. The authors analyze data from two Italian hospitals through the lens of the operational failure literature and anchor the findings to the theory of dynamic capabilities.

Findings

Five different dynamic capabilities emerged as crucial for managing operational failure. Furthermore, in relation to these capabilities, medical, organizational and patient-related knowledge surfaced as major enablers. Finally, the findings allowed the authors to better explain the role of knowledge in healthcare resilience and how digital technologies boost this role.

Practical implications

When trying to promote a culture of patient safety, the research suggests healthcare managers should focus on promoting and enhancing resilience capabilities. Furthermore, when evaluating the role of digital technologies, healthcare managers should consider their importance in enabling these dynamic capabilities.

Originality/value

Although operations management (OM) research points to resilience as a crucial behavior in the supply chain, this is the first research that investigates the concept of resilience in healthcare systems from an OM perspective, with only a few authors having studied similar concepts, such as “workaround” practices.

Case overview

Practo Technologies Pvt Ltd is a company established in 2008 by entrepreneurs Shashank, N.D. along with Abhinav Lal. At Practo Technologies engineers practice to create technology that supports healthcare industry. In India, there is not enough assistance given to the patients in terms of searching for appropriate doctors and fixing an appointment with them easily. The patient's data lie scattered in reports and the patient's medical history goes unrecorded. Practo provides a robust platform for doctors and patients to collaborate together under one umbrella. It provides patients with open basket of doctor profiles and special search based on doctor's specialty. The case showcases the journey of transformation healthcare sector in India has gone through. The change was resisted, it never came that easy! The case explores issues in implementing information technology for clinics and hospitals in India. This case illustrates interesting facts Indian healthcare and the preparation they need to equip with to manage global technology tide.

Expected learning outcomes

The case can be used for teaching service innovation. Managing innovative practices and low-cost online business models is illustrated. In particular, it deals with how the early start up employs innovation to set up new working ideas. The case is designed to stimulate discussions of broad array of issues regarding deployment of IT services specifically in the hospitals and clinics across India. Abstract

Purpose

Service‐oriented architecture is becoming increasingly important for healthcare delivery as it assures seamless integration internally between various teams and departments, and externally between healthcare organizations and their partners. In order to make healthcare more efficient and effective, we need to understand and evaluate its processes, and one way of achieving that is through process modeling. Modeling healthcare processes within a service‐oriented environment opens up new perspectives and raises challenging questions. The purpose of this paper is to investigate one of these questions, namely the suitability of web service orchestration and choreography, two closely related but fundamentally different methodologies for modeling web service‐based healthcare processes.

Design/methodology/approach

The authors use a case‐based approach that first developed a set of 12 features for modeling healthcare processes and then used the features to compare orchestration and choreography for modeling part of the scheduled workflow.

Findings

The findings show that neither methodology can, by itself, meet all healthcare modeling requirements in the context of the case study. The appropriate methodology must be selected after consideration of the specific modeling needs. The authors identified usability, capabilities, and evolution as three key considerations to assist with selection of a methodology for healthcare process modeling. Further, sometimes one method will not meet all modeling needs and hence the authors recommend combining the two methodologies in order to harness the benefits of modeling healthcare processes in a service‐oriented environment.

Originality/value

Although literature exists on process modeling of web services for healthcare, there are no criteria describing necessary features for micro‐level modeling, nor is there a comparison of the two leading service composition methodologies within the healthcare context. This paper provides some necessary formalization for process modeling in healthcare.

Abstract

Purpose

The purpose of this paper is to contribute to the extant literature regarding the exploitation of digital technologies by illustrating how this type of IT can influence business process improvements in the healthcare industry.

Design/methodology/approach

The paper reports an illustrative case study for MSD Italy, the Italian subsidiary of the USA-based company Merck & Co., Inc. The group sells drugs for human use in Italy but is also active in the veterinary (MSD Animal Health) industry, with Vree Health, and in solutions and software-based services for the healthcare industry.

Findings

The results show that the adoption of digital technologies could improve the performance of main healthcare business processes, particularly those processes that can be simplified with the adoption of information technology. More specifically, digital technologies could increase efficiency and, at the same time, allow for the delivery of better quality and reduced response times, with many benefits for several stakeholders, such as national health systems, clinicians and patients.

Originality/value

Although some studies report the need for effective business processes for sustainable healthcare systems, there is a lack of literature regarding the specific implications of the adoption of such digital technologies for the business process management of healthcare firms. This paper attempts to fill in this gap.
