Abstract

Purpose

The purpose of this paper is to probe experiences of entrepreneurs in the social and health care service provision.

Design/methodology/approach

Information was collected regarding entrepreneurs’ views on the factors affecting the collaboration between public and private sectors. A sample of social and health care entrepreneurs was interviewed using open-ended questions. The interviews were transcribed and analysed using inductive content analysis.

Findings

Three main categories of factors affecting the success of partnership were identified: the nature of partnership, business aspects and tension builders.

Research limitations/implications

The research was undertaken in rural Finland and the sample consisted 13 entrepreneurs. The results must be considered as observations with more generalised conclusions.

Practical implications

The results of this study support municipalities in their social and health care service strategy work and especially in consideration of how to also facilitate a fruitful public-private partnership (PPP)-framework, which will largely depend on mutual understanding and consensus.

Originality/value

The reform of the social and health care system has raised intensive public debate throughout Europe. Key issues include the reorganising of social and health care processes as well as PPPs in provision of services. This study observes the views and experiences of private entrepreneurs and points out where some potential problems and solutions of social and health care PPPs are.

Abstract

Purpose

Partnership is both a goal and an approach to family-centered care (FCC). Family members play an important role alongside the health-care team when an older family member is admitted to the hospital. Family involvement in care for an older person forms a partnership approach where health professionals and the family engage collaboratively in care. This enhances the quality of care and family satisfaction with care. The purpose of this paper is to highlight the potential areas of partnerships of family members with health-care professionals while caring for older people based on the perspective of FCC.

Design/methodology/approach

A literature review was carried out.

Findings

The findings of this study focus on how healthcare professionals can listen to, respect the perspectives of family members, and share useful information with the family while caring for an older person. Family participation in providing care and collaboration between healthcare professionals and families is a seminal goal strategy in caring for older people during hospitalization. It is helpful to family members as a way of training and preparing them to assist their loved one after hospital discharge. Furthermore, it can establish a good relationship between healthcare professionals and families.

Originality/value

Partnership between health-care professionals and families helps and supports the older people and the family in managing the health condition the following discharge from the hospital.

Abstract

Purpose

The research analyzes good practices in health care “management experimentation models,” which fall within the broader range of the integrative public–private partnerships (PPPs). Introduced by the Italian National Healthcare System in 1991, the “management experimentation models” are based on a public governance system mixed with a private management approach, a patient-centric orientation, a shared financial risk, and payment mechanisms correlated with clinical outcomes, quality, and cost-savings. This model makes public hospitals more competitive and efficient without affecting the principles of universal coverage, solidarity, and equity of access, but requires higher financial responsibility for managers and more flexibility in operations.

Methodology/approach

In Italy the experience of such experimental models is limited but successful. The study adopts the case study methodology and refers to the international collaboration started in 1997 between two Italian hospitals and the University of Pittsburgh Medical Center (UPMC – Pennsylvania, USA) in the field of organ transplants and biomedical advanced therapies.

Findings

The research allows identifying what constitutes good management practices and factors associated with higher clinical performance. Thus, it allows to understand whether and how the management experimentation model can be implemented on a broader basis, both nationwide and internationally. However, the implementation of integrative PPPs requires strategic, cultural, and managerial changes in the way in which a hospital operates; these transformations are not always sustainable.

Originality/value

The recognition of ISMETT’s good management practices is useful for competitive benchmarking among hospitals specialized in organ transplants and for its insights on the strategies concerning the governance reorganization in the hospital setting. Findings can be used in the future for analyzing the cross-country differences in productivity among well-managed public hospitals.

Abstract

Purpose

This study aims to provide insights into the public–private partnerships (PPP) concept and its performance measurement in the health-care sector, identifying and refining critical success factors, including the perceived quality of health care, as evidenced by patient satisfaction and policy requirements for successful PPP implementation.

Design/methodology/approach

This theoretical study explores the existing literature on the relationship between service quality and patient satisfaction, to propose a culture-specific conceptual model interlinking the drivers of patient satisfaction with PPP. The in-depth theoretical research focuses on the qualitative performance indicators of PPPs, as well as their corresponding peripheral factors.

Findings

The research presents theoretical evidence that the concept of patient satisfaction can only be viewed through a multifactor perspective that incorporates demographics of patients, perceived service quality factors and emotions. It is found that significant improvements in service quality and patient satisfaction do, indeed, emphasize the effective role of PPP in hospitals.

Practical implications

The theoretical model is based on a comprehensive set of both cognitive and affective determinants. And considering these, as well as their causes, effects and interrelations, sets the foundations for testing and for further research to develop. Moreover, the outcomes of this study can be used as a theoretical base for the development of a PPP qualitative performance measurement framework.

Originality/value

This study attempts to fill the gap in knowledge on service quality and patient satisfaction as qualitative indicators for hospital performance after and toward PPP, while setting explicit factors and opening clear research avenues for further studies to follow.

Abstract

Purpose

The purpose of this paper is to describe the process of transformation in the Northern Health region of British Columbia (BC), Canada, based on a new relationship between governments of BC, Canada, and First Nations.

Design/methodology/approach

Written from the perspective of four key participants in the transformation process, this paper highlights the ways in which partnership has been integral to the transformation of health service delivery to First Nations communities in Northern BC.

Findings

In sharing their experiences with health system transformation through partnership, the authors of this paper hope to contribute to a growing set of promising practices as indigenous health service organizations take a greater role in health service provision, and non-indigenous health service organizations improve their understanding of and relationships with indigenous communities.

Originality/value

This paper outlines a unique and leading edge transformation in health service delivery, told from the perspectives of key partners involved in the transformation process.

Abstract

Purpose

Nongovernmental organizations (NGOs) play a critical role in modern health-care systems complementing state organizations. The purpose of this paper is to investigate the challenges of NGOs' participation in Iran's health-care system.

Design/methodology/approach

This qualitative study was conducted in 2020. Data were collected through semi-structured interviews with 26 participants: 11 managers and experts at the office of NGOs in the Ministry of Health (MOH) and universities of medical sciences and 15 chief executive officers (CEOs) of health-related NGOs in Tehran, Iran. The participants were chosen using the non-probabilistic approach of purposive sampling with maximum variation. Data analysis was performed using the thematic analysis method and MAXQDA 10 software.

Findings

Challenges of NGOs' participation in Iran's health-care system are categorized into external and internal challenges. Financial issues, the process of getting permissions, lack of trust in NGOs, weak relationships with the public sector and lack of law are identified as main challenges. Building trust in NGOs, passing the NGOs' law, making people more familiar with NGOs and capacity development are the most important measures that improve the role of NGOs and resolve barriers to their participation in Iran's health-care system.

Originality/value

In this study, for the first time, the challenges of NGOs' participation in Iran's health-care system are investigated by examining the views of both the public sector and NGOs. Improving the participation of NGOs in the health-care system provides Iran's MOH with the opportunity to make use of the NGOs' capacities to eliminate public health issues.

The Commission on Accreditation of Rehabilitation Facilities (CARF) has experienced considerable market growth in recent years. Growth has occurred in the health care industry with exceptional growth occurring in the fields of persons with disabilities and children’s services. Expansion of their services beyond the American boarders has resulted in CARF accrediting organizations in Canada, Ireland and Sweden with active work occurring in Denmark, Finland, France, Scotland, Italy, England and Australia. In Canada, policy makers at all levels of government began demanding greater community involvement in consumer service delivery. Policy makers and consumers made it clear that a system of accountability needed to be incorporated to ensure quality of service. In order to address the resulting growth in Canada and listen to the needs of consumers it became apparent that a separate office was required to meet the unique needs of Canadians. CARF Canada was established to meet the needs.

Abstract

Purpose

This chapter focuses on an innovative effort in the Italian context in which a complex web of partnerships was created as the foundation of an alternative model of health care. More specifically, the start-up of a health-care organization – Welfare Italia Servizi (WIS) – is analyzed and discussed with respect to its sustainability.

Design/methodology/approach

The process of organizing a sustainable health care is analyzed through the theoretical lenses of multi-stakeholders management and partnership perspectives.

The possibility of developing dense knowledge about the WIS’s case has stemmed from our collaboration with the organization board with regard to a research process intended to monitor the organizational start-up and its sustainability challenges.

Findings

The case provides new insights into the dynamic nature of building multi-stakeholder partnership in a complex environment; the developmental life-cycle challenge of multi-stakeholder partnership, and the meaning of sustainability. The case suggests a tapestry of issues such as how sustainability may be “paradoxical,” dynamic, led by different and sometimes conflicting logics, and changeable over time like a growing tree in an intricate forest.

Originality/value

The case can stimulate learning and discussions both within the community of practitioners and the community of academics with respect to which promising conditions could help address the challenge of starting-up a sustainable organization in the health-care field.

The quality and relevance of what happens in prisons are measurably improved once prisons are seen, and have to act, as an integral part of society and the community. The provision of health care has benefited a great deal from the involvement of national and local agencies whose professional expertise is in health and education. HM Inspectorate of Prisons (HMIP) has developed the World Health Organisation's concept of a ‘healthy prison’ (WHO, 1998) to judge the treatment of prisoners and the conditions in which they are held against four core criteria. Partnership working between the Prison Service and NHS was formalised in April 2000, and the majority of prisons have transferred the commissioning of health care to the NHS. This paper considers the challenges and developments of partnership working in improving health outcomes for prisoners, and identifies future improvements.
