Abstract

Purpose

Despite HIV being reported as one of the major global health issues, availability and accessibility of HIV services and supplies remain limited, especially in low- and middle-income countries. The effective and efficient operation of HIV supply chains is critical to tackle this problem. The purpose of this paper is to give an introduction to HIV supply chains in low- and middle-income countries and identify research opportunities for the operations research/operations management (OR/OM) community.

Design/methodology/approach

First, the authors review a combination of the scientific and grey literature, including both qualitative and quantitative papers, to give an overview of HIV supply chain operations in low- and middle-income countries and the challenges that are faced by organizing such supply chains. The authors then classify and discuss the relevant OR/OM literature based on seven classification criteria: decision level, methodology, type of HIV service modeled, challenges, performance measures, real-life applicability and countries covered. Because research on HIV supply chains in low- and middle-income countries is limited in the OR/OM field, this part also includes papers focusing on HIV supply chain modeling in high-income countries.

Findings

The authors conclude this study by identifying several tendencies and gaps and by proposing future research directions for OR/OM research.

Originality/value

To the best of the authors’ knowledge, this paper is the first literature review addressing this specific topic from an OR/OM perspective.

Abstract

Roger Osayende, a former management consultant, must advise the Ministry of Health of Ektu, a fictional country in Central Africa, on how to implement a new point-of-care diagnostic test for infants with HIV. In Ektu, mothers often transmitted HIV infection to infants during pregnancy, delivery, or breastfeeding due to inadequate resources to invest in prevention efforts. The existing procedure to diagnose infants with HIV required collecting dried blood samples at more than two hundred healthcare facilities around the country and transporting them to a central laboratory in the capital for testing. This process was characterized by significant delays due to long transportation times, batching of samples in transportation and processing in the lab, and concomitant congestion in the lab. This delay resulted in loss to follow-up, that is, lost patients due to mothers not collecting their infants' results. A new point-of-care device was about to be introduced, which would obviate the need for this centralized processing and the resulting diagnostic delay. The key decision under consideration is where to place the devices to maximize their effectiveness.

Abstract

Purpose

This study examines the performance effect of working capital for a large sample of Indian manufacturing firms in light of supply chain disruption, i.e. the COVID-19 pandemic.

Design/methodology/approach

This study is based on secondary data collected from the Prowess database on Indian manufacturing firms listed on the Bombay Stock Exchange (BSE) 500. Panel data regression analyses are used to estimate all models. Moreover, this study has employed robust standard errors to consider for heteroscedasticity concerns.

Findings

The results challenge the current notion of working capital investment and reveal that higher working capital has a positive and significant impact on firm performance. Further, it highlights that Indian manufacturing firms suffered financially post-COVID-19 as they significantly lack the working capital to run day-to-day operations.

Originality/value

This research contributes to the scant literature by examining the association between working capital financing and firm performance in light of the COVID-19 pandemic, representing typical developing economies like India.

Corporate Social Responsibility (CSR), in the form of corporate philanthropy or charity, has been practiced in the United States since the late 1800's (Sethi, 1977). Today's concept of CRS originated in 1953 with the publication of Bowen's book entitled “Social responsibilities of Businessmen”. In his book Bowen asked the question: “What responsibilities to society can business people be reasonably expected to assume?” At this time, the emphasis was placed on business people's social conscience, rather than on the company itself. Further on the academics became much more precise in defining the firms’ responsibilities. Carroll (1999) divided companies’ responsibilities into economic, legal, ethical, and philanthropic. Lantos (2002) narrowed down CSR to ethical, altruistic, and strategic responsibility. According to Davis (1973, p. 312) CSR refers to a company's concern for “issues beyond the narrow economic, technical and legal requirements of the firm.”

Study level/applicability

This case is designed for teaching entrepreneurship at master’s level. Depending on students’ interests and exposure levels however, it can be applied to teaching undergraduate entrepreneurship courses that are taken after at least the two basic entrepreneurship courses.

Case overview

Nigeria’s tomato industry is one of the most dysfunctional in the entire nation’s economy. Although the country is West Africa’s largest tomato producer, nearly half of the produced tomatoes rot on the way to the market, which makes Nigeria heavily reliant on imported tomato paste. Amidst growing concerns among stakeholders of the need to address the dysfunction of the tomato industry, Tomato Jos emerged as the earliest social impact venture in the tomato paste industry. Nigeria’s changing macro-economic conditions clearly call for a tomato processing industry and the entry of Tomato Jos is well timed. Within a span of two years, the company successfully raised $600,000 in equity, debt and grant financing that has catered for start-up expenses and expansion to 150 hectares of farmland. The company plans on raising an additional $25m to cater for their planned Stage III growth. Amidst growing excitement over the entry of Tomato Jos in the industry with a social enterprise, Africa’s top business tycoon, Aliko Dangote, announced entry into the tomato processing industry with a major tomato processing facility in the same region as Tomato Jos. The Dangote Group of companies is seen as very tough competition to contend with, due to their sheer size, political leverage and financial capital. This case study primarily teaches how multiple aspects of start-up entrepreneurship may be handled rather than being taught separately as is often the case in mainstream business education. Entrepreneurs in the field rarely confront real challenges in this way. This case study introduces a practice of teaching a collection of key aspects of entrepreneurship, their nuances and inter-relationships in an integrated fashion.

Expected learning outcomes

The overarching objective of this case is to teach students how to interpret the shifts in industry position resulting from the entry of an important competitor within the larger context of growing a social venture. At the end of the case study analysis, students will be able to analyze the effects of changing forces and conditions in a country’s business environment on a start-up social enterprise; interpret the shifts in a venture’s industry position after the entry of an important competitor; identify appropriate funding sources and financing strategies to fuel the growth of a social enterprise; identify areas of a start-up’s business model that need improvement and/or iteration to support faster growth; and develop an effectuation-based strategy for a growing venture.

Abstract

Purpose

The purpose of this paper is to explore opinions of health workers on priorities and challenges of the HIV/AIDS commodities management system in Abuja, Nigeria. The results from this research are to inform future guideline revisions on the management of these commodities, in order to produce the most efficient and cost‐effective approach.

Design/methodology/approach

A qualitative research was conducted from a social constructionist perspective. Purposive sampling was used to recruit 12 health workers, comprising pharmacists, laboratory scientists and logisticians involved in HIV/AIDS commodity management in Abuja, Nigeria. Views and experiences of these workers were explored on HIV/AIDS commodity management through in‐depth interviews, to ascertain challenges of the current system and needed interventions. The data were subjected to thematic content analysis.

Findings

Results of the study include the need for coordinated and integrated health commodity management system, sustainable human resource and infrastructure development and an effective data management system to address current challenges.

Research limitations/implications

The data collection approach did not provide an opportunity for idea exchange and review in a focused group and the number of respondents was limited to 12 due to time constraints. However, the respondents were experts in the field.

Practical implications

Government needs to commit financial resources to support of the health supply chain to ensure sustainability beyond donor support. Donors should change their strategy of “quick fix” approach to health supply chain challenges, as these often result in Government laxity in addressing public health needs.

Originality/value

The paper provides insight into areas on which the Government of Nigeria should focus attention in health supply chain strategy, which is currently perceived as not coordinated.

Abstract

Purpose

The primary objective of this article is to examine current procurement, inventory control and management practices in modern healthcare, with a particular focus on the procurement and management of surgical supplies in a prominent public, highly specialized healthcare sector.

Design/methodology/approach

This study was conducted in three phases. In Phase 1, the study team interacted with various hospital management stakeholders, including the surgical hospital store, examined the current procurement process and identified challenges. Phase 2 focused on selecting items for a detailed study and collected the qualitative and quantitative details of the store department of the healthcare sector chosen. A detailed study analyzed revenue, output/demand, inventory levels, etc. In Phase 3, a decision-making framework is proposed, and inventory control systems are redesigned and demonstrated for the selected items.

Findings

It was observed that the demand for many surgical items had increased significantly over the years due to an increase in disposable/disposable items, while inventories fluctuated widely. Maximum inventory levels varied between 50 and 75%. Storage and availability were important issues for the hospital. It is assumed the hospital adopts the proposed inventory control system. In this case, the benefits can be a saving of 62% of the maximum inventory, 20% of the average stock in the system and optimal use of storage space, improving the performance and productivity of the hospital.

Research limitations/implications

This study can help the healthcare sector administration to develop better systems for the procurement and delivery of common surgical items and efficient resource allocation. It can help provide adequate training to store staff. This study can help improve management/procurement policies, ordering and delivery systems, better service levels, and inventory control of items in the hospital business context. This study can serve as a pilot study to further investigate the overall hospital operations.

Practical implications

This study can help the healthcare sector administration develop better systems for procuring and delivering common surgical items and efficient resource allocation. It can help provide adequate training to store staff. This study can help improve management/procurement policies, ordering and delivery systems, better service levels and inventory control of items in the hospital business context. This study can serve as a pilot study to further investigate the overall hospital operations.

Originality/value

This study is an early attempt to develop a decision framework and inventory control system from the perspective of healthcare inventory management. The gaps identified in real hospital scenarios are investigated, and theoretically based-inventory management strategies are applied and proposed.

Abstract

Purpose

The purpose of this paper is to investigate vendor managed inventory (VMI) for the supply of medicines between distributors and hospitals to identify factors that may affect VMI adoption.

Design/methodology/approach

Two contrasting VMI initiatives involving five organizations (three hospitals, one distributor and one manufacturer/supplier) are studied. A case study method with semi-structured interviews is used with triangulation in data collection, site visits and document analysis to enhance reliability and validity. The cases are analyzed and compared with respect to hospital, supplier, product and supply chain integration characteristics.

Findings

A successful public sector VMI initiative and an unsuccessful private sector VMI initiative are identified. The public sector supplier focuses on improving service level while the private sector supplier seeks to strengthen relationships with a key customer. Hospital characteristics, including type of hospital, top management perspectives and the hospital’s willingness to share information, are critical in decisions on VMI initiation or termination. Relatively stable demand products are preferred for a VMI approach. Hospitals may perceive risks in VMI adoption for medicines as it involves relinquishing control of critical supplies and may result in “lock-in” with a particular supplier.

Research limitations/implications

The cases have been conducted in one country, which may affect generalization of the findings. Wider empirical evidence from other countries in both developed and less developed regions will be beneficial.

Practical implications

VMI is advocated as being beneficial in many supply contexts. However, it is challenging to implement. The study identifies factors that affect the adoption of VMI for hospital pharmaceuticals and provides guidance on initiating VMI in a hospital context.

Social implications

The potential for VMI in public health projects to enable greater access to critical medicines is highlighted.

Originality/value

The paper provides supply side and demand side perspectives on VMI adoption in an important sector. It highlights the need for greater understanding of the perceived and actual risks in VMI from the perspective of both the hospital and the supplier and for much clearer advice on which pharmaceutical products are appropriate for VMI control in a hospital context.

Public procurement in many countries is one of the most important factors in governance and is a priority target of reform. In many humanitarian situations however, service delivery cannot wait for procurement reform. The needs of many of the Millennium Development Goals are immediate, while procurement reform may take years to institutionalize. Under these circumstances, international organisations such as the United Nations have both implementation and capacity-building roles, often placing them in high-risk situations. This has led to the development of procurement risk assessment and management tools, designed to provide objectivity in country procurement risk monitoring and review, as well as assist capacity building. The procurement risk assessment methodology that follows uses established risk modeling to provide procurement risk ratings in 60 HIV/AIDS, Tuberculosis and Malaria programs in 26 countries, and is successfully promoting procurement strengthening within high-risk country offices.

Abstract

Purpose

Uganda is one of many African countries struggling to develop adequate healthcare, particularly in regard to local treatment and access to drugs. The purpose of this paper is to contribute to the understanding of how reducing supplychain complexity can improve health in developing countries.

Design/methodology/approach

This study and evaluation included 50 interviews and 27 site visits of the public healthcare system in Karamoja, northeast Uganda. A mapping of drug‐supply chains was undertaken to identify causes of stock shortages and possible solutions. A model for logistics process redesign was used for the analysis. Results were quantified with use of a simple tool developed for this specific purpose.

Findings

The main conclusion is that less supplychain complexity can produce higher customer service in terms of less stock shortages, while keeping costs down. By reducing lead times and uncertainty, increasing order frequencies, and moving order points and safety stocks, there may be better integration between information and goods flows and bottlenecks in the supply chain may be reduced.

Research limitations/implications

While the empirical study is extensive, there are uncertainties in the data that must be taken into account. The effects of the suggested solutions remain to be analyzed and documented upon implementation.

Practical implications

The study was rooted in a practical problem and provides practical solutions for developing countries and agencies providing aid.

Social implications

Stock shortages of life‐saving drugs are a general problem in countries with lack of financial and technical infrastructure. Improvements will impact the lives of many people.

Originality/value

The paper provides an understanding of the applicability of traditional logistics principles in a new context. It provides the academic community with a much‐needed in‐depth understanding of humanitarian logistics. The approach can be used in other studies.
