Abstract

Purpose

The purpose of this article is to describe and analyze how three principals attempting to establish effective inclusive schools for students with disabilities identified and learned from failure, with a specific focus on how failure was used to drive school improvement efforts.

Design/methodology/approach

This qualitative study involved interviews and observations during one school year in an urban school district in the southwestern United States. Participants varied in their levels of experience and their perceptions about establishing inclusion in their schools. In addition, each school ranged in the length of time working toward implementing inclusion and the relative success of their reform efforts.

Findings

This study presents insights into several types of failures with a range of causation, from what we term “unnecessary failures,” to “complex coordination failures” and “trial failures.” In response to these failures, the principals in this study attempted to foster a culture of professionalism and trust by creating avenues for relationship-building that would allow teachers and staff to buy into the inclusive mission of the school. Principals also attempted to build school capacity by working to strengthen teams and pre-existing structures and to cultivate teacher-leaders that could improve communication, motivate others and effectively lead meetings that would encourage authentic collaboration.

Originality/value

Analyzing and learning from instances of failure is an important practice, especially for failures that are unexpected, yet much of the literature on leading from failure exists in business management scholarship, with very limited examples in the area of special education and special education school leadership.

Abstract

Purpose

Educational organizations confront a number of failures along with successful practices. Although a potential learning source for organizations, failures encountered are not normally welcomed and utilized to improve future practices. However, the existing literature emphasizes that individuals and organizations can learn a lot from their failure by adopting a pragmatic understanding toward the concept of failure and implementing a learning-from-failure (LFF) approach in their practices. Drawing on these assumptions, the purpose of this study is to explore how school principals identify educational failures and implement an LFF approach in their managerial practices.

Design/methodology/approach

This study adopted a qualitative research paradigm. The research was conducted in a middle-sized city in Turkey. The data for the study came from individual semi-structured interviews conducted with 12 school principals. The interview data were coded and analyzed using a conceptual coding framework.

Findings

Findings indicated that school principals’ definitions of failure are classified as learning related and nonlearning related. Failures were generally considered to be a learning opportunity rather than a complete loss, although principals’ identification of important examples of failure varied across school levels. In operating an LFF approach at schools, certain limitations existed such as a lack of institutional policy and professional skills, heavy workload and limited autonomy.

Originality/value

This study attempted to explore domains of educational failures and the application of an LFF approach at educational organizations. The LFF approach has previously been studied and discussed in business organizations. This study applied the concept to the education field.

Abstract

Purpose

Framed within the theoretical domain of attribution theory, this study aims to investigate the antecedents of experienced regret following an entrepreneur’s business failure (defined as firm discontinuance, closure or bankruptcy) and the impact of regret on personal well-being.

Design/methodology/approach

The population of interest was business owners whose businesses had failed within the past five years. The data was collected from 319 failed entrepreneurs using an online survey. Structural equation modelling was used to test the hypotheses presented in this study.

Findings

External attribution, including economic uncertainty and contract restrictions, was positively related to feelings of regret. Considering internal attribution, due diligence had a positive effect on regret whereas customer relationship development ability can reduce feelings of regret. Moreover, prevention-focused entrepreneurs were likely to experience higher levels of regret when engaging in extensive consideration in using information. Finally, regret had a detrimental effect on the entrepreneurs’ well-being.

Research limitations/implications

The research provides fresh perspectives on experienced regret, a relatively unexplored emotion in the entrepreneurship literature. In the context of small business operations, the locus of attribution (associated with business failure) is the key influence on learning following failed business attempts.

Practical implications

This study extends current knowledge of regret in the context of entrepreneurial failure, which has a significant catalytic effect on employment and entrepreneurial mobility.

Originality/value

This research sheds light on how emotional responses are derived from an entrepreneur’s self-assessment of their performance and attribution of blame for failure.

Abstract

Purpose

The prediction of accurate failure strength and a composite laminate failure load is of paramount importance for reliable design. The progressive failure analysis helps to predict the ultimate failure strength of the laminate, which is more than the first ply failure (FPF) strength. The presence of a hole in the laminate plate results in stress concentration, which affects the failure strength. The purpose of the current work is to analyze the stress variation and progressive failure of a symmetric laminated plate containing elliptical cutouts under in-plane tensile loading. The effect of various parameters on FPF and last ply failure (LPF) strength is studied.

Design/methodology/approach

The ply-by-ply stresses around elliptical cutouts are obtained analytically using Muskhelishvili's complex variable formulation. To predict the progressive failure, Tsai–Hill (T-H) and Tsai–Wu (T-W) failure criteria are used, and depending on the mode of failure, lamina modulus is degraded.

Findings

The study has revealed that fiber orientation and stacking sequence for given loading have the most significant effect on the laminate's failure strength.

Originality/value

Complex variable method and conformal mapping are simple and proficient for studying failure analysis of a laminated plate with elliptical cutout.

Abstract

Purpose

This study aims to discover the impact of failure factors on stakeholder coordination performance (SCP) in the finishing phase of high-rise building projects.

Design/methodology/approach

Firstly, this study identifies potential failure factors affecting coordination performance as well as criteria for measuring SCP in the finishing phase of high-rise building projects. Afterwards, a survey questionnaire is designed to collect data from high-rise building projects in Vietnam. Using the factor analysis method, the study discovers the failure constructs. A structural equation model is then built to uncover the relationships between failure constructs and SCP.

Findings

The study identified four failure constructs which could significantly affect SCP, namely traditional adversarial relationship (TAR), incompetent parties (IP), poor project planning and organization (PPO) and delays of parties toward construction works (DP). The developed model indicated that TAR, PPO and IP significantly affected stakeholders' coordination performance in the finishing phase of high-rise building projects.

Originality/value

The results of the study fill the gap in knowledge by discovering the causal relationships between failure constructs and SCP in high-rise building projects. The results might provide an initial guideline for stakeholders during the finishing phase of high-rise building projects to enhance their coordination performance.

Abstract

Purpose

Despite their high number, most mergers end in failure. Academic studies of how these failures occur have remained rare, first, because of the difficulty of accessing the cases, and second, because of the difficulty of obtaining – for the purposes of qualitative analysis – objective and freely shared perceptions from the stakeholders, who tend to avoid speaking about failure. This is unfortunate, however, as failure can serve as a stimulus for organizational learning and readaptation for the future.

Design/methodology/approach

The author investigated how an organization managed failure during the post-merger integration stage. The author described the merger of two listed French companies using longitudinal data.

Findings

This in-depth case study provides new insights into failure during post-merger integration. The paper highlights the complexity of post-merger integration processes and the failures that the integration stakeholders had to address. The author underlined how they recognized failures and put into place solutions. They particularly highlighted two failures and how they were managed by the managers who acted as knowledge brokers within the new organization and by stakeholders who deconstructed the organization to ensure its future.

Research limitations/implications

The limitations are those concerning a single case study.

Practical implications

The paper identified trigger events in the merger process that prompted stakeholders to step in and manage and resolve failures during the integration period. Such triggers can be considered as steps for managers and stakeholders to solve organizational issues in the merger process. The paper highlighted the complexity of post-merger integration processes and the failures faced by integration stakeholders. The analysis thus contributes to an inclusive and integrative view of the challenges in this process.

Social implications

Despite their high number, merger and acquisition failures remain surprisingly high. This paper explored how stakeholders deal with failures by identifying which solutions are best adapted to their organization.

Originality/value

The case provides a vivid illustration of failure management during a merger process. Theoretical concepts and empirical findings from the literature are combined to present a single consistent picture.

Abstract

Purpose

This paper aims to presents a novel integrated fuzzy decision support system for analyzing the issues related to failure of a milk process plant unit.

Design/methodology/approach

Process failure mode effect analysis (PFMEA) approach was implemented to list failure causes under each subsystem/component and fuzzy ratings for three risk criteria, i.e. probability of failure occurrence (O_f), severity (S) and non-detection (O_d) are collected against the listed failure causes through experts feedback. A new doubly technique for order of preference by similarity to ideal solution (DTOPSIS) approach was implemented within fuzzy PFMEA tool for ranking of listed failure causes. The proposed decision support system overcomes the restrictions of classical PFMEA and IF-THEN rule base PFMEA approaches in an effective way.

Findings

Failure causes such as electrical winding failure (RM4), high pressure in plate region (C1), communication problem in supervisory control and data acquisition control (MS3), insulation problem (ST2), lever breakage (B2), gasket problem (D3), formation of holes (PHE5), cavitations (FP7), deposition of milk particle inside the pipeline because of improper cleaning (MHP2) were acknowledged as the most critical one with the application of proposed decision support system.

Research limitations/implications

The analysis results are based on subjective judgments of the experts and therefore correctness of risk ranking results are totally dependent upon the quality of input data/information available from these experts. However, the analyst has taken proper care for considering the vagueness of the raw data by incorporating fuzzy set theory within the proposed decision support system.

Practical implications

The proposed fuzzy decision support system has been presented with its application on milk pasteurization plant of a milk process industry. The analysis based ranking results have been supplied to maintenance manager of the plant and a consent was shown by him with these results. Once the top management of the plant took decision for the implementation of these results, the detailed robustness of the proposed decision support system could be evaluated further.

Social implications

The analysis result would be highly useful for minimizing sudden breakdowns and operational cost of the plant which directly contributes to plant's profitability. With the decrease in the chances of sudden breakdowns there would be high safety for the people working on/off the plant's site. Further, with increase in availability of the considered plant the societal daily demand related to dairy products could be easily fulfilled at reasonable prices.

Originality/value

The performance and proficiency of the proposed decision support system has been evaluated by comparing the ranking results with classical TOPSIS and VlseKriterijumska Optimizacija I Kompromisno Resenje (VIKOR) approaches based results.

Abstract

Purpose

In this paper, a maintenance strategy based on improved imperfect maintenance actions with stochastic repair times for multiperiod randomly failing equipment is developed. The main objective is to minimize the total maintenance cost by jointly finding the optimal preventive maintenance (PM) cycle and planning horizon.

Design/methodology/approach

A model based on the mathematical theory of reliability is developed to minimize the total maintenance cost by jointly finding the optimal couple: PM cycle T* and planning horizon H*. The proposed model aims to characterize the evolutionary impact of imperfect PM actions on the equipment failure rate and the resulting mean number of failures. The conventional threshold accepting (TA) algorithm is implemented to solve the proposed model. A numerical example for a given set of input parameters is presented in order to show the usefulness of the proposed model. A sensitivity analysis of some of the key parameters is performed to demonstrate the coherence of the developed maintenance policy.

Findings

The obtained results showed a sensitive trade-off between PM frequency and the total maintenance cost. Performing PM actions more frequently helps significantly to reduce the expected number of corrective maintenance actions and the corresponding total cost. It has also been found that improving the efficiency of the PM actions allows for maintaining the equipment less frequently by increasing the time between successive PM actions.

Research limitations/implications

Given the complexity of the objective function to be minimized and the stochastic nature of the model's parameters, the authors limited this study to equally cyclic production periods over the planning horizon.

Practical implications

The present model aims to provide an integrated maintenance/production comprehensive framework to assist planners in establishing maintenance schedules considering multiperiod randomly failing production systems and the evolutionary impact of imperfect PM actions on the equipment failure rate.

Originality/value

Contrary to the majority of existing works in the literature dealing with maintenance strategies, the authors consider that repair times are stochastic to provide a more realistic framework. In addition, the developed model considers the impact of imperfect maintenance on the equipment's mean time to failure. Thus, the evolutionary impact of imperfect PM actions on the equipment failure rate and the resulting mean number of failures is characterized. Simultaneously, the production planning horizon along with the length of each PM cycle is optimized in order to minimize the total maintenance cost over the planning horizon.

Abstract

Purpose

This paper presents the performance analysis of the automatic ticket vending machine (ATVM) through the functioning of its different hardware and software failures.

Design/methodology/approach

Frequent failures in the working of ATVM have been observed; therefore, the authors of the paper intend to analyze the performance measures of the same. Authors have developed a mathematical model based on different hardware and software failures/repairs, which may occur during the operation, with the help of the Markov process. The developed model has been solved for two kinds of failure/repair rates namely variable failures (very much similar to real-time failure) and constant failures. Lagrange's method and Laplace transformation are used for the solution of the developed model.

Findings

Reliability and mean time to failure of the ATVM are determined. Sensitivity analysis for ATVM is also carried out in the paper. Critical components of the ATVM, which affect the performance of the same, in terms of reliability and MTTF are also identified.

Originality/value

A mathematical model based on different hardware and software failures/repairs of ATVM has been developed to analyze its performance, which has not been done in the past.

Abstract

Purpose

The healthcare system is regarded as one of the most critical service industries. The surgical unit is the heart of hospitals in that any failures directly affect the safety of patients, so they should be managed thoroughly. It is an intricate multi-attributes decision-making problem with uncertainty. Uncertain information in the form of fuzzy sets theory has been applied widely to describe the different aspects of system uncertainty. This study aims to present a new methodology to manage the healthcare system failures due to the multi-attributes decision-making process.

Design/methodology/approach

This study introduces a new risk evaluation methodology by failure mode and effect analysis (FMEA) and MULTIMOORA method. Group decision-making process in this methodology is presented under uncertain information in the form of interval-valued hesitant fuzzy linguistic sets (IVHFLSs). IVHFLSs encompass both qualitative and quantitative interpretation of experts to reflect their preferences, as well the ability and flexibility of derivation of linguistic information by several linguistic terms increase. To avoid the different ranking order of MULTIMOORA approaches, a new interval multi-approaches multi-attribute methodology, namely, technique of precise order preference (TPOP), is extended to provide precise ranking order.

Findings

The application and precision of proposed integrated IVHFL-MULTIMOORA methodology with TPOP is examined in a case study of healthcare systems. The results indicate the superiority of proposed methodology to prioritize and assess the failures as well as handling system uncertainty.

Originality/value

This study addresses the challenges of an organization to prioritize potential failures by implementing FMEA method. Moreover, this paper contributes to making the manager's ability in decision-making. The value of this study can be discussed in two aspects. First and foremost, this study provides a new FMEA-based methodology to rank failures precisely. The results prove that the proposed methodology is more robust to changes of different ranking order methods, applied by FMEA. On the other hand, using the capability of IVHFLSs allows collecting accurate information in an ambiguous and uncertain environment.
