Abstract

Purpose

This article aims to provide an overview on key trends in public sector policy and professional development and how they intersect with gender and diversity. It seeks to explore new configurations in the relationship between gender and the professions and to develop a matrix for the collection of articles presented in this volume.

Design/methodology/approach

The authors link social policy and governance approaches to the study of professions, using the health professions and academics as case studies. Material from a number of studies carried out by the authors together with published secondary sources provide the basis of our analysis; this is followed by an introduction of the scope and structure of this thematic issue.

Findings

The findings underline the significance of public policy as key to better understand gender and diversity in professional groups. The outline of major trends in public sector professions brings into focus both the persistence of gender inequality and the emergence of new lines of gendered divisions in the professions.

Practical implications

The research presented here highlights a need for new models of public sector management and professional development that are more sensitive to equality and diversity.

Originality/value

This article focuses on the “making” of inequality at the interface of public policy and professional action. It introduces a context sensitive approach that moves beyond equal opportunity policies and managerial accounts and highlights new directions in research and policy.

Abstract

Purpose

Recent scholarship reveals the imagery of the professional as the “ideal citizen”. The linkage between professionalism and citizenship is here approached from the perspective of democratic social justice in order to examine the persistence of gendered inequalities in the health care system. The paper aims to examine the ideas framing professionalism, both in sociological theory and historically, asking what gendered hierarchies mean in modern health care systems, and why and how they persist in the conditions of liberal democracy.

Design/methodology/approach

The question is approached through both sociological literature and an analysis of historical framings of professionalism; the Finnish health care system is employed as a case. The reason for keeping the discussion close to a specific case is that different professional fields, countries and historic contexts differ from each other in democratically relevant respects.

Findings

Traditional sociological theory assumed that professional privilege was based on essentially neutral expertise that benefits democracy only if protected from bureaucracy and politics. The recent theoretical turn reframes professional knowledge as socially defined, but the destabilisation of professional knowledge claims is not without problems. The paper refers to the persisting tensions between changing governance and gendered hierarchies in health care and argues for new approaches that suggest ways through which professional expertise can be democratically represented in politics.

Originality/value

The interdisciplinary framework uses political theory on social justice to examine how health care politics frame professionalism.

Abstract

Changing market conditions, new modes of labour and decreasing legitimisation of experts, as well as an increasing ratio of women, pose new challenges to the professions. These ongoing dynamics are especially visible in the health care system – a traditional professional field with strongly formalised rules governing entrance, initiation and career paths. In addition, this field is highly segregated according to sexes. How do the bove‐mentioned processes of change present themselves and what economic, social or structural factors cause them? What role does gender play within these processes? What potential lies in the re‐structuring processes of health care systems as far as a gender equal architecture and design of professions is concerned? These and other questions are addressed in this collection of papers. For the main part they grew out of a thematic focus event organised and coordinated by the editors for the 5th Conference of the European Sociological Association (ESA) Research Network Sociology of Professions that was held in 2001 in Helsinki. Inspired by the richness of the research results on professions and gender in health care systems in various European countries and new horizons which opened up from the comparative perspective in different countries, professions, and theoretical approaches, and finally motivated by very constructive ensuing discussions, we decided to continue the discussion with a publication.

Abstract

What part does gender play in professionalism? How does gender translate into hierarchies in professions? This paper attempts to deal with these questions and to move towards an embedded approach that can account for paradoxes of gender and professions. It focuses on the notion of context and the hypothesis that gender is both stable and variable. Using empirical material from a multi‐method study of the German dental profession, three main issues are considered: the impact of different structuring modes and diverse actors on gender relations, the relation of gender difference and hierarchy, and differentiation between women as a group. Gender becomes manifest on the symbolic level as well as in structural arrangements. However, its impact on each of these levels varies. Most striking is the empirical proof that gender does not lead to the construction of hierarchies under all conditions. Female symbolism can also favour women. Professionalisation theories help to reconstruct these processes.

Abstract

Purpose

The new “agenda of choice” in public services emphasises service users' needs and agency. The ideals of consumerism and user involvement have set new challenges for professionals. This paper aims to explore the effects of consumerism and user involvement at the level of day‐to‐day service delivery, looking at the encounters between professionals and service users.

Design/methodology/approach

The paper applies an ethnographic approach. Observation data were collected at two Finnish drug treatment institutions: a needle exchange and health counselling service, and a specialised maternity clinic for pregnant women. Data from each institution consist of seven to 12 months of participant observation notes, which were subsequently systematically coded and analysed comparing the two institutions.

Findings

The promotion of new ideals does not automatically result in the empowerment of service users or the erosion of professional power. The two institutions differ greatly: institutional context and gender shape the everyday realisation of the new ideals. In both institutions, however, professionals have adopted new practices and rethought their role. The cultivation of service users' choice and agency can become valuable professional capital, a new kind of “know how” that can also be used by the professionals to justify the importance of their work.

Originality/value

The paper provides a nuanced and contextualised insight into “how” the ideals of consumerism and user involvement are translated into everyday encounters between service providers and users.

Abstract

Purpose

This paper seeks to identify different medical strategies adopted in relation to the new hospital payment systems in Germany, The Netherlands and England and analyse how the medical strategies have impacted on the emergence of these New Public Management policy tools between 2002 and 2007.

Design/methodology/approach

A comparative approach is applied. In addition to secondary sources, the study uses publications in professional journals, official publications of the (national) physician organisations and a (non‐random) expert questionnaire to obtain the views of the medical corporate bodies in the three countries.

Findings

The results reveal differences in the medical strategies in the three countries that point towards the significance of institutional and interest configurations. The Dutch corporate medical body was most willing to solve the conflict, while the German and English corporate medical bodies seem to be keen to use a strategy of confrontation. The differences in medical strategies also impact on the ways in which hospital payment systems have emerged in the three countries.

Research limitations/implications

Further research is necessary to study the medical strategies in healthcare reforms from a broader perspective, for instance by including other countries.

Originality/value

The paper gives insights into the interplay between the medical profession and the government in the context of new managerial governance practices in the hospital sector. It adds to the scholarly debates about the role of the medical profession in health policy‐making.

Abstract

Purpose

This article aims to explore the gendered dimensions of the heterogeneous path of non‐native graduates in nursing and medicine in the Portuguese National Health Service, including institutional factors like the process of credential recognition, the professional integration and internal labour market segmentation.

Design/methodology/approach

Biographical interviews with nurses and physicians (23 males and 35 females) from Spain and from some Eastern European countries who migrated to work in Portugal were conducted, and additionally semi‐structured interviews with 12 institutional participants to contextualise the experiential data.

Findings

The research highlights complex intersections between gender and the resources for action. Institutional conditions, like legal frameworks and the self‐regulatory mechanisms of the professions, and individual characteristics shape successful translation of foreign credentials into a new social and cultural context. The specific configurations may create gender traps that push women into low status professional segments and also new forms of inequality in the professions.

Research limitations/implications

The development of successful professional action and agency in a socio‐culturally different health care labour market can be differentiated by gender, place of graduation and citizenship status. The factors identified in this small‐scale qualitative study would need further investigation.

Practical implications

The findings highlight new demands for integration policies for foreign‐trained professionals, such as policies to promote equal opportunities for women and men in the workplace.

Originality/value

The article reinforces women as professional participants in the international migration process and examines the opportunities for successful integration.

Abstract

Purpose

The purpose of this paper is to present information on gender studies and feminist approaches in Germany and cross‐national perspectives on the challenges and chances of changing science systems.

Design/methodology/approach

The paper is report on an international conference in Germany “Re‐visioning the future. Perspectives in gender studies”. It discusses selected topics – race, class, gender and gender and professions – to highlight the need for and benefit of an international debate on gender studies.

Findings

International and comparative research into gender expands the scope of gender studies both internationally and in one country.

Originality/value

The paper adds new geopolitical perspectives to a debate on gender studies and equal opportunities in academia that is shaped by Anglo American research.

Abstract

Purpose

This article aims to address topical issues in the debate on gender (in)equality in higher education in Europe and beyond, and highlights future perspectives of research and practice.

Design/methodology/approach

It is based on a conference report of the 5th European Conference on Gender Equality in Higher Education that was hosted by Humboldt University in Berlin, Germany, 28‐31 August 2007.

Findings

Number matters (“wore women into science”), but quality matters more: more women are needed in top positions in higher education, science, technology and engineering, and more female participation in higher education decision‐making processes. It is necessary to reveal gender biases of recent strategies in higher education. Exclusion happens also among women; an intersectional approach is therefore called for, which includes primary and secondary educations as well as labour market and careers in the academe.

Practical implications

Equality programmes have to consider the diversity of gender in higher education and to strengthen women's lobbying in European education and science politics.

Originality/value

This conference brought together about 350 participants from 56 countries. Thus, it provided an excellent opportunity for knowledge exchange with European scholars and beyond, which is reported in this paper.
