Abstract

This chapter more clearly identifies the distinction between Electronic Health Record (EHR) and Electronic Medical Record (EMR), and states their value in obtaining individual-level data. Synthetic medical records may be used as a surrogate for EHR data in order to ensure digital data privacy is maintained during the development of the LHS. Synthea is an open-source simulation tool available through GitHub.1 Extensive descriptive analysis of synthesized data is provided as a foundation for the analysis in Chapter 7.

Abstract

Purpose

This study aims to reveal the topic structure and evolutionary trends of health informatics research in library and information science.

Design/methodology/approach

Using publications in Web of Science core collection, this study combines informetrics and content analysis to reveal the topic structure and evolutionary trends of health informatics research in library and information science. The analyses are conducted by Pajek, VOSviewer and Gephi.

Findings

The health informatics research in library and information science can be divided into five subcommunities: health information needs and seeking behavior, application of bibliometrics in medicine, health information literacy, health information in social media and electronic health records. Research on health information literacy and health information in social media is the core of research. Most topics had a clear and continuous evolutionary venation. In the future, health information literacy and health information in social media will tend to be the mainstream. There is room for systematic development of research on health information needs and seeking behavior.

Originality/value

To the best of the authors’ knowledge, this is the first study to analyze the topic structure and evolutionary trends of health informatics research based on the perspective of library and information science. This study helps identify the concerns and contributions of library and information science to health informatics research and provides compelling evidence for researchers to understand the current state of research.

Abstract

Purpose

This study aims to evaluate the dementia assessment and diagnosis care provided to adults with intellectual disability. The authors selected recommendations from the National Institute for Health and Care Excellence (NICE) standards which could be evidenced in clinical notes and aimed to identify characteristics which may be associated with improved adherence to these recommendations.

Design/methodology/approach

The study population was adults with an intellectual disability who were diagnosed with dementia between January 2019 and December 2022 by a UK-based intellectual disability service. Data to demonstrate adherence to selected recommendations and demographic and clinical characteristics were extracted from electronic patient records.

Findings

The authors identified 41 individuals. A mean of six of the eight recommendations were adhered to. There was low adherence with structural imaging to support dementia subtype diagnosis (9 individuals, 22%). This may be linked with the low percentage of people diagnosed with vascular dementia (1 individual, 2%) despite a national figure of 20%. No demographic or clinical characteristics were associated with level of adherence recorded. The authors found incomplete recording of diagnostic clinical coding in electronic patient records. This may disadvantage this population, as they cannot be readily identified for post diagnostic support or resource allocation.

Originality/value

To the best of the authors’ knowledge, this is the first study to examine adherence to these NICE guidelines in this population.

Abstract

Purpose

With the function to store individual’s data input, personal health record (PHR) enhanced the accessibility to personal health information. This study aims to assess the factors that impact the intention of Malaysian internet users to use PHR and create a modified technology acceptance model (TAM) for eHealth.

Design/methodology/approach

Multivariate statistical analysis was performed on a total of 216 responses using the partial least square technique based on the cross-sectional survey among Malaysian internet users.

Findings

Behavioral intention was positively associated to PHR. Subjective norm significantly influenced both attitude and intention to use, whereas trust and perceived usefulness significantly influenced attitude. There was no significant positive impact in the relationships between compatibility and perceived ease of use and intention to use; nevertheless, they positively influenced perceived usefulness. Attitude exhibited mediating influence between trust, perceived usefulness and subjective norm and intention to use. Nonetheless, perceived risk did not affect behavioral intention. Thus, PHR acceptance was well-justified by the modified TAM in evaluating eHealth acceptance.

Practical implications

The eHealth vendors can enhance their marketing and development strategies on related products.

Originality/value

Literatures and empirical evidence on eHealth are still scarce, especially in emerging markets. The role of attitude may not be well-researched in health-care context, therefore was included in this study’s modified TAM. Critical determinants, namely, trust and risk, were added to the model.

Abstract

Purpose

Taking a business lens of telehealth, this article aims to review and provide a state-of-the-art overview of telehealth research.

Design/methodology/approach

This research conducts a systematic literature review using the scientific procedures and rationales for systematic literature reviews (SPAR-4-SLR) protocol and a collection of bibliometric analytical techniques (i.e. performance analysis, keyword co-occurrence, keyword clustering and content analysis).

Findings

Using performance analysis, this article unpacks the publication trend and the top contributing journals, authors, institutions and regions of telehealth research. Using keyword co-occurrence and keyword clustering, this article reveals 10 major themes underpinning the intellectual structure of telehealth research: design and development of personal health record systems, health information technology (HIT) for public health management, perceived service quality among mobile health (m-health) users, paradoxes of virtual care versus in-person visits, Internet of things (IoT) in healthcare, guidelines for e-health practices and services, telemonitoring of life-threatening diseases, change management strategy for telehealth adoption, knowledge management of innovations in telehealth and technology management of telemedicine services. The article proposes directions for future research that can enrich our understanding of telehealth services.

Originality/value

This article offers a seminal state-of-the-art overview of the performance and intellectual structure of telehealth research from a business perspective.

Abstract

Purpose

Blockchain systems have been proposed as a solution for exchanging electronic health records (EHR) because they enable data sharing in decentralised networks. This paper aims to analyse the user acceptability of blockchain technology in enabling EHR exchange and to formulate practical implications for increasing user acceptability.

Design/methodology/approach

A technology acceptance model [extended Unified Theory of Acceptance and Use of Technology (UTAUT) model] was used as a framework to measure the effects of 13 factors. The authors conducted a survey and analysed data from 214 participants using partial least square path modelling.

Findings

The acceptance of blockchain for EHR sharing is positively influenced by performance expectancy, social influence and perceived trust. Effort expectancy and facilitating conditions do not influence acceptance. The UTAUT model explains the variance in acceptance at 58.4%. Self-efficacy influences effort expectancy, incentives influence facilitating conditions and security predicts perceived trust.

Practical implications

Three implications are drawn: (1) Users need to clearly understand system’s purpose, functions, security mechanism and environmental impacts. (2) Users are incentivised to share health data via a blockchain solution if the technology offers personalising options and health information. (3) Health personnel can socially impact patients to use blockchain-based solutions.

Originality/value

Studies have shown that blockchain technology is a valuable solution for exchanging EHR. The novelty of this work is to identify how and why patients may accept this emerging technology for EHR exchange.

Abstract

Purpose

Today, e-government (electronic government) applications have extended to the frontiers of health-care delivery. E-Nabız contains personal health records of health services received, whether public or private. The use of the application by patients and physicians has provided efficiency and cost advantages. The success of e-Nabız depends on the level of technology acceptance of health-care service providers and recipients. While there is a large research literature on the technology acceptance of service recipients in health-care services, there is a limited number of studies on physicians providing services. This study aims to determine the level of influence of trust and privacy variables in addition to performance expectancy, effort expectancy, social influence and facilitating factors in the unified theory of acceptance and use of technology (UTAUT) model on the intention and behavior of using e-Nabız application.

Design/methodology/approach

The population of the study consisted of general practitioners and specialist physicians actively working in any health facility in Turkey. Data were collected cross-sectionally from 236 physicians on a voluntary basis through a questionnaire. The response rate of data collection was calculated as 47.20%. Data were collected cross-sectionally from 236 physicians through a questionnaire. Descriptive statistics, correlation analysis and structural equation modeling were used to analyze the data.

Findings

The study found that performance expectancy, effort expectancy, trust and perceived privacy had a significant effect on physicians’ behavioral intentions to adopt the e-Nabız system. In addition, facilitating conditions and behavioral intention were determinants of usage behavior (p < 0.05). However, no significant relationship was found between social influence and behavioral intention (p > 0.05).

Originality/value

This study confirms that the UTAUT model provides an appropriate framework for predicting factors influencing physicians’ behaviors and intention to use e-Nabız. In addition, the empirical findings show that trust and perceived privacy, which are additionally considered in the model, are also influential.

Abstract

Purpose

This study aims to investigate the working way of the e-Pulse portal in Türkiye as a sample of a next-generation digital tool for health data management. Accordingly, this study focuses on explaining the structure and key services of the e-Pulse portal in the context of health data management.

Design/methodology/approach

This study is a technical paper that will explain how the e-Pulse portal works in Türkiye. Accordingly, the data are based on secondary sources and mostly the official website of the e-Pulse portal. As a sample case, this study investigates the e-Pulse portal from Türkiye. The data are categorized by tables, and some key factors are classified based on review results.

Findings

As a result of the review of the e-Pulse portal's sample account, it is seen that the e-Pulse portal provides comprehensive data for personal health data for both individuals and healthcare professionals. By permitting healthcare professionals, users or patients can share their personal health data on specific dates and numbers whenever they need and want. When sharing recorded personal health data, citizens or patients can get more efficient healthcare service on the time.

Research limitations/implications

By giving descriptive evidence and review through the e-Pulse portal, countries with high-populated can see the key e-services and elements to manage health data through digital tools. On the other side, this study has some limitations. This study investigated the e-Pulse portal and its e-services for Türkiye and gave some findings mostly based on subjective deduction. Another digital portal can give different findings for the literature.

Practical implications

Based on the e-Pulse portal case, it is determined that by creating a digital portal with recorded personal up-to-date health data, healthcare services can be ensured more efficiently among high-populated countries in the long term. While population growth and pandemic possibilities such as COVID-19 increase throughout the world, serving more patients with these portals will increase efficiency and service quality, provided that patient information is well protected.

Originality/value

This study reveals key e-services and segments to provide personal health data management by a next-generation digital tool based on the e-Pulse portal. The main contribution of this study is expected to guide other countries when adapting next-generation technology or systems to manage health data in the future.

Abstract

Purpose

The purpose of this project was to develop research support services that address local and external research data management (RDM) support drivers within the existing organizational culture at the University of Florida. The goal was to prompt organization change to support a campus-wide electronic lab notebook.

Design/methodology/approach

This project used a mixed-methods research approach to cultivate an organizational change program that support technological infrastructure to benefit researchers. The mixed-methods research involved participation action research integrated with a stakeholder approach.

Findings

The development of the grant proposal which was unfunded led to development of continued project goals. This project confirmed the development for support for an institution-wide electronic research notebook (ERN) solution requires adherence to the summary of five key actions for developing RDM services. Failure to complete all of the key actions engenders fragmentation culture.

Research limitations/implications

This project includes implications for institutions to develop grant proposals with integrated budgets for research support services of funded projects; and to use the summary of key actions for developing RDM services articulated by Jones et al. (2013) in “How to Develop RDM Services – a guide for HEIs.” Both are need to support findable, accessible, interoperable and reusable data for researchers.

Practical implications

This project has practical implications for higher education institutions interested in leveraging socio-technical processes to advance the role of libraries as collaborator, partner and stakeholder in developing institution-wide adoption, support and training for ERN as a research support service to RDM.

Social implications

This paper contributes to the body of developing literature on ERN as support services to RDM lead by academic research libraries.

Originality/value

This project contributed to the change in organization culture resulting in the successful collaboration between the Research Office and College of Medicine to support an institution-wide ERN technological infrastructure for one year as a pilot at a large academic research institution in the southeast USA.

Abstract

Purpose

This paper aims to examine the global pattern of growth and development of eHealth research based on publication headcount, and analysis of the characteristics, of the keywords used by authors and indexers to represent their research content during 1945–2019.

Design/methodology/approach

This study adopted a bibliometric research design and a quantitative approach. The source of the data was Elsevier’s Scopus database. The search query involved multiple search terms because researchers’ choice of keywords varies very significantly. The search for eHealth research publications was limited to conference papers and research articles published before 2020.

Findings

eHealth originated in the late 1990s, but it has become an envelope term for describing much older terms such as telemedicine, and its variants that originated much earlier. The keywords were spread through the 27 Scopus Subject Areas, with medicine (44.04%), engineering (12.84%) and computer science (11.47%) leading, while by Scopus All Science Journal Classification Health Sciences accounted for 55.83% of the keywords. Physical sciences followed with 30.62%. The classifications social sciences and life sciences made only single-digit contributions. eHealth is about meeting health needs, but the work of engineers and computer scientists is very outstanding in achieving this goal.

Originality/value

This study demonstrates that eHealth is an unexplored aspect of health literature and highlights the nature of the accumulated literature in the area. It further demonstrates that eHealth is a multidisciplinary area that is attractive to researchers from all disciplines because of its sensitive focus on health, and therefore requires pooling and integration of human resources and expertise, methods and approaches.
