Abstract

Purpose

Economists have recognized knowledge management as a promising tool regarding all aspects of the economy, including health care. The volume of biomedical literature is currently growing at an exponential rate, and the vast number of studies makes it extremely difficult for researchers to keep up with new developments in their research areas. Therefore, the efficient management of huge amounts of data and the accuracy of the knowledge thus obtained are vital concerns. Electronic health (e-health) has emerged as a useful concept to provide data for solo self-care management. Although health care is a common topic on the internet, patients rarely share their health care-related knowledge on social media. This study aims to examine the impact of knowledge on e-health.

Design/methodology/approach

This paper complies with the methodological requirements of systematic literature reviews. The present paper has investigated the newest systems and studied their practical techniques in detail. The effects of knowledge on e-health have been categorized into major groups.

Findings

The outcomes indicate that the capabilities of information and communication technology certainly promote the exchange of knowledge within clinics. The results also show that institutional architectures have significant impacts on knowledge-sharing exercises, significantly improving patient safety.

Practical implications

These findings will be essential in the understanding of the interplay among various signals in theory and in the understanding of patients’ choice in the e-health community in practice. The results have implications for existing health management and e-health literature. The present paper will help policymakers, health-care executives and project managers to effectively set their operations and make them maintainable, prevent unpredicted obstacles and better allocate their resources. Overall, the results of this paper will guide researchers who are working in the field of e-health.

Originality/value

E-health attempts have mostly focused on answering questions using context-specific technical answers, regardless of the key role of knowledge resources. The present paper has provided an innovative viewpoint on how knowledge resources and knowledge-sharing initiatives may have a role in the innovative work behaviors shown by health-care employees. As noted before, there have been only a few studies regarding the effects of knowledge on health care, so the present paper contributes to the previous literature, particularly about e-health.

Abstract

Purpose

This study aims to investigate how e-health system characteristics (information quality, system quality, perceived ease of use, perceived usefulness) contribute to the enhancement of medical staff performance, patient care, and doctor–patient relationships at UNRWA-Gaza healthcare centers. It aims at testing an integrative single model comprising Technology Acceptance Model (TAM), D&M model and e-health system utilization.

Design/methodology/approach

This study followed the quantitative methodology and the deductive research approach. Data were collected from 241 medical staff who use the system employed in 19 different healthcare centers across the Gaza Strip. Partial least square/structural equation modeling technique was used to analyze the collected data and to test study hypotheses.

Findings

Study concluded that information quality of the adopted Health Information System (HIS) has both direct and indirect positive impact on staff performance, only direct positive impact on patient care and only positive indirect impact on doctor–patient relationship. System quality, on the other hand, was found to have negative direct impact and positive indirect impact on staff performance and has both direct and indirect positive impact on both doctor-patient relationship and patient care.

Research limitations/implications

Noteworthy that HIS has availability, speed and error detection and error prevention issues. It is recommended that these shortfalls be addressed together with improving user perception towards ease of use and usefulness of the system.

Practical implications

Management should also work to raise confidence in its medical staff to improve the effect of HIS on medical performance and patient care. It is also recommended that UNRWA should implement crowed management techniques such as queuing systems and on-phone booking to minimize patient waiting time.

Originality/value

The importance of the study stems from its context being conducted in a developing region (Gaza Strip-Palestine) which has a fragile economic, political and social environment with many other complexities. It is also conducted at United Nations Relief and Work Agency (UNRWA) healthcare centers, which provide medical services to Palestinian refugees. In addition, this study is among the few studies that address the impact of individual e-health success factors on both doctor-patient relationship and patient care constructs. Most previous studies concentrated on the impact of health system adoption as a whole on these two subject variables and one can hardly ever stop at studies that address effect of individual success factors on them. It also integrated both D&M system success model and TAM model with some additional amendments creating and tested a new model.

Abstract

Purpose

Health service quality is an important determinant for health service satisfaction and behavioral intentions. The purpose of this paper is to investigate requirements of e‐health services and to develop a measurement model to analyze the construct of “perceived e‐health service quality.”

Design/methodology/approach

The paper adapts the C‐OAR‐SE procedure for scale development by Rossiter. The focal aspect is the “physician‐patient relationship” which forms the core dyad in the healthcare service provision. Several in‐depth interviews were conducted in Switzerland; first with six patients (as raters), followed by two experts of the healthcare system (as judges). Based on the results and an extensive literature research, the classification of object and attributes is developed for this model.

Findings

The construct e‐health service quality can be described as an abstract formative object and is operationalized with 13 items: accessibility, competence, information, usability/user friendliness, security, system integration, trust, individualization, empathy, ethical conduct, degree of performance, reliability, and ability to respond.

Research limitations/implications

Limitations include the number of interviews with patients and experts as well as critical issues associated with C‐OAR‐SE. More empirical research is needed to confirm the quality indicators of e‐health services.

Practical implications

Health care providers can utilize the results for the evaluation of their service quality. Practitioners can use the hierarchical structure to measure service quality at different levels. The model provides a diagnostic tool to identify poor and/or excellent performance with regard to the e‐service delivery.

Originality/value

The paper contributes to knowledge with regard to the measurement of e‐health quality and improves the understanding of how customers evaluate the quality of e‐health services.

Abstract

Purpose

The purpose of this paper is to develop e-health and patient monitoring systems remotely to overcome the difficulty of patients going to hospitals especially in times of epidemics such as virus disease (COVID-19). Artificial intelligence (AI) technology will be combined Internet of Things (IoT) in this research to overcome these challenges. The research aims to select the most appropriate, best-hidden layers numbers and the activation function types for the neural network (NN). Then, define the patient data sent through protocols of the IoT. NN checks the patient’s medical sensors data to make the appropriate decision. Then it sends this diagnosis to the doctor. Using the proposed solution, the patients can diagnose and expect the disease automatically and help physicians to discover and analyze the disease remotely without the need for patients to go to the hospital.

Design/methodology/approach

AI technology will be combined with the IoT in this research. The research aims to select the most appropriate’ best-hidden layers numbers’ and the activation function types for the NN.

Findings

Decision support health-care system based on IoT and deep learning techniques was proposed. The authors checked out the ability to integrate the deep learning technique in the automatic diagnosis and IoT abilities for speeding message communication over the internet has been investigated in the proposed system. The authors have chosen the appropriate structure of the NN (best-hidden layers numbers and the activation function types) to build the e-health system is performed in this work. Also, depended on the data from expert physicians to learn the NN in the e-health system. In the verification mode, the overall evaluation of the proposed diagnosis health-care system gives reliability under different patient’s conditions. From evaluation and simulation results, it is clear that the double hidden layer of feed-forward NN and its neurons contain Tanh function preferable than other NN.

Originality/value

Abstract

Purpose

This paper proposes a Web-based patient portal based on the electronic medical record. Such a portal can allow patients to manage their own health care, reduce health-care visits and significantly improve the quality of their health care.

Design/methodology/approach

A patient portal prototype and an accompanying online survey were distributed to assess the adoption readiness among a group of people in the United Arab Emirates (UAE).

Findings

The results from 470 survey participants demonstrated an enhanced awareness of this technology, and support the study hypotheses indicating that both intrinsic and extrinsic factors are important when considering the implementation of a patient portal in the UAE.

Originality/value

This study adds value to the few research studies undertaken in the Middle East discussing online health information technology and its adoption and usage among the population at large. The extended technology acceptance model, which contains two additional constructs, had not been previously validated in terms of a patient portal in the UAE, according to the author’s knowledge, adding more value. The UAE’s health-care system must use the benefits from the available IT infrastructure to provide a user-friendly online portal to encourage patients to manage their health care and health information.

Abstract

Purpose

Technology has revolutionized the delivery of health-care services, with e-consultations becoming popular mode of service delivery, especially during the pandemic. Extant research has examined the adoption of e-health consultation services, with little attention paid to examine the switching behavior. This study aims to identify factors affecting patients’ intentions to switch from conventional mode i.e. visiting hospitals/clinics to e-health consultations.

Design/methodology/approach

To understand this we use the push–pull–mooring (PPM) framework and integrate variables from status quo bias framework to the model. A cross-section research design was used, which rendered 413 valid responses which were obtained from the patients visiting a traditional hospital setup. The data was analyzed using partial least square – structural equation modeling using SmartPLS 3.0.

Findings

Findings suggest that push effects (inconvenience and perceived risk), pull effects (opportunity for alternatives and ubiquitous care), mooring effects (trust) and inertia significantly influence patients’ switching intentions from visiting hospitals/clinics to e-health consultations. Further, habit and switching cost positively influence inertia.

Practical implications

This study shall enable online health-care service providers and practitioners to understand patients’ intentions to switch to online health platforms and accordingly develop related marketing strategies, services and policies to encourage them to switch to the new offerings.

Originality/value

The current study enriches the previous research on e-health services by applying and extending PPM framework as the base model and showing its efficiency in predicting individuals switching intentions in the context of emerging economies. This study bridges the gap by focusing on switching behavior in context of health services.

Purpose

With the development of emerging engineering technology and industrialization, there are greater changes in the life style of people in smart urban cities; therefore, there is also more chance of various health problems in urban areas. The life style of persons in metro urban areas with the expansive volume of population is similarly influenced by different application and administration frameworks. These are affecting the human health system up to an extended extent and there are more health-related issues and health hazard concerns that can be identified in urban areas. The purpose of this paper is to present an analytical study on various aspects of the smart health care system in a smart perspective by analyzing them with respect to emerging engineering technologies such as mobile network, cloud computing, Internet of Things (IoT), big data analytics and ubiquitous computing. This paper also carries out a detailed survey of health issues and improved solutions in automated systems using these technologies. Second, the paper also presents a novel health care system using smart and safe ambulances and their appropriate control at traffic points with safety and security features in a smart city, so that the valuable life of patients can be saved in time by immediate treatment in nearest hospital or health care units.

Design/methodology/approach

In this paper, an analytical survey was conducted for improvement in the health care sector using computer technology and IoT-based various modern health care applications. An idea of Smart Health Care Hospital using sensors, mobile agent smart vehicle configuration and safety traffic control for ambulance was proposed.

Findings

A simulation was carried out to see the performance of a safety mechanism in the proposed approach. Comparative analysis was carried out with other approaches to know the execution time, response time and probable delay due to the implementation of this approach.

Originality/value

It is an original research work with motivation inspired from current emergent technology to apply in the health care system.

Purpose

This paper aims to validate an initially developed e-Health readiness assessment model.

Design/methodology/approach

The authors thematically analysed an initial qualitative data collected and used the outcome to develop survey instruments for this study. To collect the quantitative data, the authors used the drop and collect survey approach given the research setting. The quantitative data was analysed using factor and regression analyses of SPSS 23 in which hypotheses formulated were tested.

Findings

The results suggest that the model [R2 = 0.971; F (5, 214) = 1414.303], which is made up of readiness assessment factors (constructs) and measuring tools explain about 97% of the variance of the overall health information technology/e-Health adoption readiness at Komfo Anokye Teaching Hospital. The measuring tools were reliable for assessing the composite variables (constructs): technology readiness; operational resource readiness; organizational and cultural readiness; regulatory and policy readiness; and core readiness, which have significant influence on eHealth adoption readiness assessment..

Originality/value

This study has successfully validated empirically developed eHealth readiness assessment model with complete reliable indicators given that existing eHealth readiness assessment models have not been effective due to a general lack of standard indicators for measuring assessment factors. The study also contributes to the growing research on the adoption of information technology/systems in health-care environment using the Technology–Organization–Environment framework.

Abstract

Purpose

The increasing importance of electronic health (e-health) has raised the significance of exploring the factors influencing the users’ acceptance of e-health applications. There has been an extensive usage of the technology acceptance model (TAM) in e-health applications acceptance research. However, not all TAM relationships are borne out in all the studies. There is a variation in predicted effects in several studies with different user type and application type. The purpose of this paper is to bridge a research gap by providing a holistic view of the e-health applications acceptance research by integrating the findings of existing relevant literature.

Design/methodology/approach

A statistical meta-analysis of the effect size of causal relationships between common TAM constructs was conducted on 111 peer-reviewed academic studies published in various journals.

Findings

The results confirm the validity and robustness of the TAM in e-health applications acceptance research. Further, a moderator analysis based on user type and e-health application type demonstrated that the effect size of causal relationships between TAM constructs majorly depends on the user type, but not on e-health application type.

Research limitations/implications

This research provides a ready reference of the existing studies on e-health applications acceptance to the researchers. Further, if researchers or practitioners want to learn more about the particular user or application type, they may find the results valuable.

Practical implications

This research suggests that the general public can be used as the surrogates for patients in e-health applications acceptance research. The implementation strategy found successful for medical staff may not work for general public and patients. This research also suggests ways to enhance acceptance of e-health applications among different user groups.

Originality/value

The research is original and is based on the existing literature and its interpretation.

Abstract

Purpose

The study examines the amplifying role of users in the e-healthcare sector and holistically show its current state and potential. The paper aims at contributing to the scientific literature with a comprehensive review of the current state of the art on the application of user innovation (UI) in the e-healthcare sector, as a solid step for discussing the potential, trends, managerial gaps and future research avenues in this field. Despite the crucial importance of the topic and increasing attention toward it in the last few years, there is a lack of comprehensive scrutiny on different angles of involving users in health technology innovations so far.

Design/methodology/approach

This study combines two methods of bibliometric analysis and extensive content analysis of 169 journal articles on Scopus and Web of Science to unfold five research questions regarding the mechanisms of involving users, innovations characteristics and the role of users throughout the innovation process.

Findings

A clear result of the applied methodology is the profiling of users involved in e-health innovations in seven categories. The results of this study shed light on the current practice of not involving users in all the stages of the innovation process of m-health, telemedicine, self-managing technologies, which is contrary to the best practices of the UI application.

Research limitations/implications

Collection of relevant studies due to lack of comprehensibility of the keywords.

Practical implications

The offered propositions can act as a roadmap to potential research opportunities as well as to organize such innovations from a managerial perspective in particular healthcare organization managers and the middle managers operating at R&D sectors and policymakers.

Originality/value

This study is the first of its kind that digs out the application of UI strategies such as user-centered design in the context of e-healthcare and provides a bibliometric and extensive content analysis of the studies conducted in this theme over the years.
