Abstract

Purpose

The race concordance hypothesis suggests that matching patients and health providers on the basis of race improves communication and patients’ perceptions of health care, and by extension, encourages patients to seek and utilize health care, which may reduce health disparities. However, relatively few studies have examined the impact of race concordance on the utilization of health services. This chapter is grounded on Andersen’s Emerging Model of Health Services Utilization (Phase 4) and extends that model to include race concordance.

Methodology/approach

The data were collected from a stratified random sample of adult beneficiaries enrolled in North Carolina Medicaid’s primary care case management delivery system in 2006–2007. Propensity score matching techniques were used to sort respondents on their propensity for race concordance and indices were constructed to generate key control variables. Poisson regression was used to examine the impact of race concordance on the utilization of primary care and emergency room care, under the assumption that race concordance would increase the use of primary care and decrease the use of emergency care for minority patients.

Findings

While blacks (compared to whites) used less primary care and had more emergency care visits, race concordance was not a statistically significant predictor of either primary care or emergency room use. However, patients’ satisfaction with their primary care providers was associated with significantly fewer primary care and emergency care visits while trust in one’s provider was associated with more primary care visits.

Research implications

The study findings suggest that the central premises of the race concordance hypothesis require further study to confirm the assumption that better patient – primary care provider relationships result in less utilization of more costly and resource-intensive forms of health care.

Value of chapter

The study makes a valuable contribution by expanding the relatively small body of literature dedicated to exploring the impact of race concordance on health services utilization. Additionally, by virtue of researching the experience of Medicaid enrollees, the study controls for health insurance status.

Abstract

Purpose

Previous studies have vague views about whether employees who are required to complete large amounts of work (i.e. role overload) would proactively create a change in their job characteristics (i.e. job crafting), because the cognitive mechanism underlying the nexus between role overload and job crafting is unclear. The aim of this study is to identify why and when role overload has an impact on job crafting.

Design/methodology/approach

This study builds a second-stage moderated mediation model. Using a two-wave panel field study of 213 employee–supervisor matched data, this study examines the proposed hypotheses.

Findings

Results show that role overload decreases construal level, which can determine the tendency of employees to focus on the feasibility (low level of construal) or desirability (high level of construal) of behaviors. Goal self-concordance is the degree to which employees pursue their personal goals based on feelings of personal interests and values. The authors find that goal self-concordance guides employees who have higher levels of construal to exert more effort in job crafting. The authors further find that goal self-concordance moderates the mediating role of construal level. Specifically, for employees in pursuit of self-concordant goals, role overload reduces their construal level, resulting in less effort in job crafting. For employees who do not pursue self-concordant goals, role overload decreases their construal level, thereby improving job crafting.

Originality/value

The findings of this study enrich the literature on role overload and job crafting by revealing the mechanism and boundary conditions of the relationship between role overload and job crafting.

Abstract

Purpose

Achieving optimal nutrition among mothers and children is still a challenge in many developing country settings, including Ethiopia. Study on dietary diversity concordance of mother-to-child dyads is limited. Hence, this study aims to assess dietary diversity concordance among mother-to-child pairs and its associated factors in Gedeo zone, Southern Ethiopia.

Design/methodology/approach

A community-based cross-sectional study was carried out in Gedeo zone, South Ethiopia, from January 1 to February 15, 2019. A multistage sampling technique was used to select study participants. Data was collected by using a pretested and structured questionnaire. Data entry and analysis were done by Epi data version 3.1 and SPSS version 23.0 software, respectively. Bivariate and multivariate logistic regression models were used. Variables with p < 0.25 at bivariate analysis were chosen for multivariate analysis. Variables with a p-value of <0.05 were considered statistically significant.

Findings

A total of 665 mother and child dyads participated, with a response rate of 98.6%. High concordance of dietary diversity among mother-to-child pairs was 9%. Age of the mother (adjusted odds ratio [AOR] = 0.21; 95% confidence interval [95% CI]: 0.05–0.84), age of the child between 6 and 11 months (AOR = 5.2; 95% CI: 2.3–11.6), being female (AOR = 1.8; 95% CI: 1.0–3.4), previous history of infection (AOR = 4.4; 95% CI: 1.8–11.5), source of food from the market (AOR = 0.34; 95% CI: 0.12–0.81) and rich wealth index (AOR = 5.7; 95% CI: 2.5–12.9) were the significant factors of high concordance of dietary diversity.

Originality/value

The prevalence of high-concordant dietary diversity among mother and children dyads was very low. Therefore, ensuring multisectoral nutrition interventions that focus on both mothers and children should be considered to improve the good dietary diversity practice.

Abstract

Purpose

Psychology studies have gradually evolved in the past decades from examining negative to positive states of mind. This has led to an increasing interest in understanding the well-being of individuals. Further, organizational research has demonstrated a positive impact of employee well-being on key business indicators. Drawing from positive psychology, this study examines the impact of happiness-enhancing activities and positive practices (HAPP) on employee well-being.

Design/methodology/approach

This study was conducted in an emerging information technology firm in India. Using validated measures, this study designed an online survey that was completed by 54 employees of the firm.

Findings

Employees largely perceived the HAPP as important but reported low levels of engagement in most activities or practices. Using hierarchical linear regression, this study found that self-concordant work significantly impacted employee’s job satisfaction and job-related affective well-being.

Practical implications

Leaders must harness the synergy between individual and organizational growth by providing self-concordant work contexts in which employee’s key skills, competencies and strengths are leveraged.

Originality/value

This study contributes to the existing scant literature on HAPP. Further, in a first, this study uses these psychological interventions in the context of workplace and assesses their impact on employee well-being which itself is an area of growing interest among academicians and practitioners.

Abstract

Purpose

The purpose of this paper is to test the combined influence of working towards self-concordant goals with goal planning and supervisory support on goal progress and job satisfaction.

Design/methodology/approach

The data were collected among prison guards. The analysis takes into account a multilevel perspective on goals by differentiating between within- and between-person variance.

Findings

The results showed that there was no direct effect of self-concordance on goal progress. Goal progress depended on combining self-concordant goals with conscious planning and receiving supervisory support. Furthermore, it was found that job satisfaction and goal progress influenced each other over time.

Research limitations/implications

The findings confirm that to understand the influence of self-concordant goals within organizations, planning and supervisory support are essential elements for achieving goal progress. This research is the first to confirm the interrelatedness of goal progress and job-satisfaction over time.

Originality/value

The multilevel intra and interpersonal approach provides a more thorough insight into the processes involved with goal striving. It emphasizes the importance of differentiation between the different levels of motivation in Deci and Ryan’s self-determination theory, especially when applied within the work context.

Abstract

Purpose

Professional interpreter use improves care in patients with limited English proficiency (LEP) but inequalities in outcomes remain. We explore the experience of US Latinos with LEP and diabetes in language discordant care.

Methodology/approach

We conducted in-depth interviews of 20 low-income Latino patients with diabetes and LEP. We interviewed participants in Spanish, digitally recorded and transcribed interviews, and read transcripts to identify themes and interpret meanings using interpretive phenomenology as theoretical framework.

Findings

While patients preferred, and experienced greater trust in, language concordant clinical encounters, they did not believe that language discordance affected outcomes because they felt that these depended largely on their compliance with physicians’ recommendations. Patients also downplayed structural barriers to care and outcomes. Self-blame was paradoxically encouraged by physicians’ praise vis-à-vis favorable outcomes.

Research limitations/implications

Limitations include small and convenience sample and limited generalizability. However, findings illustrate communicational dynamics between patients and clinicians with important implications for health care practice and policy. They support the perception that trust develops best within language concordant care, which underscores the importance of recruiting clinicians with diverse language skills. They highlight the importance of sensitizing clinicians to the social determinants of health, which may be overlooked when treating patients with conditions requiring substantial self-management, like diabetes. Language barriers in health care must be understood in the broader context of structural inequalities in health care. The necessary emphasis on self-management may (inadvertently) strengthen the hegemonic view that places responsibility for diabetes outcomes on patients’ ability to self-manage their condition to the neglect of social/political determinants of diabetes.

Originality/value

Studies have quantitatively examined the effects of language discordant care on diabetes outcomes, yet few have done so qualitatively. To our knowledge, no study has attempted to understand the experience of language discordance from the perspective of LEP patients with diabetes and how this experience may explain observed differences in outcomes.

Abstract

Purpose

The purpose of this study is to explore spatiotemporal factors that affect the empirical analysis of whether crime rates in buffer areas surrounding abandoned properties transferred to a Land Bank that differed among three regimes: before transfer, during Land Bank stewardship and after disposition and whether those differences were associated with differences in relative crime activity in the neighborhoods in which they were located.

Design/methodology/approach

This study analyzed crime incidents occurring between 2010 and 2018 in 0.1-mile buffer areas surrounding 31 abandoned properties sold by the Land Bank and their neighborhoods in which those properties were located. Using Copulas, researchers compared concordance/discordance in the buffer areas across the three regime states for each property and approximately matched time periods for associated neighborhoods.

Findings

In a substantial number of cases, the relative crime activity levels for buffer areas surrounding individual sold properties as measured by the Copulas shifted from concordant to discordant states and vice versa. Similarly, relative crime activity levels for neighborhoods shifted from concordant to discordant states across three matched regimes. In some cases, the property and neighborhood states matched, while in other cases they diverged. These cross-level interactions indicate that criminal behavioral patterns and target selection change over time and relative criminal activity. The introduction of Copulas can improve the reliability of such models over time and when and where they should be customized to add more granular insights needed by law enforcement agencies.

Research limitations/implications

The introduction of Copulas can improve the spatiotemporal reliability of the analysis of criminal activity over space and time.

Practical implications

Spatiotemporal considerations should be incorporated in setting interventions to manage criminal activity.

Social implications

This study provides support for policies supporting renovation of abandoned properties.

Originality/value

To the best of authors’ knowledge, this research is the first application of Copulas to crime impact studies. As noted, Copulas can help reduce the risk of applying intervention or enforcement programs that are no longer reliable or lack the precision provided by insights into convergent/divergent patterns of criminal activity.

Abstract

During the transition to parenthood, the gender division of paid and unpaid work undergoes a profound redefinition in response to both attitudes and resources. These attitudes may be concordant or discordant between two partners, they may or may not clash with perceived financial or labour market constraints, and they may or may not provoke explicit conflicts and negotiations. In this study, by combining quantitative and qualitative data, we focus on Italian couples with young children or in transition to first child, and we explore what happens when partners have discordant views. The findings show that the division of domestic and care work seems more resistant to change and more responsive to the husband’s attitudes than does the division of paid work, as the latter is mainly driven by the woman’s education and attitudes. The findings also show that very few couples overtly disagree. If they do so, the main issue in dispute is the allocation of domestic work and the main solution consists more in hiring external help than in obtaining the husband’s greater participation. Compared with domestic work, the allocation of care is a less disputed and more flexible issue: when women start negotiations on a more equal sharing, men are more willing to increase their participation. However, when a more equal sharing is not attained, couples’ narratives tend to give the “cause” to the constraints of the man (typically his work) than of the woman, while they point at a redefinition (for the best of the family) of her rather than his preferences.

Purpose — Ranking is a natural task for a search engine; a search engine result page is the most common example. This chapter aims at illustrating the motivations and the concepts of rank correlation in a practical way for the researchers active in the different domains of search engines.

Methodology/approach — To this end, this chapter provides a survey according to a topic-oriented basis of the search engine evaluation literature specifically devoted to or based on rank correlation; the chapter explains and illustrates how statistics is the only approach to rank correlation.

Findings/research limitations/implications — The chapter introduces the pros and cons of rank correlation measures through a light-weight formal description and a number of concrete examples to find the measure that better fit a context.

Practical implications — This chapter provides a blueprint for the application of rank correlation within scientific experimentation or item/service recommendation.

Social implications — Rank correlation analyses impact on the success or failure of a search engine in performing the tasks for which it has been designed and hence on the people's daily life activities.

Originality/value of paper — This chapter places rank correlation within a scientific research perspective and in particular connects to and complements documentation on search engine evaluation.

Abstract

Purpose

What copulas are, their estimation, and use is illustrated using a geographical diversification example. To accomplish this, dependencies between county-level yields are calculated for non-irrigated wheat, upland cotton, and sorghum using Pearson linear correlation and Kendall's tau. The use of Kendall's tau allows the implementation of copulas to estimate the dependency between county-level yields. The paper aims to discuss these issues.

Design/methodology/approach

Four parametric copulas, Gaussian, Frank, Clayton, and Gumbel, are used to estimate Kendall's tau. These four estimates of Kendall's tau are compared to Pearson's linear correlation, a more typical measure of dependence. Using this information, functions are estimated to determine the relationships between dependencies and changes in geographic and climate data.

Findings

The effect on county-level crop yields based on changes of geographical and climate variables differed among the different dependency measures among the three different crops. Implementing alternative dependency measures changed the statistical significance and the signs of the coefficients in the sorghum and cotton dependence functions. Copula-based elasticities are consistently less than the linear correlation elasticities for wheat and cotton. For sorghum, however, the copula-based elasticities are generally larger. The results indicate that one should not take the issue of measuring dependence as a trivial matter.

Originality/value

This research not only extends the current literature on geographical diversification by taking a more detailed examination of factors impacting yield dependence, but also extends the copula literature by comparing estimation results using linear correlation and copula-based rank correlation.
