Abstract

Children's services agencies in England are increasingly expected to work together to plan needsled interventions in their locality. Such planning requires good information on local need and service take‐up, yet this is often lacking. Connecting data from single agency, multi‐agency and community studies in one district shows how the severity of need overlaps with different service and administrative categories. The findings point to the value of such mapping exercises, while the pattern that emerges highlights the importance of planning provision for all children and of linking intervention to thresholds of severity of need. The approach will have relevance to other care groups.

Abstract

Access to health care, particularly for children, remains a topic of great importance to policy makers in the United States. Recent attention focuses primarily on the enactment and subsequent expansions of the Children’s Health Insurance Program (CHIP) (Kenney, Ullman & Weil, 2000). Though the legislation affects all qualifying low-income children, the unique service needs of children with disabilities justify a closer look at the relationship between health insurance, income, and needs amongst children with disabilities. This analysis seeks to answer the following questions. To what extent do children with disabilities need various supportive health services? Does this need vary across type of disability or income level? Do children with health insurance, either public or private, have fewer unmet needs than children without health insurance? Answers to these questions will assist policy makers when determining who should be targeted to receive additional assistance in the future as well as evaluate the effectiveness of current mechanisms in delivering supportive health services to children with disabilities.

Abstract

The Government of Indonesia has started committing to promoting inclusive education since the release of the Minister of National Education of Indonesia's Regulation Number 70 of 2009, which focuses on discussing inclusive education. Indonesia has been facing some challenges with implementing inclusive education, including teachers' attitudes and skills, community acceptance, and support systems. However, considerable efforts have been made by related parties to implement this type of education. Besides issuing regulations, the government has produced and developed many programs regarding inclusive education. School members, professionals, and therapists work hand in hand to help special needs children in an inclusive school setting. Furthermore, the community has played its role as advisors, supporters, controllers, and mediators of the lives of children with special needs. At last, international organizations have been taking part in programs dedicated to inclusive education in Indonesia. It is hoped that all these collaborations will highly benefit the implementation of this education system.

Abstract

There is more to primary care than solely medical and nursing services. Models of Child Health Appraised (MOCHA) explored the role of the professions of pharmacy, dental health and social care as examples of affiliate contributors to primary care in providing health advice and treatment to children and young people. Pharmacies are much used, but their value as a resource for children seems to be insufficiently recognised in most European Union (EU) and European Economic Area (EEA) countries. Advice from a pharmacist is invaluable, particularly because many medicines for children are only available off-label, or not available in the correct dose, access to a pharmacist for simple queries around certain health issues is often easier and quicker than access to a primary care physician or nursing service. Preventive dentistry is available throughout the EU and EEA, but there are few targeted incentives to ensure all children receive the service, and accessibility to dental treatment is variable, particularly for disabled children or those with specific health needs. Social care services are an essential part of health care for many extremely vulnerable children, for example those with complex care needs. Mapping social care services and the interaction with health services is challenging due to their fragmented provision and the variability of access across the EU and EEA. A lack of coherent structure of the health and social care interface requires parents or other family members to navigate complex systems with little assistance. The needs of pharmacy, dentistry and social care are varied and interwoven with needs from each other and from the healthcare system. Yet, because this inter-connectivity is not sufficiently recognised in the EU and EEA countries, there is a need for improvement of coordination and with the need for these services to focus more fully on children and young people.

Abstract

This chapter focusses on contradictions within existing legislation to develop an equitable and effective educational system for those with special educational needs and disabilities (SEND). It highlights tensions between the underlying aims of the legislation and the implementation of policies, specifically on admissions and exclusions with reference to the current government proposals for an increase in Alternative Provision (AP). It considers the response of Multi-Academy Trusts (MATs) to ensure an acceptable practice of inclusion while raising overall standards of attainment. It also considers the Government Green paper response to increasing pressure from parents for more appropriate levels of support for their children with SEND.

Abstract

This paper considers issues to do with the assessment of, and provision made for, children with autism. Reference is made to some of the key principles of current legislation and guidance, together with developments in the voluntary sector. These are discussed in the context of personal experience of managing a school for children with autism and diagnostic and assessment services for children with disorders of development and communication. Greater awareness has led to an expansion of services, often in response to pressure from parent organisations, but careful attention needs to be given to ensuring that continued development meets the very complex and specific needs of children with autism.

Examines issues related to the integration of children with special needs into regular classrooms. A clear definition of integration is crucial before it can be considered on a system, school, or individual basis, so that all stakeholders can operate within a common frame of reference. Details the Regular Educational Initiative in the United States and the Alberta movement towards a provincial integration policy which offer insight into the need for these matters: (1) clear principles and practices for identifying children′s special needs and determining the best teaching environments for meeting those individual needs; (2) systematic research on service delivery models, and educational change based on demonstrated programme effectiveness; (3) adequate preparation for all classroom teachers on individualizing student educational programmes; and (4) development of adequate school and community support systems for children with special needs and their families.

Abstract

Children's social services in England and Wales deal with a wide range of referrals of children who are or may be ‘in need’. Finding ways of describing the issues that present in such referrals is important if we wish to understand the nature of the work of children's services and explore different interventions and outcomes. Yet there have been few attempts to describe the full range of needs presenting to social services, and no studies of the reliability or validity of attempts to define the types of need. In this article the legal definitions of need, a typology developed by Sinclair et al, a related one used by the Department of Health and one developed within the current study were compared for reliability and construct validity. There were two main findings. First, it was found that while the presence of needs could generally be agreed on in all the schemes, ascriptions of a ‘main’ need were not made reliably. This is important because a ‘main’ need has been used in both research and statistical returns to government. Second, while existing schemes appeared well suited to describing allocated cases, they were less able to describe the range of needs presenting in all referrals to social services.

Abstract

This article discusses how complexity theory is being used to understand social phenomena. It notes that published articles tend to discuss these ideas in relation to social care without quantification. It demonstrates that there is quantitative evidence that one aspect of complexity thinking, ‘self‐organising criticality’, could be at work in generating children in need in England as defined by the Children Act 1989. The article is based on a secondary analysis of data on the weekly costs of children in need derived from the Children in Need Census 2005. Data were provided by the Department for Children, Schools and Families. It concludes that the distribution of the frequency of weekly cost of children in need shows that a mechanism involving self‐organising criticality may indeed be at work in creating children in need served by local authorities.
