Abstract
Purpose
The purpose of this paper is to assess the effectiveness of a toothbrushing intervention delivered in primary schools in Yorkshire and the Humber, a Northern district of England. The toothbrushing intervention was designed with the intention of improving the oral health of young children. The paper reports the effectiveness of the intervention and explores process issues related to its co-ordination and delivery.
Design/methodology/approach
The evaluation had three data gathering approaches. These were: in-depth case studies of three selected schools participating in the toothbrushing programme; interviews with oral health promoters responsible for the programme in the district; and a small scale questionnaire-based survey which was sent to the 18 schools participating in the intervention.
Findings
The intervention was accepted by children and they enjoyed participating in the toothbrushing scheme. Children had often become more knowledgeable about toothbrushing and the consequences of not regularly cleaning their teeth. The scheme was contingent on key staff in the school and the programme was more successful where school's embraced, rather than rejected the notion of improving children's health alongside educational attainment. Whether the intervention made differences to brushing in the home requires further investigation, but there is a possibility that children can act as positive “change agents” with siblings and other family members.
Practical implications
This paper suggests that schools can be an effective setting for implementing toothbrushing interventions.
Originality/value
Toothbrushing in schools programmes are a relatively new initiative that have not been fully explored, especially using qualitative approaches or focusing on the views of children. This paper makes a particular contribution to understanding the process and delivery of toothbrushing interventions delivered in primary schools. The implications for programmes outside of the UK context are discussed.

Abstract
Purpose
The purpose of this paper is to explain the rationale and potential for the WHO health promoting schools (HPS) to improve children ' s oral health, and describe validated quantitative methodologies and qualitative approaches to measure program impact.
Design/methodology/approach
Critical discussion of the impact of poor oral health and potential for school-based educational intervention, and evaluation methodologies used by the authors.
Findings
Using HPS to improve oral health is relevant because dental caries and gingivitis/periodontitis negatively impact children ' s health and quality of life worldwide. WHO has called for effective community-based oral health promotion programs; intervention is simple and low cost; robust evaluation measures exist – the decayed missing filled teeth index and change in cavity rate allow quantitative comparison of oral health status; and questionnaires document changes in knowledge, practices, diet, health-related quality of life, and pain.
Practical implications
Poor oral health is a major health issue. Established measures to improve oral hygiene offer an achievable, low-cost HPS entry point; the “knowledge” and “healthy practices” components central to the WHO HPS model are tried and tested and multiple potential benefits are documented. Poor oral health is a non-stigmatized issue, hence intervention is readily accepted, and effective evaluation tools provide evidence of program effect over a short (two to three years) timeframe.
Originality/value
Oral health promotion is more affordable and sustainable than the cost of traditional restorative treatments especially in middle- and low-income countries. Success with oral health leads to confidence for expansion of HPS activities to address other health issues relevant to the school community.

Abstract
Purpose
By applying ecological models of health behaviour to marketing communications to achieve behaviour change, this paper aims to illustrate the importance of taking into account various economic, environmental and social influences.
Design/methodology/approach
A two-part study was undertaken. Part one involved exploring the lived worlds of the targeted population. Part two explored how the needs of the target audience informed a social marketing communications strategy. This was illustrated through Childsmile, a Scottish Government funded oral health institution.
Findings
A variety of intra- and inter-personal influences where identified that encouraged or discouraged oral health. Complementing this was how these needs are incorporated into an ecological social marketing communications campaign. Although the long term effects of the ecological social marketing campaign will not become evident for a number of years, initial results indicate its important role in changing behaviour.
Practical implications
The importance of engaging with various groups within social marketing is shown. Specifically, the need to understand and encourage interaction between individuals, their community, health institutions and the Government.
Social implications
Behaviour change, through social marketing communications, is possible among socio-economic deprived groups. Change supported with face to face interactions with health professionals.
Originality/value
Previous criticisms of social marketing research being American-centric, and avoiding issues around socio-economic deprivation are addressed. In addressing this, the paper also answers calls for research into ecological models of social marketing communications to understand how influences affect its applicability.

Abstract
Purpose
The purpose of this paper is to examine the effectiveness of oral hygiene promoting program (OHPP) using a smartphone endomicroscope among students in elementary schools in Bangkok, Thailand, to enable their oral hygiene maintenance and to compare oral health outcomes in the intervention program between the experimental and control groups.
Design/methodology/approach
This is a quasi-experimental research study consisting of two groups, a pretest, a posttest and a follow-up design. The student sample consisted of 59 fifth graders who matched the study criteria on medium and low levels of teeth brushing skills. There were 29 experimental group participants who completed the six-week OHPP using a smartphone endomicroscope. Another 30 participants formed the control group involved in the ordinary oral health education program. The comparison data for oral health behaviors (knowledge, attitude and practice (KAP)) and oral hygiene were statistically analyzed by using multivariate analysis of variance (MANOVA).
Findings
Results indicated that the experimental group exhibited higher oral health behaviors regarding KAPs and oral hygiene related to teeth cleanliness and gum health than the control group in the postexperimental and follow-up phases. However, there were no statistically significant differences between the two groups with regard to tongue cleanliness.
Originality/value
The students maintain their own oral hygiene due to a provision of activities related to KAP concerns. The instrument helped the students to find tooth decay and dental plaque associated with teeth cleanliness after brushing their teeth.

Abstract
Purpose
The paper examines African immigrant parents' views on dental decay and whether such views affect their decision to obtain dental insurance for their children. The paper also examines the cultural underpinnings of the immigrants' oral health care practices.
Design/methodology/approach
The data for the study were collected in the states of Indiana and Virginia. The data were collected in 2005 by the author and a research assistant. A total of 420 parents (220 males and 200 females) took part in the research, answering questions about 601 children.
Findings
The results indicate that a sizeable number of African immigrant school‐aged children are denied dental health due to a combination of cultural perceptions on dental health (such as families taking responsibility for cleaning their teeth themselves), parental ignorance on dental health, parents' perception of dental health practice in the USA, and the parents' weak financial standing.
Originality/value
The study is of considerable importance to dental care providers, public health specialists, social workers, and more especially the school systems that have African immigrant children, because as people who provide information and guidance to such immigrants, knowing about how the immigrants' cultural backgrounds influence their decisions about dental care will help to serve the immigrants more effectively.

Abstract
Purpose
Parents' health literacy is an important element to ensure the sound health of children. One of the major purposes of this study was to systematically collect and review the literature that identified the relationship between two variables: parents' health literacy and their child's health.
Design/methodology/approach
To meet the purpose, the authors searched four well-reputed research databases such as Web of Science, Scopus, Google Scholar and LISTA (Library, Information Science and Technology Abstracts) comprehensively to collect the relevant research on the topic. Except for books, various types of content from different years were used for this review. The investigators used Preferred Items for Systematic Reviews and Meta-analysis guidelines to select eligible studies for this review. The authors also assessed the quality of the reviewed research in the light of well-reputed quality guidelines shared by Boynton and Greenhalgh (2004).
Findings
The majority of the reviewed studies found a statistically significant relationship between parents' health literacy and the health of their children. Findings also identified that the majority of the reviewed literature scored from 9 to 12 on the quality assessment criteria. Results also highlighted that the majority of the reviewed research on parents' health literacy and child health has been conducted in the USA and utilized quantitative methods to conduct research on the topic under this study.
Research limitations/implications
This systematic review has multiple theoretical and practical implications for parents, policymakers and researchers.
Originality/value
This is the first systematically collected and reviewed paper on the relationship between parents' health literacy and child health.

Abstract
Purpose
The study aims to test the efficacy of brief-caries risk assessment form with standard caries risk assessment form and to evaluate the application of caries risk assessment following American Academy of Pediatric Dentistry (AAPD) between brief and standard caries risk assessment on dental health status among preschool children.
Design/methodology/approach
Brief-caries risk assessment form was developed. Then, experimental study was conducted in public health center 54 among 70 patients (35 test and 35 control) from January to July 2019. Test group used brief-caries assessment form, and control group used standard form. Both groups received the same caries risk assessment criteria and management protocol from AAPD. At baseline, 3-month and 6-month follow-up, caries risk and dental health status (plaque index, cavitated caries lesion and non-cavitated caries lesion) were assessed. Data were analyzed by descriptive statistic, t-test, chi-square test, Fisher's exact test and repeated measures ANOVA.
Findings
Percentage of high caries risk decreased from baseline (93.9%: test and 96.9%: control) to 6-month follow-up (66.7%: test and 65.6%: control) in both groups, with no statistically significant differences between groups. Plaque index, cavitated caries lesion and non-cavitated caries lesion were not statistically significant differences between groups. Brief-caries assessment decreased times/visit from 10-15 minutes to 5 minutes.
Originality/value
Brief-caries assessment form decreased caries risk and prevented dental caries as the standard form. Using brief-caries assessment form could save time, is cost-effective and is appropriate for use in public health centers. However, a short follow-up time might have insufficient power to detect the differences between groups.

Abstract
Purpose
Children’s health and life chances are affected by many factors, with parents and schools holding influential roles. Yet relatively little is known about parental engagement in school-based health education and specifically, from the perspectives of health and education professionals. The purpose of this paper is to examine professionals’ perspectives on parental engagement in school-based health education.
Design/methodology/approach
An exploratory qualitative study was conducted with ten health, education and local authority professionals from a socio-economically deprived area in England. Semi-structured interviews explored the role of professionals within the school health curricula, roles that parents played in school health, and barriers and enablers to parental engagement in school health education.
Findings
Reported barriers to engagement related to assumptions about parents’ own health behaviours, impacts of funding and inspection regimes, and protected time for health within the school curriculum. Enablers included designated parental support workers based in the school, positive role modelling by other parents, consultation and engagement with parents and a whole school approach to embedding health within the wider curriculum.
Practical implications
Findings from this study suggest the importance of building meaningful partnerships with parents to complement school health education and improve child health outcomes.
Originality/value
This paper addresses an important gap in the research on parental engagement in school-based health education from the perspectives of health and education professionals. Effective partnerships with parents are crucial to the success of school health education.

Purpose
The purpose of this investigation is to gain insight into parents' perceptions of benefits vs burdens (value) of educational and healthcare service received for their child with ASD. Parents are the main integrators of long-term educational and healthcare service for their child with ASD.
Design/methodology/approach
Design/methodology/approach included (1) a sentiment analysis of discussion forum posts from an autism message board using a rule-based sentiment analysis tool that is specifically attuned to sentiments expressed in social media and (2) a qualitative content analysis of one-on-one interviews with parents of children diagnosed with ASD, complemented with interviews with experienced educators and clinicians.
Findings
Findings reveal the link between customized service integration and long-term benefits. Both parents and service providers emphasize the need to integrate healthcare and educational service to create holistic long-term care for a child with ASD. Parents highlight the benefits of varied services, but availability or cost are burdens if the service is not publicly provided, or covered by insurance. Service providers' lack of experience with ASD and people's ignorance of the challenges of ASD are burdens.
Practical implications
Ensuring health outcomes for a child with ASD requires an integrated service system and long-term, customer-centric service process because the scope of service covers the child's entire childhood. Customized educational and healthcare service must be allocated and budgeted early in order to reach the goal of a satisfactory service output for each child.
Originality/value
This is the first service research to focus on parents' challenges with obtaining services for their child with ASD. This paper provides service researchers and managers insight into parents' perceptions of educational and healthcare service value (i.e. benefits vs. burdens) received for their child with ASD. These insights into customer-centric perceptions of value may be useful to research and may help service providers to innovate and provide integrated service directly to parents, or indirectly to service providers, who serve children with ASD.

Abstract
Aims to assess the effectiveness of a child‐to‐child approach to promote healthier snacking in primary school children. A total of 55 schools in North and West Belfast were matched for socio‐economic status (SES). Ten schools were randomly selected and allocated into intervention and control groups. A total of 482 children took part. Older intervention children were given the “snacks facts” programme and became “teachers” in the child‐to‐child intervention. All children had baseline and final assessments made of their dental health knowledge (older children only), snacking knowledge and behaviours using questionnaires and rubbish bags. Older intervention children had greater increases in their mean knowledge scores compared with control children. Older intervention children had greater decreases in mean cariogenic snacking scores compared with control children. Younger children attending higher SES schools had significant decrease in mean cariogenic snacking score compared with children attending lower SES schools. Concludes that the child‐to‐child approach provided an avenue by which children improved their dental health knowledge and modified their snacking habits during break‐time at school.
