Abstract

Achieving reliable instrument reprocessing requires finding the right balance among cost, productivity, and safety. However, there have been few attempts to comprehensively examine sterile processing department (SPD) work systems. We considered an SPD as an example of a socio-technical system – where people, tools, technologies, the work environment, and the organization mutually interact – and applied work systems analysis (WSA) to provide a framework for future intervention and improvement.

The study was conducted at two SPD facilities at a 700-bed academic medical center servicing 56 onsite clinics, 31 operating rooms (ORs), and nine ambulatory centers. Process maps, task analyses, abstraction hierarchies, and variance matrices were developed through direct observations of reprocessing work and staff interviews and iteratively refined based on feedback from an expert group composed of eight staff from SPD, infection control, performance improvement, quality and safety, and perioperative services. Performance sampling conducted focused on specific challenges observed, interruptions during case cart preparation, and analysis of tray defect data from administrative databases.

Across five main sterilization tasks (prepare load, perform double-checks, run sterilizers, place trays in cooling, and test the biological indicator), variance analysis identified 16 failures created by 21 performance shaping factors (PSFs), leading to nine different outcome variations. Case cart preparation involved three main tasks: storing trays, picking cases, and prioritizing trays. Variance analysis for case cart preparation identified 11 different failures, 16 different PSFs, and seven different outcomes. Approximately 1% of cases had a tray with a sterilization or case cart preparation defect and 13.5 interruptions per hour were noted during case cart preparation.

While highly dependent upon the individual skills of the sterile processing technicians, making the sterilization process less complex and more visible, managing interruptions during case cart preparation, improving communication with the OR, and improving workspace and technology design could enhance performance in instrument reprocessing.

In his report for the year 1919, Dr. WILLIAM J. HOWARTH, C.B.E., Medical Officer of Health for the City of London, makes some very important observations in regard to the conveyance and handling of meat. He points out that a considerable responsibility rests upon the Sanitary Committee to ensure that the food passing through the City is of a satisfactory character, and the following matters are of interest. Meat is purchased at Smithfield by butchers from all parts of Greater London, and even from districts outside. It is removed from the market either in the purchaser's own carts, or in vans belonging to the numerous carriers who attend the market. From the stalls in the market it is carried to the waiting carts either by the purchasers or by market porters. The meat is either conveyed in trucks which are provided or on the backs and shoulders of these persons. The number of carts in waiting to receive meat is so considerable that during the busier hours they form practically a continuous barricade round the market. The rear parts of the carts are brought up to the edge of the causeway. These vans and carts carry a considerable amount of meat, in the form of quarters or cuts of larger or smaller size and offal. The division of the meat facilitates increased loading. It often happens that the supply arrives at the carts more quickly than it can be packed, and, as a consequence, it is allowed to stand about on the trucks for a longer time than is desirable. During this waiting period the meat is sometimes deposited on the footway. It should be noted that the sectioning of meat results in large areas of muscular tissues being exposed, and as the cut surface is moist, dust readily settles on it with the resulting disadvantages which are common to dust deposition; dust is excessive in dry weather as the streets in the loading‐up areas are fouled by the large number of horses which stand about. As regards the pavement, I need only mention that there is considerable fouling of the surface by blood and particles of fat, etc., which are trodden into a hard layer mixed with street refuse. The further risk of contamination by animals is obvious. Dr. HOWARTH further observes, I have had occasion officially to complain of one more than usually gross instance of piling meat on the ground. In this case the meat was separated from the wet ground by a layer of coarse sacking, the edge of the cover being also practically flush with the sides of the pile. The surroundings were foul. As regards the carts, the general practice is to cover the bottom of the cart with straw. In other cases a kind of sacking is used with or without an under layer of straw, and in exceptional cases white calico or some similar matter is used. Some of the carts do not come to the market in a thoroughly clean condition. They are certainly washed at times but not every day. The straw may be clean but it is not a suitable material on which to place the cut sections of meat. The cloths I have seen used day by day, being washed, in some cases, not oftener than once a week. Blood‐stained cloths should be washed before being used again. White cloths are more desirable, as staining and dirt readily show and this results in greater care being displayed. The procedure during packing is open to criticism. I have often seen, even on wet days, men, whose boots were soiled with road dirt, get into the cart to fill the front part. In doing so they soil the straw or cloth at the rear and on this soiled part meat is afterwards laid. If a white cover were used this would probably be rolled up at the back, whilst the front was being packed, and it would be straightened out as the packing progressed. I have seen a dog in a cart in which uncovered joints of meat were lying, and the dog would have been driven away in the cart if I had not objected. My protest in this and other cases was received badly. This is simply mentioned as an instance of disregard of an obvious precautionary essential. I have seen men stretching over uncovered meat to reach the front of the cart, and in exceptional cases have seen them with their feet on it. I have also noticed, at times, meat soiled with roadway dirt. As regards the transfer of meat from the stalls to the cart, I take exception to the infrequency of change of overalls by the porters, and to meat being carried on a man's head when he is wearing a cap which has done duty for weeks without being washed. Meat is not allowed to be placed on the floor of the stalls inside the market, and practically every trader has slightly raised wooden benches. There are no such facilities outside the market, nor are there any powers to require them to be provided. It seems strange that whilst in the slaughterhouses due regard must be paid to the observance of cleanliness, and in the markets the traders recognise the advantage of cleanliness and order, and further that butchers at their shops encourage brightness and cleanliness as is evidenced by good lighting, clean benches bright brass and steel fittings, and clean‐looking tiles to line the shop, so little regard should be paid to the elementary rules of cleanliness in the interval which elapses between purchase from the wholesale dealer and the reception of the meat at the shop.

Abstract

Purpose – This study aims to introduce an application of web‐based data mining that integrates online data collection and data mining in selling strategies for online auctions. This study seeks to illustrate the process of spider online data collection from eBay and the application of the classification and regression tree (CART) in constructing effective selling strategies. Design/methodology/approach – After developing a prototype of web‐based data mining, the four steps of spider online data collection and CART data mining are shown. A business dataset from eBay is collected, and the application to derive effective selling strategies for online auctions is used. Findings – In the web‐based data‐mining application the spiders can effectively and efficiently collect online auction data from the internet, and the CART model provides sellers with effective selling strategies. By using expected auction prices with the classification and regression trees, sellers can integrate their two primary goals, i.e. auction success and anticipated prices, in their selling strategies for online auctions. Practical implications – This study provides sellers with a useful tool to construct effective selling strategies by taking advantage of web‐based data mining. These effective selling strategies will help improve their online auction performance. Originality/value – This study contributes to the literature by providing an innovative tool for collecting online data and for constructing effective selling strategies, which are important for the growth of electronic marketplaces.

Abstract

Purpose

The informal segment compared to the formal segment is developing rapidly in urban areas of many developing Muslim countries. However, matter of fact, the contribution factor of the food cart business to the economic outline of the country has been vastly overlooked or neglected. The authority, policymakers, social organisations, vendors and the customers together need to play a contributing role based on intervention tactics regarding the major focus areas related to the development of this business segment. As a result, this study aims to explore concerns or issues that are hampering future business prospects considering the philosophy of halalan tayyiba.

Design/methodology/approach

The study has organised 16 semi-structured interview procedures with relevant food cart vendors of Chittagong, Bangladesh. The study selected participants (food cart vendors) from lower- to medium-income areas occupying at least one high school, one college in relation to several commercial premises (ex-mini shopping malls, offices and small factories) considering 6 out of 41 wards of Chittagong city corporation. All meetings with reflected participants were audiotaped and transcribed verbatim and transcripts were loaded into NVivo for coding and analysis procedure.

Findings

This investigation revealed that issues with food security, weather, lack of proper legislation, lack of awareness related with halalan tayyiba and erratic policies are the major challenges for the development of the food cart business in Chittagong. This study also advances how street food security well-being and hygiene practices are a piece of the halal idea and should in this way be adjusted by the halal food segment to accomplish halalan tayyiba affirmation. The outcomes also indicate an agenda for future research in this area.

Research limitations/implications

More reflections from the different stakeholders do have both pertinence and potential for incorporation in suggestions.

Practical implications

The detected findings may help both authority and other concerned entities to identify the critical dynamics to initiate appropriate strategic activities towards local economic growth considering halalan tayyiba outlines.

Social implications

It will ensure social well-being and food security.

Originality/value

This explorative research is one of few studies in the Bangladesh context, which is investigating halalan tayyiba-oriented business development outline for micro entrepreneurs.

Purpose

Principal preparation program pedagogy and course delivery are critical to principal candidates' preparedness to lead. Research around online program delivery, however, is relatively sparse. This study examined the extent to which university-based educational leadership programs offered fully online (FOL) pathways to the principalship, as well as program geographic locations and institutional characteristics most associated with FOL offerings.

Design/methodology/approach

Data were collected through website reviews and coding checks, and then merged with national postsecondary data. Data were analyzed using descriptive statistics, classification tree analysis, and geographic information system (GIS) mapping.

Findings

Roughly 43 percent of all reviewed programs offered an FOL pathway to licensure, which suggests substantial growth in FOL offerings over the last 10 years. While a number of factors were deemed important, geographic characteristics were most associated with FOL status. GIS mapping further illustrated findings with a visual landscape of program FOL offerings.

Research limitations/implications

This study considered only programs for which degrees or certificates could be earned without ever visiting campus in-person for classes. Hybrid programs were excluded from the analysis.

Practical implications

Findings make a clear call for more research into online principal preparation program design and course delivery.

Originality/value

This study provides the first overview of fully online university-based principal preparation programs in the United States while also offering a previously unavailable landscape of all programs specifically leading to licensure. It is also the only higher education study to map or investigate factors associated with FOL offerings and raises questions about prior FOL higher education research.

Abstract

During the past twenty‐five years the importance of chemistry as applied to the practical affairs of everyday life has increased. In every Secondary School of repute, chemistry now forms an important part of the teaching. A large number of Technical Schools have been founded and at least partly endowed or assisted out of the Public Funds. Numerous Societies have been formed with the object of furnishing means and opportunities for discussing chemistry in its relations to arts and manufactures. Such facts are, in themselves, sufficient proof of the economic value of the science. Inducements are held out to the student to avail himself of the means offered on every side to adopt applied chemistry as a calling. We find teachers of chemistry asserting the claims of chemistry as the one science on which modern industry depends for its development. There is no industry, from biscuit manufacture to sulphuric acid manufacture, that does not find its chances of success certainly increased by employing scientific chemists to control the details of manufacture and its ultimate failure assured by its declining to avail itself of the resources of chemistry.

Abstract

Purpose

The purpose of this paper is to determine whether man-hour efficiency of picking is affected by the use of batch preparation, compared to preparation of one kit at a time. This paper focuses on small kit preparation areas.

Design/methodology/approach

This paper is based on two experiments that were performed at a vehicle assembly plant and then analysed quantitatively.

Findings

The results provide a strong indication of the advantages associated with batch preparation, in terms of man-hour efficiency.

Practical implications

The fact that the effects identified during the experiments are substantial, over 20 per cent reduction of average time per picked component in Experiment 1 and 7 per cent in Experiment 2, indicates that the option of batch picking holds potentials for large cost reduction and should be considered when kit preparation systems are designed.

Originality/value

Limited research has dealt with the design of kit preparation systems, thus leaving considerable knowledge gaps. Previous research dealing with batch picking focuses on other environments than kitting and on large picking areas where batching can reduce walking distances. In contrast, the current paper focuses on small picking areas, which are common in industrial kitting applications. This paper provides a considerable contribution by demonstrating improvements in time efficiency that batch preparation can offer to small picking areas in addition to larger areas. The discussion also provides a basis for future research, which could focus on aspects other than time efficiency, such as the quality of kit preparation, and variables that might moderate the effect of batching.
