Abstract

As the population ages in the United States and globally, health-care demands are rising and varied, including the growth of home health care. Small, regional, qualitative studies indicate both satisfaction and exploitation in home health-care work. These intimate, caring relationships with clients may be especially challenging for minorities due to client prejudice and structural marginalization. This study broadens the scope of current research by addressing issues facing home health-care workers using large-scale, nationwide data.

Using nationally representative data of home health aides in the US, the National Home Health Aide Survey (NHHAS), we evaluate which features of work are related to overall satisfaction. The prevalence and sources of discrimination and working conditions are examined according to workers’ intersectional gender, race, ethnicity, and class identities.

Satisfaction was highest for those who were extremely satisfied with challenging work, learning new skills, and were most supported in their caring labor. Salary was the area with the most frequent dissatisfaction. Support for reproductive and caring labor was often inadequate. Black women and men reported the highest levels of discrimination (about 28.0%), followed by Hispanic women and men (16.5% and 10%, respectively). The largest source of discrimination was patients (80.4%). There were differences in job outcomes according to intersectional identities of race, class, and gender.

Discrimination, low wages, and not having enough support for both reproductive and caring labor are problems for home health aides. Improving home health aide work is also likely to improve patient outcomes.

This article addresses contemporary presentations of migrants, particularly women, as dependents and a ‘burden’ on welfare. Focusing mainly on Britain, it shows that, while immigration policies increasingly restrict their access to official welfare, migrants are crucial to the provision of welfare both to their own family and community and in mainstream services, including professional roles as well as in informal employment. Migrants are involved in complex networks of caring relations, often across national boundaries, in which they may provide care to others in order to provide for dependents back home.

Abstract

Purpose

The purposes of this conceptual paper are fivefold: first, to present a resource definition of care in work organizations that allows business and its managers to reconnect human wealth with social progress and economic wealth in order to create a responsible, sustainable, and healthy world; second, to examine the sociological and feminist origins of care; third, to discuss identifiable qualities of care; also, to compare and contrast care with the knowledge resource; and then, to identify future research directions for care.

Design/methodology/approach

Theory development and literature review were carried out to present a conceptual definition of care.

Findings

A definition of care is proposed: care is a universal construct and is inherent in all human beings; care is the core foundation, the core energy, of all human activity, work, and interaction; care may be seen as a socioeconomic resource that acts similar to the knowledge resource and may be built into organizational strategy, management, and leadership and serves as a measurable and trainable managerial capability; and care comprises identifiable qualities in individual, relational, and managerial decision‐making categories. This definition has important cross‐cultural implications and is valid for any culture, team, and organization.

Research limitations/implications

This paper is conceptual in nature and lays the foundation for further research, which is outlined in the discussion. Follow‐on work should include case studies and other qualitative research methods as well as quantitative research methods to substantiate the definition with evidence.

Practical implications

In this paper, the author proposes that care lies at the core of being human. Care energizes all human work activity and may be employed by leaders, managers, and strategists across all organizations and cultures to maximize human potential, integrate care with the wealth creation process, and create healthy, sustainable organizations. Care is proposed as a driver for economic success and human well‐being that can give rise to the next major transformation in business and thinking. The proposed care definition, and especially, its comparison to the knowledge resource, offers scholars and practitioners a new orientation to apply to the value creation process in organizations. Care can be seen as an essential aspect of management practice, organizational strategy, and socioeconomic transformation.

Originality/value

This paper offers a unique and profound definition of care. Care has never been studied or examined in terms of energy or socioeconomic resource before. It leverages the definition of care associated with ethics of care research and provides a broader and deeper means to energize and transform work environments, management practice, and organizational strategy.

Abstract

Purpose

The purpose of this paper is to use a case study of an online parenting forum to theorise how mothers’ everyday environmental and caring labour is a form of environmental and social activism in the home, that while not organised as such, is still collectivised in a “hybrid activist collective”.

Design/methodology/approach

Using ethnographic data and content analysis from an online parenting forum for the nappy-free infant hygiene practice known as “elimination communication”, the author compares the matters of key concern arising for this group of mothers with economic activist concerns as identified by Gibson-Graham et al. (2013) in their community economies work.

Findings

The paper finds a high degree of resonance between the key concerns of the elimination communication forum members with the key concerns of community economies. Furthermore, the author identifies the components of what might comprise a “hybrid activist collective” of mothers and others undertaking direct action for environmental and social change.

Social implications

Mothers and others acting for social and environmental change through domestic practices should be recognised for their important environmental and caring labour.

Originality/value

The paper proposes the “hybrid activist collective” as a way of understanding the human and non-human elements that gather together to act for environmental and social change in a collectivised, but not formally organised manner.

Abstract

This qualitative study explores the intersection between organizational masculinity and emotionality within a pre‐hospital emergency services organization. The existence of multiple masculinities within a male‐dominated and emotion‐laden organization indicates that men who work within this context are required to negotiate multiple forms of masculinities within heavily emotionalised organizational regions or spaces. This study found that there were competing tensions between at least two forms of masculinity within the organisation in question. While militarized and managerial/technical forms of masculinity dominate as the principal hegemonic form, a heroic and caring masculinity is also essential to how the organization in question produces its key services. It is argued that forms of masculinity that are closer to the hegemonic ideal type are not “compensatory”, but have to co‐exist with other, albeit more marginalized, masculinities.

Abstract

Purpose

This chapter explores variation in direct care workers’ health risks within institutional and home-based settings, according to the demographic composition of workers and the gendered, raced, and citizenship-based expressions of their work roles.

Methodology/Approach

This quantitative intersectional study draws on two nationwide datasets from the US National Center for Long-term Care Statistics, a division of the Centers for Disease Control and Prevention, the National Nursing Assistant Study (NNAS), and National Home Health Aide Survey (NHHAS).

Findings

Workplace context was the strongest predictor of workers’ health risks and working conditions. Physical injuries affected more than half of facility-based workers annually compared to less than 10% of home-based workers. Facility-based workers are more likely to report insufficient time for tasks, lower job satisfaction, and less respect and appreciation from patients. Home-based workers may be more likely to experience emotional distress, be offered fewer benefits, but experience fewer injuries, due to the better relative health of their patients and having more time for client care. Women reported more injuries and more time pressure than men across racial and citizenship groups within the same work setting.

Research Limitations/Implications

There are limitations to the NHHAS and NNAS public-release data file data. We are unable to fully capture citizenship, some racial/ethnic categories, workers over age 65, supervisory workers, facilities with fewer than three residents, and facilities not certified with Medicare or Medicaid. The exclusion of these questions, workers, and contexts is a weakness of the present study.

Originality/Value of Paper

Analyses draw on data from the first nationally representative sample surveys of home health aides and nursing assistants in the United States. Direct care workers are an important population to capture through intersectional research since care work is done predominantly by multiracial women and immigrants. This research also underscores the importance of workplace contexts in shaping the labor performed and the workers’ experiences.

Abstract

Nursing, as a gendered occupation, is one that requires vast amounts of emotional labor to be performed. As careworkers, nurses are required to assume multiple roles at work: medical expert, companion, and personal care provider. Roles, or expected behaviors associated with different statuses, have the potential to spillover between work and home environments. The purpose of this chapter is to investigate how nurses perceive their role-taking and emotional labor processes to influence experiences of work–family spillover.

Rooted in interactionist role theory, this investigation seeks to qualitatively examine how nurses assign meaning to their various roles and how they perceive their roles to influence work–family spillover. Using audio diary and interview data, this chapter proposes that nurses who practice role-person merger (Turner, 1978) and empathic role-taking (Shott 1979) will also perceive work–family spillover to be related to their caretaking roles as nurses. Three distinct themes emerged in this qualitative analysis related to how experiences of work–family spillover are influenced by the emotional labor demands of the job and the practice of empathic role-taking by nurses: (1) spillover related to required emotional labor is experienced both positively and negatively; (2) nurses actively exercise personal agency in an attempt to decrease negative spillover; and (3) nurses reported increased work–family spillover when they practiced empathic role-taking.

This analysis extends the literature in this area by demonstrating the connection between the structural influences on emotion, the individual perceptions of roles, and the subsequent experiences of work–family spillover.

Abstract

Purpose

While the main emotional labor strategies are well-documented, the manner in which professionals navigate emotional rules within the workplace and effectively perform emotional labor is less understood. With this contribution, I aim to unveil “the good, the bad and the ugly” of emotional labor as a dynamic theatrical performance.

Methodology/Approach

Focusing on three geriatric long-term care units within a French public hospital, this qualitative study relies on two sets of data (observation and interviews). Deeply rooted within the field of study, the chosen methodological approach substantializes the subtle hues of the emotional experience at work and targets resonance rather than generalization.

Findings

Using the theatrical metaphor, this research underlines the role of space in the practice of emotional labor in a unique way. It identifies the main emotionalized zones or emotional regions (front, back, transitional, mixed) and details their characteristics, before unearthing the nonlinearity and polyphonic quality of emotional labor performance and the versatility needed to that effect. Indeed, this research shows how health-care professionals juggle with the specificities of each region, as well as how space generates both constraints and resources. By combining static and dynamic prisms, diverse instantiations of hybridity and spatial in-betweens, anchored in liminality and trajectories, are revealed.

Originality/Value

This research adds to the current body of literature on the concept of emotional labor by shedding light on its highly dynamic and interactional nature, revealing different levels of porosity between emotional regions and how the characteristics of each type of area can taint others and increase/decrease the occupational health costs of emotional labor. The study also raises questions about the interplay of emotional labor performance with the level of humanization/dehumanization of elderly people. Given the global demographics about an aging population, this gives food for thought at a social level.

Abstract

Purpose

This paper aims to evaluate the literature on emotional labour in the health‐care sector and the benefits and costs of such performance for both the carer and the patient. The aim is to develop a new health care model of emotional labour that has implications for health‐care management in terms of policy and education as well as for future research in this field.

Design/methodology/approach

A new model to explain the antecedents and consequences of emotional labour within a health‐care setting is developed that builds on existing research.

Findings

The model distinguishes between types of emotional conflict to which emotional labour‐inducing events in health‐care settings might lead. The negative and positive consequences, specific to health‐care settings, of emotional labour performance are also outlined.

Practical implications

Emotional labour should be formally recognised as a key skill in facilitating the patient journey, with emotional skills being taught in innovative ways outside the formal classroom setting. Health‐care professionals should be offered training on coping with the effects of emotional labour performance. Finally, more research should be carried out to further develop the model, particularly in identifying causes of emotional labour within health‐care settings and in differentiating the effects that different kinds of emotional labour performance might have.

Originality/value

The paper draws together previous research on emotional labour within health‐care settings to develop a coherent model that can be used to guide future research and practice.
