Abstract

Purpose

A plethora of publications have reported several cytotoxic effects associated with chronic consumption of thermoxidized palm oil. This research aims to investigate the effects of garlic and Aloe vera on liver function and hepatic cytoarchitecture in rats fed thermoxidized palm oil diet.

Design/methodology/approach

Thirty-five male albino Wistar rats weighing 150-180 g were used for this study. They were randomly assigned into five groups (n = 7): control, thermoxidized palm oil diet fed (TPO), TPO plus garlic juice (TPO + G), TPO plus Aloe gel (TPO + A) and TPO plus garlic/Aloe gel (TPO + G + A). The TPO diet was prepared by mixing 15 g of cooled thermoxidized palm oil with 85 g of rat feed. The juice and gel were orally administered at doses of 2.00 ml/kg and 19.12 ml/kg, respectively. After 3 months of feeding and administration, the animals were sacrificed using standard methods and blood collected via cardiac puncture for analysis.

Findings

Serum alanine aminotransferase (ALT), aspartate aminotransferase (AST) and alkaline phosphatase were significantly increased (p < 0.001) in the TPO group compared to the control. This increase was reduced (p < 0.001) in all the treated groups compared to TPO, with the greatest decrease (p < 0.001) seen in TPO + A. Total bilirubin was increased (p < 0.001) in the TPO group compared to the control, whereas there was a significant decrease (p < 0.001) in all the treated groups. Serum proteins and plasma fibrinogen were lowered (p < 0.001) in the TPO group compared to the control but increased progressively in all the treated groups. TPO induced prominent histopathological derangements of the liver tissues. However, there were marked improvements following treatment with garlic and Aloe vera.

Originality/value

The results obtained in this study have revealed that chronic consumption of thermoxidized palm oil is hazardous to health by inducing hepatotoxicity, as seen in increased ALT, AST and total and unconjugated bilirubin and decreased total protein. However, these debilitating effects were seen to be greatly ameliorated following garlic juice and Aloe vera gel administration. If these results are to be extrapolated to humans, then the chronic consumption of thermoxidized palm oil diet should be seriously discouraged.

Abstract

Purpose

Chronic feeding with thermoxidized palm oil causes tissue damage. The purpose of this paper is to ascertain whether chronic feeding of oxidized and fresh palm oil affects biliary secretion.

Design/methodology/approach

Albino Wistar rats were divided into three groups of ten rats each. The first group was the control and were fed on normal rat chow, while the second (FPO) and third group (TPO) were fed diet containing either fresh or thermoxidized palm oil (15per cent (w/w)) for 14 weeks. Biliary secretion, bilirubin, electrolytes, cholesterol and serum levels of alkaline phosphatase, aspartate aminotransferase and alanine aminotransferase enzymes were measured.

Findings

Biliary secretion in TPO was significantly (p < 0.05) lower compared with the control or FPO. Electrolytes (Na+, Cl and HCO3) content of bile were significantly (p < 0.05) lower in TPO compared with control or FPO group. Conjugated and un‐conjugated bilirubin levels were significantly (p < 0.05) elevated in TPO compared with control or FPO. The levels of serum liver enzymes were significantly (p < 0.001) higher in TPO than control or FPO. The cholesterol level in the TPO was significantly higher (p < 0.001) than in control or FPO.

Research limitations/implications

The non‐clinical approach and use of experimental animals present limitations to the study.

Originality/value

These results suggest that long‐term feeding of thermally oxidized palm oil alters bile composition and decreases biliary flow due to its hazardous free radicals.

Abstract

Purpose

This paper aims to evaluate hematological parameters, blood glutathione (GSH), serum glucose-6-phosphate dehydrogenase (G6-PD) and liver function in favism animals' models after oral intake with a mixture of pickled olives and Vicia faba (V. faba) seeds.

Design/methodology/approach

Favism is a life-threatening hemolytic crisis. It results from the ingestion of V. faba by the individuals. The hemoglobin (Hb), hematocrit (Hct), red blood cells (RBCs), white blood cells (WBCs), serum glucose, blood GSH, serum G6-PD, serum thiobarbaturic acid reactive substances (TBARS), liver protein and liver function were evaluated after oral administration with a mixture of pickled olives with V. faba seeds in favism animals' models.

Findings

The favism-, vicine- and convicine-treated animals showed a significant decrease (p < 0.01) in Hb (6.42, 7.23 and 5.96 g/dl), Hct (25.4, 26.4 and 25.1%), RBCs (2.56, 2.45 and 2.60 106 cells/mm3, WBCs (4.35, 4.25 and 4.30 103 cells/mm3), serum glucose (95.8, 97.1 and 96.5 mg/dl), blood GSH (24.7, 26.6 and 23.8 mg/dl), serum G6-PD (15.8, 15.9 and 15.7 U/L), serum aspartate aminotransferase (6.35, 6.59 and 5.97 U/L), alanine aminotransferase (4.49, 4.61 and 4.50 U/L), total protein (6.54, 6.59 and 6.40 g/dl), albumin (3.84, 3.91 and 3.75 g/dl), globulin (2.70, 2.48 and 2.65 g/dl) and liver protein (3.37, 3.10 and 3.42 g/g tissue) but a significant increase (p < 0.01) in serum TBARS (38.7, 38.9 and 39.4 nmol/dl), alkaline phosphatase (275, 271 and 281 U/L) and total bilirubin (0.93, 0.89 and 0.91 mg/dl) compared to Hb (16.3 g/dl), Hct (45.3%), RBCs (5.80 106 cells/mm3), WBCs (9.45 103 cells/mm3), serum glucose (96.5 mg/dl), blood GSH (39.7 mg/dl), serum G6-PD (36.1 U/L), serum TBARS (18.0 nmol/dl), serum aspartate aminotransferase (19.8 U/L), alanine aminotransferase (9.23 U/L), alkaline phosphatase (214 U/L), total bilirubin (0.57 mg/dl), total protein (8.76 g/dl), albumin (4.85 g/dl), globulin (3.91 g/dl) and liver protein (6.28 g/g tissue) in control group. The oral administration with pickled olives + V. faba, pickled olives + vicine and pickled olives + convicine into favism animals' models, vicine-treated animals and convicine-treated animals, respectively pushed all the above-mentioned parameters to near the control levels.

Originality/value

V. faba contains vicine and convicine glycosides. Vicine and convicine glycosides in V. faba are hydrolyzed by intestinal microflora to aglycones divicine and isouramil, respectively. Divicine and isouramil are highly reactive compounds generating free radicals where divicine and isouramil are the main factors of favism. The ß-glucosidase in pickled olives converts both vicine and convicine glycosides into aglycones divicine and isouramil, respectively, in aerobic condition outside the human body (in inactive forms) and prevent these glycosides to cause favism.

Abstract

Purpose

Several audits of clinical practice have confirmed that there is insignificant yield from screening of prolonged jaundice babies with a wide range of tests including thyroid function test (TFT) and full blood count (FBC). This paper aims to audit local practice based on a newly adopted rationalised protocol of selective screening with split bilirubin supplemented with other investigations based on clinical risk assessment.

Design/methodology/approach

The paper uses a retrospective audit of 50 babies referred for prolonged jaundice to a large district hospital in South West England. The authors also designed an easy‐to‐use, user‐friendly software application as a quick aid for implementation of the prolonged jaundice protocol.

Findings

The overall annual incidence of prolonged jaundice was 2.2 per cent of all live births. 32 patients (64 per cent) had only split biliburin test according to the protocol. In total 16 patients (32 per cent) had 18 tests requested unnecessarily including seven liver function test (LFT), eight FBC, one each of glucose, TFT and serum electrolytes. No patient with any serious liver disease was missed.

Practical implications

Appropriate recommendations for optimising the implementation of the current protocol based on the available scientific evidence were introduced.

Originality/value

After two years of implementation of the protocol, it has been found to be cost‐effective and safe. The electronic software provides a useful prompt in guiding the investigation of prolonged jaundice.

Abstract

Purpose

The purpose of this paper is to implement Six Sigma approach to decrease the length of stay (LOS) of neonatal jaundice patients in an Indian government rural hospital situated in northern hill region.

Design/methodology/approach

Six Sigma’s Define–Measure–Analyse–Improve–Control procedure is applied in order to decrease the LOS of neonatal jaundice patients. The mean and standard deviation have been computed as 34.53 and 20.01 h, respectively. The cause and effect diagram is used in the “Analyse” phase of the Six Sigma. The regression analysis and GEMBA observation techniques are used to validate the causes identified through cause and effect diagram.

Findings

The waiting time for registration, waiting time for tests, waiting time for phototherapy and time for discharge implementation are the main factors that are responsible for longer LOS. Based on the identified root causes, some recommendations are suggested to the hospital administration and staff members in order to reduce the LOS.

Research limitations/implications

The present research is limited to provide recommendations to the hospital administration to reduce LOS and it entirely depends upon the implementation of the administration. However, target of administration is to reduce the LOS up to 24 h.

Practical implications

Six Sigma model will reduce bottlenecks in LOS and enhance service quality of hospital. The developed regression model will help the doctors and staff members to assess and control the LOS by controlling and minimising the independent variables.

Social implications

The project will directly provide benefits to society, as LOS will decrease and patients’ satisfaction will automatically increase.

Originality/value

Six Sigma is a developed methodology, but its application in paediatric department is very limited. This is the first ever study of applying Six Sigma for neonatal jaundice patients in India.

Abstract

Purpose

To illustrate the application of chromatic approaches to complexity with two illustrations each involving the interaction of two complex systems biological and polychromatic light.

Design/methodology/approach

Chromatic parameters (H, L, S) were derived from the monitored biological systems plant growth monitored by CCTV with light enhancement by light emitting diodes (LED) and human tissue oxygenation data that may be obtained from optical fibre‐based monitoring. A single chromatic parameter was derived from the H, L, S parameters.

Findings

The derived chromatic parameter CT appears to vary linearly with the aspect being monitored plant growth as indicated by chromatic parameters and blood oxygenation as indicated by chromatic parameters.

Practical implications

Illustrates the possibility of cost‐effective chromatic‐based monitoring and control of complex systems plant growth enhanced by high efficiency/low energy LEDs and human tissue blood oxygenation (TBO) as well as additional applications in advanced pulse oximetry and neonatal bilirubin monitoring.

Originality/value

Illustrates the application of a generic technology in the area of complex systems, where there are interactions between two complex systems, biological and polychromatic light. The chromatic technology supports the derivation of linear relationships for monitoring and control.

Abstract

Purpose

The purpose of this paper is to evaluate safety of the enzymatic treatment (garlic peroxidase) for removing aflatoxin B1 (AFB1) from foods.

Design/methodology/approach

An in vivo study was carried out on male wistar rats for three weeks. Since AFB1 is a hepatotoxic; the effect of AFB1 and the enzymatic detoxification of AFB1 on the liver function was studied by determining the serum activities of various hepatic enzymes in different groups of experimental rats.

Findings

The peroxidase (POD) isolated from garlic possess AFB1 degrading properties and the overall detoxification is safe to be used in various foods/feeds as the reaction products produced no toxicity in the adult male wistar rats. Rats fed with aflatoxin contaminated diet possessed increased activities of all the hepatic enzymes investigated; showing symptoms of liver damage compared to rats fed with enzyme treated diet.

Practical implications

The results of this study can be utilized for commercialization of the garlic POD mediated AFB1 detoxification from feeds/foods.

Originality/value

The aflatoxin detoxyfying abilities of garlic POD and its in vivo safety studies have not been reported so far.

Abstract

Purpose

To introduce chromatic methods and their application to monitoring complex systems.

Design/methodology/approach

Complexity is introduced and the need for holistic monitoring is suggested. The chromatic methodologies are introduced and their broad‐ranging applicability is illustrated with reference to a number of diverse applications.

Findings

The generic chromatic monitoring technology has much to offer as a way of visualising, understanding, monitoring and controlling complex processes and system behaviour.

Practical implications

The technology is leading to new ways of characterising and evaluating aspects of system behaviour, in a holistic and non‐intrusive manner, that are difficult to measure – e.g. walking behaviour of the elderly, tiredness of drivers.

Originality/value

Draws out the generic basis of the chromatic methodology and how it may be applied to a wide range of complex systems and situations to predict when and where human intervention is required.

Abstract

Purpose

Phytochemicals are known for their iron chelation ability with no side effects. Lignans and polyphenols in flaxseed have iron-chelating and antioxidant abilities. Therefore, this study aims to assess the ability of flaxseed to reduce iron overload and minimize its negative health effects.

Design/methodology/approach

A randomized control trial was conducted. A total of 96 patients were enrolled in this study and randomized into three groups: two intervention groups had 3 or 7 g of whole flaxseed and a control group took a placebo (gram seeds). Interventions were administered for 12 weeks. A hematological profile, ferritin quantification in serum, and liver function tests were performed to evaluate the effects of flaxseed.

Findings

It was observed that flaxseed with synthetic iron chelators caused a significant reduction in ferritin levels compared with synthetic chelators. The pre-transfusion hemoglobin level was also improved in the intervention group. A significant reduction (p value 0.0003) in ferritin levels among intervention groups indicated that flaxseed reduced the iron overload. It can be concluded that flaxseed reduced iron overload in thalassemia major patients with no side effects and may reduce iron overload solely when taken in adequate quantity.

Originality/value

Although the study targets a non-specific protein-ferritin for the evaluation of iron-overload, the initial ferritin value and reduction in these levels indicate the efficacy of ferritin for reducing iron overload in thalassemia major patients. Moreover, this is the first study, to the best of the authors’ knowledge, that demonstrates the iron-chelation ability of flaxseed in thalassemia major patients, suggesting the use of flaxseed for iron chelation.
