Abstract

Purpose

The purpose of this study is to examine the acute toxicity and antidiabetic activity of Asystacia gangetica leaf ethanol extract.

Design/methodology/approach

The study was designed as completely randomized in vivo experimental model. Where acute toxicity study was carried out using 30 albino mice, randomly assigned into six groups of five mice each. Toxicity signs and mortality were observed in the rats within a period of 24 h. The acute and sub-acute antidiabetic study was carried out using 50 rats, randomly assigned into five groups of 10 rats each. The rats’ blood glucose levels were determined and used to assess the acute and sub-acute antidiabetic activity of the extract.

Findings

Results obtained from the acute toxicity study indicated no death in any of the study groups, even at 5,000 mg/kg body weight and showed no signs of toxicity. The acute antidiabetic study showed that treatment with 400 mg/kg of the extract significantly (p = 0.01) lowered glucose level in the diabetic rats from 430.6 to 177.4 mg/dl while 800 mg/kg brought down glucose level from 370 to 144.2 mg/dl by the end of 6 h following administration when compared with the diabetic control group. It was observed that the effect of the extract mostly at 800 mg/kg also compared favorably with that of the standard drug (glibenclamide), which lowered glucose level in diabetic rats from 374.2 to 176.4 mg/dl. Furthermore, the significant reduction was evident from 4, 2 and 2 h for 400 mg/kg extract, 800 mg/kg extract and 5 mg/kg glibenclamide, respectively. At sub-acute level the blood glucose was lowered from 155.6 to 127.2 mg/dl, 137 to 124.4 mg/dl and 151.8 to 121.8 mg/dl for diabetic rats treated with 400 mg/kg, 800 mg/kg and 5 mg/kg glibenclamide, respectively, when compared to the diabetic untreated rats, which ranged from 417.6 to 358.6 mg/dl. The biochemical profile, lipid profile and hematological examination were all positively restored near to normal with the herbal treatment at the different doses. At histopathology level, the liver of the rats treated with 400 mg/kg had moderate portal inflammation without interface or lobular hepatitis while that of 800 mg/kg showed severe portal inflammation with the interface and lobular hepatitis with extensive confluents necrosis. The pancreatic cells of the treated rat showed no significant difference in the β-cells of the islets of Langerhans with hyperplasia of the acinar cell when compared to the diabetic untreated.

Research limitations/implications

The record of no death and signs of toxicity implies that the extract is safe for consumption even at a high dosage of 5,000 mg/kg body weight. The significant (p = 0.01) reduction in the plasma glucose level of the treated rats as compared to the control is an indication of blood glucose-lowering potential of the extract at two different doses. The significant reduction evident of the extract at different hours and days for the two doses implies that the extract rate of lowering potentials is dose-dependent. The evidence of moderate-severe portal inflammation with the interface and lobular hepatitis at 800 mg/kg treatment is an indication that the intake of this herb at high dosage for long period is not safe for the liver tissue.

Practical implications

The outcome of this study suggested that the Asystacia gangetica should also be used as a vegetable in-home food preparation and food processing to use its antidiabetic effect. The herbal extract could also be incorporated into a food product and processed into herbal tea bag for convenient. The subjection of this herbal plant to heat treatment during processing could be a possible avenue to make it safe.

Social implications

The economic burden and complications of diabetes mellitus management will be reduced if the practical implication of this research finding is implemented in foods as vegetable and in functional food production.

Originality/value

This study revealed that Asystacia gangetica leaf extract may be safe and effective for use at a low dose for acute management of diabetes mellitus. This research may be of value to those living with diabetes mellitus.

Abstract

Purpose

The purpose of this paper is to systematically review the functionalities, safety regulations and product applications of herb Stevia rebaudiana extract. This plant material is embedded with multiple functionalities such as antioxidant, antidiabetics, anti-inflammation and antimicrobial. The regulations released from global authorities are covered to ensure the safety premise of stevia. Besides, the product applications of the extract of aerial parts of the herb S. rebaudiana helps us to recognize its value from commercial side.

Design/methodology/approach

Relevant literatures are selected and obtained from main scientific databases such as Google Scholar, Web of Science, PubMed and trade magazines published between 2000 and 2023. The keywords and their possible combinations such as sweetening, antioxidant, antidiabetics, anti-inflammation, safety and product development were used to ensure the preciseness and completeness of literature searching. Major data such as sweetness, total phenolic content and dose together with latter critical conclusions from searched publications were appropriately used and discussed. In this review, approximately 150 scientific literatures were meticulously ordered and analyzed. In applications, it is the first time that sentiment analysis was used to obtain a market assessment of the stevia-containing products.

Findings

This review paper helps rearrange the scientific affairs of those stevia extract’s functions like sweetening, antioxidant, antidiabetics and inflammation. Sweetness indexes of steviol glycosides were summarized together for comparison while various in vitro and in vivo approaches were reviewed to quantify those functions’ capacities and to depict the related mechanism. The regulation of steviol glycoside compounds such as rebaudioside A was established by global authorities such as US Food and Drug Administration and Joint FAO/World Health Organization Expert Committee to ensure the safety endorsement before commercialization. Then, this study discussed about the market performance of stevia ingredients or products with the self-developed data analytics. This study also investigated the product development progress of stevia-containing food products in the categories of beverage, bakery, dairy and confectionery. Those stevia-containing food consumer goods can be acceptable by certain consumers.

Originality/value

This review paper precisely presents the evidential information about the stevia’s multiple functionalities with mechanisms and global regulation milestones. To the best of the authors’ knowledge, it is then the first time to probe the stevia-containing products’ market performance through data analytics.

Purpose

In the Indian system of medicine – Ayurveda, Musa paradisiaca has been mentioned as a remedy for various diseases and ailments. Based on the folkloric use, the purpose of this paper is to verify and compare the hypoglycemic potential of unripe, ripe and overripe fruit extract of Musa paradisiaca.

Design/methodology/approach

Hypoglycemic activity of fruit extracts has been evaluated using various in vitro methods, namely, determination of glucose adsorption capacity, glucose uptake in yeast cells, amylolysis kinetics and glucose diffusion.

Findings

The extracts of unripe, ripe and overripe fruits of Musa paradisiaca adsorbed glucose, and the adsorption of glucose increased remarkably with an increase in glucose concentration. In the amylolysis kinetic experimental model, the rate of glucose diffusion was found to increase with time, and all the extracts of unripe, ripe and overripe fruits of Musa paradisiaca demonstrated significant inhibitory effects on the movement of glucose into external solution across the dialysis membrane as compared to the control. The extracts under study also promoted glucose uptake by the yeast cells in all the five glucose concentrations used in the study.

Practical implications

Here, the authors have verified and compared the hypoglycemic potential of Musa paradisiaca, its unripe fruit extract was found to show a better activity than ripe and overripe fruit extracts.

Originality/value

Banana, being an all season readily available fruit, is widely consumed due to its ready availability and low cost. It acts as a complete food for even low socio-economic classes of society, owing to its rich nutritional values. Even in a processed and unprocessed manner, it is an important constituent of diet. The research suggests that instead of consuming ripe and overripe fruit, the unripe fruit will help in management of diabetes.

Abstract

Purpose

Research projects involving traditional knowledge are finding new ways of dealing with intellectual property rights and commercialisation. Influenced by calls for fair and equitable protocols involving access and benefit sharing regimes, researchers are developing new standards of practice. Here this paper aims to explore the process by which the CIHR Team in Aboriginal Antidiabetic Medicine (TAAM) came to address these issues within the scope of participatory action research.

Design/methodology/approach

A case study method is applied in order to highlight key events and topics. The legally binding research agreement developed for this project is used to illustrate examples of how the needs of First Nations stakeholders and of researchers are met.

Findings

The paper finds that strong research partnerships are characterized by accountability, adaptability, transparency, good and frequent communication and ultimately, trust. Researchers should be prepared to take a more “human” approach in their studies as the establishment of personal relationships are as important as the research itself. Proposals should include both monetary and intangible outcomes where possible, which reflect aboriginal culture and decision.

Practical implications

This paper can help others to understand the needs of aboriginal peoples with regard to research. It also provides links to protocols and the legal research agreement used by TAAM that can serve as an adaptable template for future work.

Originality/value

Publicising the research agreement and experiences herein is meant to contribute to a body of knowledge that will one day lead to new research norms when dealing with aboriginal peoples and traditional knowledge.

Abstract

Purpose

Diabetes is a chronic metabolic disease that accounted for 6 per cent of total Indonesian deaths in 2016. The purpose of this study is to evaluate the ability of milk fermented using intestinal origin Lactobacillus casei strains to alleviate hyperglycemia in rats.

Design/methodology/approach

L. casei strains AP and AG were previously isolated and identified from feces of breastfed Indonesian infants (<1 month-old) and confirmed to be probiotics in vitro. These strains were used as bacterial starters to ferment milk, and the fermented product was fed to 25 diabetic rats for 15 days; metformin was used as a positive control. Blood glucose concentration, lipid profiles and total lactic acid bacteria counts were analyzed before and after treatment.

Findings

The results showed that feeding rats with milk fermented using L. casei strain AP decreased blood glucose concentration from 172.4 ± 2.1 to 147.2 ± 6.0 mg/dL (p < 0.05), whereas metformin reduced blood glucose concentration from 173.5 ± 6.5 to 124.2 ± 16.9 mg/dL (p < 0.05). Feeding milk fermented either L. casei AP or AG decreased low density lipoprotein but increased high density lipoprotein levels (p < 0.05).

Research limitations/implications

This research finding is expected to increase the development of fermented dairy products as functional food, solutions for the prevention of hyperglycemia diseases which have a higher risk to occur on humans.

Practical implications

Further study is needed to examine the effect of L. casei AP-fermented milk on humans with hyperglycemia.

Social implications

This research finding is expected to increase the development of fermented dairy products as functional food.

Originality/value

Oral administration of L. casei AP-fermented milk reduced hyperglycemia in rats similar to metformin. Further study is needed to examine the effect of L. casei AP-fermented milk on humans with hyperglycemia.

There is a statutory obligation for pharmaceutical companies to record reports of adverse reactions to their drugs. The storage and retrieval of these reports, within an operating post‐coordinate information retrieval system poses a number of problems due to their multidimensional nature. Abstract

Purpose

The downstream insulin signaling, such as phosphatidylinositol-3-kinase (PI3K)/protein kinase B (Akt) pathway, is an important step for skeletal glucose disposal through the translocation of glucose transporter (GLUT)-4. In addition, the master of energy regulator adenosine monophosphate-activated kinase (AMPK) is also involved in GLUT-4 translocation, independent from the PI3K/Akt pathway. Fermented cassava tuber or gatot is a traditional food from Indonesia with antihyperglycemic properties. However, the molecular mechanism leading to this effect is unclear. Therefore, this paper aims to evaluate whether the antidiabetic activity of gatot is through PI3K/Akt dependent or AMPK pathway.

Design/methodology/approach

Diabetes mellitus was induced in 20 male Wistar rats by intraperitoneal injection of 65 mg/kg body weight streptozotocin and 230 mg/kg body weight nicotinamide. Diabetic rats were randomly allocated into four groups; negative control, positive control (metformin 100 mg/kg body weight), fermented cassava diet replacing 50% of carbohydrate (FC-50) and 100% of carbohydrate (FC-100) in the diet. Serum glucose, insulin and lipid profile were analyzed before and after four weeks of intervention. Genes expression of PI3K subunit alpha, PI3K subunit beta, PI3K regulatory subunit, Akt and AMPK were analyzed using real time polymerase chain reaction (PCR). GLUT-4 protein expression was performed using immunohistochemistry.

Findings

There is a significant difference (p = 0.000) in serum glucose, insulin, total cholesterol, triglyceride, high density lipoprotein (HDL)-cholesterol and LDL-cholesterol between groups. Skeletal AMPK gene expression was higher and significantly different between FC-100 (p = 0.006) and healthy control groups. No significant difference was observed in the messenger ribonucleic acid (mRNA) expression of the PI3K/Akt pathway among groups. GLUT-4 expression was highly expressed in a positive control group followed by FC-100.

Research limitations/implications

This paper did not characterize the bioactive component that is responsible for increasing mRNA expression of AMPK. This paper also did not analyze the phosphorylation of PI3K/Akt and AMPK that are important in activating the protein.

Originality/value

To the best of the authors’ knowledge, this is the first study that showed the antidiabetic activity of traditional fermented food is through AMPK-dependent activity.

Abstract

Purpose

Prader-Willi syndrome (PWS) is a genetic disorder characterised by excessive appetite and progressive obesity. The causes of hyperphagia are unknown and this has implications on treatment limiting the options available. The purpose of this paper is to systematically synthesise the latest evidence regarding all causes as well as management of hyperphagia in people with PWS.

Design/methodology/approach

A computer-based literature search, citation tracking and related articles search was undertaken. Primary research studies using genetically confirmed Prader-Willi participants specifically focused on the causes and treatment of overeating was included. Studies were described, critically analysed, presented and discussed in the review.

Findings

In total, 18 relevant studies were identified. Nine studies were included in the causes of hyperphagia and focused largely on the roles of secretory hormones, and reward-related areas of the brain. Nine studies were included in the management of hyperphagia and related to physical exercise and food control, antidiabetic agents and weight loss surgery.

Originality/value

The causes and the management of hyperphagia are still to be determined despite promising findings. The small number and heterogeneity of the included studies and participants limits conclusions. There is the need for future research to use larger systematic studies to validate findings so far.
