Abstract
Purpose
The purpose of this paper was to synthesis all primary evidence relevant to the anti-diabetic effect of dandelion. Dandelion leaf and root have been used extensively for its medicinal and health benefits since hundreds of years ago. This systematic review was conducted to gather scientific evidence that are available with regards to the anti-diabetic effect of dandelion leaf and root.
Design/methodology/approach
A systematic search was conducted using PubMed, BioMed, PLUSONE and Cochrane databases between June 6, 2013 and June 30, 2013. Manual search was also done on books and journals in the KNUST library and its electronic database for possible documented effects of dandelion leaf or root on diabetic patients. Key words “dandelion”, “Taraxacum”, “dandelion and diabetes”, “Taraxacum officinale”, “Taraxacum and diabetes”, “dandelion and hypoglycemia” and “dandelion and hyperglycemia” were used in the search.
Findings
The search yielded 713 papers, and after the removal of duplicates and papers not relevant to this review, 20 papers were accepted for the review. These included studies conducted in humans and animals (rats and mice). Among the 20 studies reviewed, only 1 study examined and reported a positive hypoglycemic effect of dandelion on diabetic rats.
Research limitations/implications
The review only considered published papers and might have left out some unpublished research works.
Practical implications
The results of this review suggest paucity of data available on the use of dandelion in the treatment/management of diabetes. There is the need for well-designed clinical trials to ascertain the anti-diabetic effect of dandelion.
Social implications
The consumption of dandelion by type 2 diabetic patients to treat or manage their blood glucose has not been clinically proven to be effective, as shown by the review.
Originality/value
The paper provides a clear picture of the evidence available in the use of dandelion as an anti-diabetic herb, and this provides some preliminary data for the conduct of a clinical research on it.

Abstract
Purpose
The purpose of this paper is to investigate the health and medicinal importance of bush tea (Athrixia phylicoides DC) and special tea (Monsonia burkeana Planch. ex Harv), two of Southern African indigenous herbal teas.
Design/methodology/approach
The two herbal teas, A. phylicoides and M. burkeana were extracted individually and in combined ratios for analysis. The phenolic content was determined and the different phenolic compounds were identified using thin-layer chromatography (TLC) and high-performance liquid chromatography (HPLC). The anti-diabetic activity of the teas was determined by evaluating the inhibition of both α-amylase and α-glucosidase in vitro. The anti-proliferative activity was measured on human cervical cancer (HeLa) cell line using the MTT (3-(4,5-dimethylthiazol-2-yl)2,5-diphenyltetrazolium) assay.
Findings
Gallic acid, chlorogenic acid and quercetin were identified to be present in significant quantities by TLC. The HPLC quantified the presence of catechin (1.567 mg/g) and chlorogenic acid (1.862 mg/g) in special tea while chlorogenic acid (1.288 mg/g) was present in bush tea. Bush tea and special tea expressed significant levels of phenolic content and high antioxidant activities. Special tea (S100) expressed high inhibition of α-amylase, α-glucosidase and HeLa cell line proliferation when compared to bush tea (B100).
Originality/value
Both bush tea and special tea could provide an alternative for treatment and management of both diabetes and cervical cancer. However, future studies are needed to investigate their synergistic effect with a wide range of other commercial herbal teas.

Abstract
Purpose
The purpose of this paper is to evaluate the inhibition activity of Tribulus terrestris L. (T. terrestris) fruits extracts with solvents of increasing polarity against α-glucosidase and α-amylase, and to determine the inhibition mode of the most effective extract against both enzymes.
Design/methodology/approach
Hexane, acetone, ethanol and aqueous extracts of T. terrestris fruits were prepared using ultrasonic sequential extraction and analyzed for their α-amylase and α-glucosidase inhibitory activities by specific assay for each enzyme. The modes of inhibitions were detected using Lineweaver–Burk plots.
Findings
T. terrestris fruits extracts showed inhibition activity against α-glucosidase and α-amylase which was in the dose-dependent manner. Hexane extract had the highest α-glucosidase inhibition activity (IC50 = 27.28 μg/ml, p = 0.003), followed by acetone and ethanol extracts (IC50 = 60.58 μg/ml and IC50 = 84.21 μg/ml, respectively). The inhibition mode of hexane extract was noncompetitive. While acetone extract showed the highest inhibition activity against α-amylase (IC50 = 6.18 mg/ml, p = 0.002), hexane and ethanol extracts showed no significant difference (IC50 = 13.04 mg/ml and IC50 = 14.20 mg/ml, respectively, p = 0.09). The inhibition mode of acetone extract was competitive.
Originality/value
T. terrestris fruits extracts had strong inhibition activity against α-glucosidase and α-amylase, and they can be used as a promising anti-diabetic agent.

Abstract
Purpose
This paper aims to increase understanding of how firms can more effectively identify valuable and profitable innovations in the pharmaceutical industry and to identify the issues and challenges posed by current managerial decision-making practices.
Design/methodology/approach
A case study of a single project is presented: a drug in-licensing decision made by a team of three managers in a large Australian pharmaceutical firm. Using participant-observation, interviews and archival analysis, the authors followed the managers as they identified and evaluated 122 late-stage anti-diabetic drug variants for further development.
Findings
The managers used decision heuristics to arrive at a short list of three drugs from a choice set of 122. While the process was ostensibly rational and systematic, there was evidence of data quality issues, misleading mental models and cognitive bias. The authors concluded a high probability of accepting a poor candidate or rejecting a stronger candidate (i.e. making Type I and II errors).
Research limitations/implications
This paper focuses on initial market and technology evaluation stage only (i.e. not commercialization) and is a single case study design; therefore, care should be taken in generalizing to other decisions or other contexts. This paper highlights the need for further research integrating organizational decision-making and open innovation from a multi-disciplinary perspective.
Practical/implications
This paper raises awareness of potential decision-making pitfalls and includes a detailed audit framework to support improved managerial decision processes and double rather than single loop learning.
Social/implications
The findings support better decision-making and therefore supports higher quality drug selection and development, leading to improved population health outcomes.
Originality/value
Multi-disciplinary, draws attention of marketing and new product development scholars to open innovation research. It adds to knowledge about open innovation practices at the project level. It also provides an extended model of market opportunity analysis for high technology markets.

Purpose
Pyrus Pyrifolia (Sand Pear) is one of the most underused pear variety despite its nutraceutical potential. Therefore, this paper aims to explore the Pyrus Pyrifolia in term of origin, distribution and classification, nutritional and bioactive potential, therapeutic potential and valorization along with future prospectus.
Design/methodology/approach
A wide variety of publications (88) were identified through electronic databases (Science direct, PubMed, SciELO, Google scholar, Link springer and Research gate) under the umbrella of different keywords such as bioactive compounds, health benefits, nutrition, sand pear, Pyrus and Pyrus pyrifolia.
Findings
Pyrus Pyrifolia (Sand Pear) is abundant in nutritional and bioactive compounds such as phenolic acids, flavonoids, terpenoids, vitamins and minerals. It exhibits therapeutic potential as being an antioxidant, anti-obesity, anti-diabetic, anti-inflammatory and anti-cancer agent. However, P. pyrifolia is not much explored by food researchers and industrialists, hence remaining underused. A few attempts have been made toward the use of P. pyrifolia for jam, jelly, candy and wine preparation. However, more research is required for the commercial processing of P. pyrifolia and to enhance its availability outside its growing area.
Originality/value
In this paper, nutritional and bioactive compounds of P. pyrifolia are discussed that provide knowledge to the researchers for its use as a functional ingredient.

Abstract
Purpose
Diabetes is one of the most prevailed chronic diseases in the world. Pro-inflammatory cytokines play a key role in the type 2 diabetes mellitus. Pomegranate seed oil (PSO) has potential anti-inflammatory properties. The purpose of this study is to evaluate the antidiabetic effects of the use of PSO on the expression of peroxisome proliferator-activated receptor-gamma (PPAR-γ), pro-inflammatory biomarkers and lipid profile levels in obese type 2 diabetic patients.
Design/methodology/approach
In total, 52 patients were randomly assigned to the PSO (n = 26) and placebo (n = 26) groups. Subjects received daily PSO 3 g placebo (paraffin) in 1 g soft-gel capsules (along with breakfast, lunch and dinner meals) for eight weeks.
Findings
Serum levels of fasting blood sugar (FBS) decreased from 161.46 ± 34.44 to 143.50 ± 24.2 mg/dL (p = 0.008), IL-6 decreased from 5.17 ± 2.25 to 4.52 ± 1.90 (p = 0.049) and tumor necrosis factor alpha (TNF-α) significantly decreased from 9.17 ± 4.13 to 7.74 ± 2.44 pmol/mL in PSO group (p = 0.030). However, changes in the expression of PPAR-γ gene, serum levels of hs-CRP and lipid profile levels were not significant.
Research limitations/implications
Lack of PSO concentration measurements and the short duration of the study were the key limitations. Future randomized clinical trials with a longer period of follow-up are needed to assess the potential anti-diabetic effects of PSO.
Originality/value
Administration of PSO in obese type 2 diabetic patients reduced the levels of FBS, interleukin 6 and TNF-α; nevertheless, changes in the insulin, lipid profiles and hs-CRP were not significant.

Purpose – To estimate the impact of diabetic drug adherence on hospitalizations, emergency room (ER) visits, and hospital costs.
Methods – It is often difficult to measure the impact of drug adherence on hospitalizations since both adherence and hospitalizations may be correlated with unobservable patient severity. We control for such unobservables using propensity score methods and instrumental variables for adherence such as drug coinsurance levels and direct-to-consumer advertising.
Findings – We find a significant bias due to unobservable severity in that patients with more severe health are more apt to comply with medications. Thus, the relationship between adherence and hospitalization will be underestimated if one does not control for unobservable severity. Overall, we find that increasing diabetic drug adherence from 50% to 100% reduces the hospitalization rate by 23.3% from 15% to 11.5%. ER visits reduce by 46.2% from 17.3% to 9.3%. Although such an increase in adherence increases diabetic drug spending by $776 a year per diabetic, the cost savings for averted hospitalizations and ER visits are $886 per diabetic, a cost offset of $1.14 per $1.00 spent on diabetic drugs.
Originality – Most of the drug cost-offset literature focuses only on the impact of cost-sharing and drug spending on cost-offsets, making it impossible to back-out the empirical impact of actual drug adherence on cost-offsets. In this chapter, we estimate the direct impact of adherence on hospitalizations and costs.

Purpose
Jamun is an underutilised fruit crop of India whose utilization needs to be considerably increased. Despite its impressive nutritional profile and several health benefits, its cultivation is still very limited. Hence, this study aims to highlight the essential nutrients, health benefits and value-added products of jamun fruit and seed.
Design/methodology/approach
Major sources of bibliometric information such as Web of Science, Scopus, PubMed and Google Scholar were extensively searched with keywords such as nutritional composition of jamun, bioactive compounds, health benefits and jamun-based food products to obtain a database of 317 papers. Thirty four publications met the criteria for review.
Findings
Jamun fruit (Syzygium cumini L.) is known for its attractive colour, astringent taste, enormous nutrients and nutraceutical properties; nevertheless, this fruit is seasonal, perishable and underutilised. Furthermore, the fruit is used for the treatment of diabetes and also possess anti-cancer, anti-inflammatory, anti-obesity and cardio-protection properties. Investigations were done on the production of value-added food products such as read-to-serve beverages, jam, cookies and cake from jamun due to their several health benefits. The present review was an attempt to provide spotlights on the health benefits and food product applications of jamun fruit and its by-products.
Originality/value
This review systematically collates evidence from various published sources regarding the nutritional profile, development of food products from jamun and their health benefits. The compiled information will help researchers and professional become aware of the significance of jamun fruit and thereby enhances its processing and applications in food systems.

This article focuses on some of the reported medicinal values of bitter melon, an exotic vegetable forming part of the cuisine of various countries.
Design/methodology/approach
Available data in the literature strongly support the anti‐hypoglycaemic properties of Bitter melon. A literature view was carried out and the data were used to validate our findings regarding the effects of extracts of Momordica charantia on uptake of sugar.
Findings
Our work adds on to the growing evidence of the anti‐diabetic properties of bitter melon.
Practical implications
In view of evidence reported in the literature regarding the medicinal properties of vegetables, the importance of vegetables in a well balanced diet should therefore be strongly advocated.
Originality/value
This paper adds on to the evidence of the use of Momordica charantia as a medicinal plant in traditional medicine in the control of blood sugar level.
