Abstract

This paper examines the risk factors of medical expense in China and applies statistical models to analyze these factors. Adopting data from social and private health insurance, this paper discusses the application of some multi‐variable statistical models in analyzing the risk factors of medical service utilization and medical expense. This study concludes that while the medical service utilization rate mainly depends on some hazard characteristics of the insured, the main risk factors of medical expense come from doctors and hospitals. The study also shows that an analysis of risk factors is useful for the risk control in health insurance, but the statistical models should be used properly according to the type of data.

Abstract

The institution of food and cookery exhibitions and the dissemination of practical knowledge with respect to cookery by means of lectures and demonstrations are excellent things in their way. But while it is important that better and more scientific attention should be generally given to the preparation of food for the table, it must be admitted to be at least equally important to insure that the food before it comes into the hands of the expert cook shall be free from adulteration, and as far as possible from impurity,—that it should be, in fact, of the quality expected. Protection up to a certain point and in certain directions is afforded to the consumer by penal enactments, and hitherto the general public have been disposed to believe that those enactments are in their nature and in their application such as to guarantee a fairly general supply of articles of tolerable quality. The adulteration laws, however, while absolutely necessary for the purpose of holding many forms of fraud in check, and particularly for keeping them within certain bounds, cannot afford any guarantees of superior, or even of good, quality. Except in rare instances, even those who control the supply of articles of food to large public and private establishments fail to take steps to assure themselves that the nature and quality of the goods supplied to them are what they are represented to be. The sophisticator and adulterator are always with us. The temptations to undersell and to misrepresent seem to be so strong that firms and individuals from whom far better things might reasonably be expected fall away from the right path with deplorable facility, and seek to save themselves, should they by chance be brought to book, by forms of quibbling and wriggling which are in themselves sufficient to show the moral rottenness which can be brought about by an insatiable lust for gain. There is, unfortunately, cheating to be met with at every turn, and it behoves at least those who control the purchase and the cooking of food on the large scale to do what they can to insure the supply to them of articles which have not been tampered with, and which are in all respects of proper quality, both by insisting on being furnished with sufficiently authoritative guarantees by the vendors, and by themselves causing the application of reasonably frequent scientific checks upon the quality of the goods.

Abstract

Month after month we bring forward additional evidence of the injury resulting from the use of chemical “preservatives” in food, while the Authorities feebly hesitate to give specific legal effect to the recommendations of the Departmental Committee which made such a complete inquiry into this question. The evidence upon which those recommendations were based has been fully corroborated by a number of different observers. FERE and others have shown that, as regards boric acid and borax, even when administered in the smallest medicinal doses, there is always the risk that these drugs may aggravate, or even produce, renal diseases. These observations have been confirmed by the work of Dr. CHARLES HARRINGTON, an account of which has been recently published. Twelve cats were fed on the same food; six were treated with borax, one had no preservative, and five were given a preservative which had no apparent effect. The experiment extended over a period of 133 days, the quantity of borax given averaging about 0.5 grms, per diem. Three of the borated cats soon became ill, and one died at the end of six weeks. On the termination of the experiment the cats were all killed, and upon examination it was found that the organs of the six cats which had not taken borax were in perfectly sound and healthy condition, while the others, with one exception, were all suffering from nephritis. Of course, instances are recorded in which patients have been treated with borax and boracic acid with apparently no injurious result, but as a general rule these experiments have been of too short duration to allow of the desired information being arrived at, and the results must therefore be regarded as inconclusive and unreliable. It is perfectly evident that the kidneys may be for a short time quite capable of eliminating many objectionable substances, but the long‐continued use of such bodies, as Dr. HARRINGTON'S researches clearly indicate, sets up an inflammatory condition of the kidneys which, of course, interferes with the effective performance of their proper functions, and lays the foundations for complications of the most serious nature.

THE Conference of the Library Association may be described as one without a press. The greatest dailies had the barest references to it, a fact which is surprising and lends us matter for reflection. If an admittedly national service, almost universal in application, can be completely ignored in its annual gatherings, what is to be thought? Is it that libraries are now so normal a part of the social landscape that they may be taken for granted? Are they so insignificant that they do not merit notice? Alternatively, were our proceedings too dull for the dramatic necessities of the reporter? Or, finally, was it because the general publicity of the L.A. is not aggressive, is indeed inert? These questions every librarian and library authority may ask and have a right to the answer.

In a recent article dealing with the crimes against humanity committed by Germany, The Daily Telegraph remarks that thousands of innocent men, women, and little children murdered in cold blood by airship and submarine appeal for vengeance. The acts of Germany from the early days of the war onwards have filled decent‐minded people with feelings of loathing, and it is well that the last bonds uniting the two nations should be severed. This is no ordinary war. It has cut a deep chasm between the British and German peoples. By every means in our power we must remove, root and branch, those enemy influences in our midst which, by a process of “peaceful penetration,” were undermining our social, financial, and industrial power.

Abstract

The article explores the drivers for legislative and policy change in children's social care in England over the past 60 years. It describes the factors that led to the major children's social care legislation and explores how these ‘drivers for change’ varied in their importance over time. Particular attention is given to the impact of research evidence as a driver for change among, for example tragedy and media scandal, political developments and changes in practice prompting legislative reform. The article also notes how research has at times provided a background for change while not explicitly promoting the change itself. The use of performance information and research in shaping and monitoring change is seen to have increased in the past 30 years, but with continuing tensions between a natural and necessary research timescale and the political wish for quick and clear answers to pressing issues.

Abstract

Pre‐employment medical examinations with appropriate testing are required in many industries—a basic tenet of Occupational Medicine—and it has long been a recommendation of many in community medicine and environmental health for those food handlers whose close contact with open food, aspects of its preparation, processing, sale, exposure for sale, make their personal health important and in prevention of diseases and may constitute a health hazard to food consumers. Epidemiological studies have revealed too many instances of a human source of disease, especially in milk and water, for this to be denied or under‐estimated. Food poisioning outbreaks caused by a carrier, of chronic or limited duration, enable those investigating such outbreaks to see there could be advantages in medical screening of certain employees especially in certain areas of food trades. The main problem is to decide the extent of the discipline and who should be subject to it. The fact that by far the majority of the examinations and tests will prove negative should not be seen as removing the need for the service. After all, there are a number of similar circumstances in public health. Meat inspection, for example, in which a 100% inspection of all food animals slaughtered for human food is now fully established, it is not suggested that inspections should in any way be reduced despite the fact that a number of the diseases, eg., tuberculosis, no longer occurs as it once did, which was the prime cause of meat inspection being brought into being. Other areas where routine medical examinations reveal satisfactory health with only a few isolated cases requiring attention, is the school medical service. Here, the “de‐bunkers” have had some success, but if children are not regularly examined at vulnerable age levels and especially in between where the occasion demands, there is no question that much will be missed and ill‐health progress to a chronic state.
